Beliefs and habits: staff experiences with key word signing in special schools and group residential homes.

Even though use of aided augmentative and alternative communication (AAC) by staff has been extensively researched, few studies relate to unaided AAC strategies such as key word signing (KWS). We explored the KWS views of two groups: direct support staff in group residential homes and teachers from special education secondary schools. We examined transcripts from individual semi-structured interviews with five direct support staff and five teachers using thematic analysis. Participants discussed consistency of KWS use and reasons for implementing KWS. Compared to direct support staff, teachers

Smart homes and ambient assisted living applications: from data to knowledge-empowering or overwhelming older adults? Contribution of the IMIA Smart Homes and Ambiant Assisted Living Working Group.

OBJECTIVES: As health care systems face limited resources and workforce shortages to address the complex needs of older adult populations, innovative approaches utilizing information technology can support aging. Smart Home and Ambient Assisted Living (SHAAL) systems utilize advanced and ubiquitous technologies including sensors and other devices that are integrated in the residential infrastructure or wearable, to capture data describing activities of daily living and health related events. This paper highlights how data from SHAAL systems can lead to information and knowledge that ultimately

Planning Dementia-Capable Group Homes for Adults With Intellectual Disability: Insights From a 15-Year Longitudinal Study.

Adults with intellectual disability (ID) experience elevated risk of developing dementia, including earlier onset in some conditions such as Down syndrome. As increasing numbers of adults with ID reach older age, community service systems are confronted with the need to adapt residential supports originally designed for lifelong developmental assistance to accommodate progressive neurodegenerative conditions. One emerging response has been the development of small, community-based dementia-capable group homes that integrate specialized care practices within familiar neighborhood environments.

Advance care planning in German nursing homes from the perspective of the facilitators: A focus group study.

BACKGROUND: Since 2018, German nursing homes have been able to reimburse advance care planning (ACP) at the expense of statutory health insurance. ACP is a consultation for end-of-life care in which care preferences can be documented. The consultation is conducted by facilitators, who have completed the required training. However, limited research exists on how the ACP consultation processes are realized. Hence, this focus group study, as a part of the "Gut-Leben" research project, investigated its implementation. METHODS: Twenty-four ACP facilitators participated in four semi-structured focu

Specialized group homes for persons with severe or profound mental retardation and serious problem behaviour in England.

Thirteen people with severe or profound mental retardation identified by their local service agencies as presenting the most serious problem behaviour were transferred from institutional care to specialized group homes. A multiple time-series design and direct observation were used to evaluate whether there was any change in their level of participation in meaningful activity; social interaction and problem behaviour were also observed. Participation increased significantly on transfer and included a wider range of activities. The majority of participants did not increase their major problem b

Barriers and enablers of elderly care specialists towards deprescribing practices in the nursing homes: a focus group study.

INTRODUCTION: Polypharmacy increases as patients age. It is associated with a decline in quality of life and an increased risk of potentially inappropriate medications (PIMs). These are highly prevalent among long-term care residents in European countries. The potential benefits of deprescribing, as the process of withdrawing PIM’s are widespread, including improvements in both health and quality of life for patients. An overview of the barriers and enablers of deprescribing is still lacking in national research or policy discussions involving a larger group of Elderly Care specialist (ECS), w

Home parenteral nutrition in adults: a european multicentre survey in 1997. ESPEN-Home Artificial Nutrition Working Group.

A retrospective survey on home parenteral nutrition (HPN) in Europe was performed from January to December 1997. Data were compared to a similar study performed in 1993. A questionnaire of HPN practice was designed by the members of the ESPEN-HAN group. This involved adult patients (older than 16 years) newly registered in an HPN program between 1 January and 31 December 1997 and included: number of patients, underlying diseases and a 6-12 month outcome. Incidence and prevalence (at 1.1.1998) of adult HPN were calculated according to the estimated total population in 1997 for the countries in

Identification of strategies to optimize pain management in people with dementia living in nursing homes using nominal group technique.

INTRODUCTION: Pain management for people with dementia is widely recognized as a challenge for healthcare personnel. Multifactorial and comprehensive person-centered care models that recognize individual abilities and resources are warranted but remain lacking in nursing homes. This study aimed to provide a comprehensive perspective on how pain management for people with dementia living in nursing homes can be conducted in a way that includes and supports their individual resources. METHODS: We conducted eight group interviews with a total of 28 nurses providing care for people with dementia

Using a simplified questionnaire from the NADYA group, data referring to age, sex, diagnosis, access route, duration, form of administration, complications, and quality of life have been gathered from 1,400 patients (57% male, 43% female) who receive home enteral nutrition, and from 38 patients (20% male and 18% female) who receive home parenteral nutrition. All of these patients come from the 1996 national registry. The most common indication for home enteral nutrition are neoplasias (39%) followed by neurological alterations (33%). The most common access route is oral (48%), followed by a na

Ongoing consultation as a method of improving performance of staff members in a group home.

A model of ongoing consultation was implemented in a community group home for 8 adults with severe and profound mental retardation. Two consultants, highly experienced in working with people with mental retardation and in the procedures used in group homes, taught staff members to use a token reinforcement system, to engage the adults in a variety of activities, and to improve the content and style of the staff members' interactions with the adults. The consultants taught skills to 9 staff members through brief mini-workshops, direct observation of the staff members' use of the skills during r

Lessons learned from social isolation in nursing homes during the COVID-19 epidemic-group interview with relatives with negative experiences.

BACKGROUND: To protect older adults and their rights, social distancing, isolation and quarantine were the key measures put in place to slow down the spread of COVID-19. Yet several studies have highlighted the importance of contact and emotional support from families for residents of long-term care facilities. The purpose of the study was to elicit the experiences and perceptions on the COVID-19 epidemic preventive measures and gain insights into the perspective of people who had family members in Slovenian nursing homes during the pandemic. METHODS: A qualitative descriptive research paradig
