Late orthopedic hardware removal following fracture union is most commonly indicated in the setting of post-traumatic osteoarthritis (PTOA), whereas elective removal in asymptomatic patients remains controversial. We present the case of a 53-year-old male with a history of tibial plateau open reduction and internal fixation (ORIF) over a decade prior, who developed severe PTOA requiring total knee arthroplasty (TKA). PTOA is a known long-term complication of tibial plateau fractures, occurring in up to 44% of cases and approximately 5% of patients.Preoperative imaging demonstrated significant

BACKGROUND: While trans-syndesmotic fixation with metal screws is considered the gold standard in treating syndesmotic injuries, controversy exists regarding the need and timing of postoperative screw removal. Formal recommendations have not been well established in the literature and clinical practice is highly variable in this regard. The purpose of this systematic review is to critically examine the most recent literature regarding syndesmotic screw removal in order to provide surgeons an evidence-based approach to management of these injuries.

PURPOSE: The purpose of this study was to evaluate the effectiveness of arthroscopy combined with hardware removal for chronic pain after satisfactory healing of an ankle fracture. We hypothesized that combining hardware removal with arthroscopy for the intra-articular pathology would improve residual complaints more so than hardware removal alone.
METHODS: The outcomes of the 53 young male patients with chronic pain after healed ankle fracture treated with two different therapeutic plans: (1) conservative treatment after hardware removal (group A) and (2) arthroscopic intervention with hardw

PURPOSE: There has not been a broad national examination of complications and demographics of facial trauma reduction procedures. The literature has reported acceptable and unacceptable hardware removal rates in localized populations.
MATERIALS AND METHODS: The 2007 Nationwide Inpatient Sample was used to determine all plate removal procedures associated with common complications from facial reductions. Statistical analysis was used to compare the differences in demographics of the reduction procedure and removal procedure groups.
RESULTS: Some form of open fixation was reported in 4,879 pat

BACKGROUND: Ankle valgus deformity is associated with conditions such as clubfoot, cerebral palsy, and myelodysplasia. Guided growth strategies using a transphyseal screw provide effective correction of ankle valgus deformity. When correction occurs before skeletal maturity, screw removal is required to prevent overcorrection in the coronal plane. In this study, we reviewed the outcomes of guided growth procedures for correction of ankle valgus and related difficulty with hardware extraction.
METHODS: A retrospective review of patients with ankle valgus managed with transphyseal screw placeme

IntroductionElective ankle hardware removal is classified as a clean surgical procedure, although postoperative wound complications remain a relevant source of morbidity. The role of routine preoperative antibiotic prophylaxis in this setting remains uncertain.MethodsA retrospective observational study was conducted including patients who underwent elective ankle hardware removal between 2019 and 2025. The primary outcome was postoperative wound complications. Univariate analysis and multivariable logistic regression were performed.ResultsA total of 160 patients were included, and wound compli

BACKGROUND: This study compares clinical and functional outcomes between simultaneous hardware removal during total knee arthroplasty (TKA) and staged TKA after prior hardware removal.
METHODS: We retrospectively reviewed 155 patients who had prior knee hardware and underwent elective primary TKA between 2012 and 2024 at an urban academic institution. Patients were categorized into "simultaneous" removal during TKA (n = 127) or "staged" TKA after removal (n = 28), and stratified by hardware type (minor/moderate/major).
RESULTS: Simultaneous procedures involved significantly less "major hardw

BACKGROUND: Surgical exposure of the Lisfranc joint complex is within close proximity to the deep peroneal nerve, which can be injured in this approach. Common clinical practice is to remove Lisfranc hardware at 3 to 4 months postoperatively. However, it is unknown if this provides a clinical benefit or risks injury to the deep peroneal nerve. The rate of nerve injury is currently unknown from the published literature. This study clarifies rates of neurological injury to the deep peroneal nerve during primary surgery and hardware removal.
METHODS: This retrospective study was performed on all

INTRODUCTION: Hardware removal in healed trochanteric fractures (TF) in the absence of infection or significant mechanical complications is rarely indicated. However, in patients with persistent pain, prominent material and discomfort in the activities of daily living, the implant is eventually removed. Publications of ipsilateral femoral neck fracture after removal of implants from healed trochanteric fractures (FNFARIHTF) just because of pain or discomfort are rare. The purpose of this systematic review of the literature is to report on the eventual risk factors, the mechanisms, the clinical

