Background: HIV infection results in immune homeostasis perturbations, which is characterized by CD4+ T-cell depletion, immune activation, and inflammation. Effective antiretroviral therapy (ART) does not fully restore immunologic and clinical health in people living with HIV (PLWH). We report a case of advanced human immundeficiency virus infection and relapsing urinary tract infections due to analgesic nephropathy. The patient developed an urosepsis with multiorgan dysfunction syndrome and required dialysis. Inspite of this complicated course, for the first time thirteen years after diagnosis of the HIV-infection, an antiretroviral treatment was started, followed by an impressive improvement of quality of life, physical activity and psychological stabilization.

HIV-1 establishes a latent reservoir early in infection that persists during antiretroviral therapy (ART), typically leading to rapid viral rebound upon treatment interruption in most people living with HIV-1. However, a rare group known as post-treatment controllers (PTCs) can maintain low or undetectable viral loads for extended periods after ART cessation, offering critical insights into durable HIV remission.