PURPOSE OF THE STUDY: The use of navigation applied to total knee arthroplasty in knees with femoral hardware retained has not been studied.
MATERIAL AND METHODS: We use navigation in six patients to implant a total knee arthroplasty while retaining the femoral hardware. The retained materials were screws in two cases, diaphyseal plates in another two, intramedullary nails in one and supracondylar tube/plate in another one. Preoperative knee scores were within the 46-66 range, whereas the functional scores were within 40-68. The coronal deformity varied between 30° varus and 5° valgus.

PURPOSE: To assess the risk of tendon rupture or plate removal after volar plate fixation of distal radius fractures and to determine the incidence of hardware removal.
METHODS: We searched the surgical database of 5 attending hand surgeons at a single institution from 2009 to 2014. All patients who had undergone volar plate fixation were included. Patients were excluded if they underwent an alternate form of fixation, had less than 1 year of follow-up, or could not be reached for follow-up. Postoperative radiographs were examined for Soong grade, plate distance to the critical line, and plat

This study presents a consecutive series of patients who underwent total knee arthroplasty (TKA) after prior distal femoral fracture without hardware removal. The purpose of this study was to determine the effectiveness of computer-assisted TKA in patients with posttraumatic arthritis, specifically those with retained hardware after prior distal femoral fracture. The study group included a consecutive series of 16 patients who had developed posttraumatic knee arthritis after a distal femoral fracture with retention of hardware (group A). Patients in the study group were matched with patients w

The formation of pseudoaneuryms after orthopaedic procedures in the setting of connective tissue disorders such as Marfan syndrome (MFS) is not well documented in the literature. We report the case of a seventy-one year old male with MFS who underwent uncomplicated distal interlocking screw removal from a femoral cephalomedullary nail for symptomatic hardware. Five weeks later, he presented with a popliteal artery pseudoaneurysm. The patient underwent successful open ligation and excision of the pseudoaneurysm. This is the first reported case of a popliteal artery pseudoaneurysm following orth

BACKGROUND: Craniomaxillofacial (CMF) fractures are typically treated with open reduction and internal fixation. Open reduction and internal fixation can be complicated by hardware exposure or infection. The literature often does not differentiate between these 2 entities; so for this study, we have considered all hardware exposures as hardware infections. Approximately 5% of adults with CMF trauma are thought to develop hardware infections. Management consists of either removing the hardware versus leaving it in situ. The optimal approach has not been investigated. Thus, a systematic review o

Indications for open reduction and internal fixation (ORIF) of tibia and/or fibula fractures vary; however, some patients require removal of hardware (ROH) due to various complications. Currently, data evaluating the epidemiology of and risk factors for ROH of the tibia/fibula are limited. We examined the associations between tibia/fibula fractures and (1) characteristics of fractures requiring ORIF, (2) indications for ROH, (3) demographic risk factors for ROH; (4) length of stay, and (5) total hospital charges. The Nationwide Inpatient Sample (NIS) was used to identify patients admitted for

PURPOSE: Removal of symptomatic hardware after fracture fixation is common, especially in patients with clavicle fracture. Yet, refracture after hardware removal is a relatively common complication in those patients. The aim of this study was to identify risk factors for clavicle refractures that could be influenced by the surgical treatment provided.
METHODS: All patients from a level one trauma center from 2017 to 2022 were screened for eligibility. Inclusion criteria included hardware removal after plate osteosynthesis of the clavicle, age ≥ 18 years, and signed informed consent. Groups we

PURPOSE: To determine the incidence and reasons for hardware removal after operative fixation of distal radius fractures.
METHODS: We retrospectively reviewed 33 patients who underwent removal of a volar distal radius plate from 2007 to 2013. We recorded the primary reason for plate removal, patient sex, body mass index, AO fracture type, and plate manufacturer. The total number of both distal radius plating procedures and implant removals was analyzed.
RESULTS: Of the 33 patients who underwent implant removal, the most common reasons for removal were pain (30%), tenosynovitis (27%), malunio
