Effectiveness of different antibiotic lock solutions on catheter-related infections of hemodialysis patients: a systematic review and meta-analysis.

BACKGROUND: Antibiotic lock therapy (ALT) delivers high-concentration antimicrobials into catheter lumens and has been proposed to prevent infections in hemodialysis (HD) patients. METHODS: We conducted a systematic review and meta-analysis of randomized clinical trials (RCTs) and quasi-experimental studies comparing ALT with non-antibiotic or no-lock solutions in adults on HD. Six databases (PubMed, Embase, Scopus, Web of Science, Cochrane CENTRAL, CINAHL) were searched through 27 June 2025, identifying 17 studies involving 3,482 catheters. Two independent reviewers extracted data and assess

Comparison of the use of standard heparin and 0.9% saline as lock solution in hemodialysis catheter: Importance of closure technique.

BACKGROUND: Bleeding and thrombotic occlusion are complications of Central venous catheters. When selecting a catheter lock solution, factors such as bleeding, thrombotic occlusion, infection, and cost-effectiveness must be considered. METHODS: The study included 35 patients who used heparin as a locking solution and 35 patients who used 0.9% saline, retrospectively. In our center, after injecting the solution equal to the catheter lumen volume from the syringe, approximately 1 cc of locking solution is left in the syringe, the catheter is locked and the syringe is removed from the catheter a

Efficiency of the Genius batch hemodialysis system with low serum solute concentrations: the case of lithium intoxication therapy.

BACKGROUND: The Genius batch system consists of a 90-L closed reservoir, from which fresh dialysate is extracted at the top and to which spent dialysate is returned at the bottom. It was shown in long-term hemodialysis patients that almost the entire amount of unspent dialysate can be used before contamination of fresh with spent dialysate occurs. Separation is caused by differences in density, partly because of the presence of uremic solutes in spent dialysate. The question is raised whether this separation can be maintained during dialysis of patients who experience an intoxication without r

Efficacy of Antimicrobial Lock Solutions on Central Line-Associated Bloodstream Infection and Catheter Survival in Hemodialysis Patients: A Systematic Review and Meta-Analysis.

Hemodialysis (HD) catheters carry a high risk of life-threatening bloodstream infections and failure. Antimicrobial lock solutions (ALS) are used to reduce this risk, but their overall efficacy on key outcomes like infection rates and catheter survival requires comprehensive evaluation. This systematic review and meta-analysis assess the impact of ALS on preventing catheter-related bloodstream infections (including incidence and density) and on improving catheter survival. A systematic search was conducted across the databases of PubMed, Embase, Scopus, Cochrane Library, and Google Scholar. A

Changes in Serum Bicarbonate Levels Caused by Acetate-Containing Bicarbonate-Buffered Hemodialysis Solution: An Observational Prospective Cohort Study.

Fresenius Medical Care's NaturaLyte dialysate has been associated with increased risk of sudden cardiac death by causing metabolic alkalosis from its acetate content based on retrospective data using pre-dialysis bicarbonate levels only. The study objective was to measure inter/intra-dialytic changes in serum bicarbonate and degree of alkalosis conferred by varying concentrations of NaturaLyte bicarbonate dialysate. Thirty-nine hemodialysis patients were divided into four groups based on prescribed bicarbonate dialysate concentrations; Group 1 (N = 9): 30-32 mEq/L, Group 2 (N = 5): 33-34 mEq/L

Piloting 4% Tetrasodium Ethylenediaminetetraacetic Acid (T-EDTA) Catheter Lock Solution in Renal Hemodialysis: A Quality Improvement Evaluation.

Central vascular access devices in renal hemodialysis patients have critical utility; nevertheless, they are associated with increased relative mortality risk compared to arteriovenous fistulas and grafts. Appropriate flushing and catheter locking are key to minimizing complication and dysfunction risk. Observational pre-post quality improvement evaluation was undertaken to evaluate 4% tetrasodium ethylenediaminetetraacetic acid (T-EDTA) catheter lock solution in renal hemodialysis. Four percent T-EDTA was piloted as the standard catheter lock solution (replacing 4 mg gentamicin/5000 IU per mL

Comparative effectiveness of 30 % trisodium citrate and heparin lock solution in preventing infection and dysfunction of hemodialysis catheters: a randomized controlled trial (CITRIM trial).

BACKGROUND: Central venous catheters (CVC) are the only option when hemodialysis is needed for patients without definitive vascular access. However, CVC is associated with complications, such as infection, thrombosis, and dysfunction, leading to higher mortality and expenditures. The aim of this study was to compare the effectiveness of 30 % trisodium citrate (TSC30 %) with heparin as CVC lock solutions in preventing catheter-related bloodstream infections (CRBSI) and dysfunction in hemodialysis patients. METHODS: Randomized, double-blind controlled trial comparing the event-free survival of

UHPLC-MS/MS analysis of sphingosine 1-phosphate in joint cavity dialysate and hemodialysis solution of adjuvant arthritis rats: Application to geniposide pharmacodynamic study.

Geniposide (GE) is an iridoid glycoside compound with anti-inflammatory effect. The potential of sphingosine 1-phosphate (S1P) as a plasma marker in human diseases was suggested recently in the literature, which demonstrated that, in patients with inflammatory diseases, plasma S1P was elevated. It follows that the obstructive coronary artery disease can be predicted with serum S1P. Therefore, S1P can also be potentially used as a pharmacodynamic marker to study adjuvant arthritis (AA) rats. In the current study, a UHPLC-MS/MS method combined with the microdialysis sampling technique (using FTY

Is Hemodialysis Patient Survival Dependent upon Small Solute Clearance (Kt/V)?: If So How Can Kt/V be Adjusted to Prevent Under Dialysis in Vulnerable Groups?

Small solute clearance achieved during a single hemodialysis session has been traditionally evaluated by urea clearance, normalized for total body water (Kt/Vurea) for more than 30 years. By consensus, the target sessional KtVurea for thrice weekly treatments has been increased from 0.9 to 1.2 over the years. Although this is supported by observational studies, there is a fundamental lack of prospective studies to support this threshold target. In clinical practice achieving sessional Kt/Vurea targets are most closely followed in the US. Yet there appears to be a paradox in that by following K

Sustainable and accessible hemodialysis: life cycle assessment on central acid delivery system.

INTRODUCTION: The healthcare sector contributes over 4% of global greenhouse gas emissions. As healthcare systems pursue carbon reduction targets, sustainable innovations in clinical operations are essential. Hemodialysis (HD) relies heavily on dialysate concentrates as one of its major consumables. This study evaluates the environmental impact of implementing a central acid concentrate delivery system (CCDS) using dry concentrate powder at the University Medical Center Utrecht (UMC Utrecht) and examines its broader global applicability. METHODS: A comparative life cycle assessment (LCA) was

Intracardiac (superior vena cava/right atrial) ECGs using saline solution as the conductive medium for the proper positioning of the Shiley hemodialysis catheter: is it not time to forego [correction of forgo] the postinsertion chest radiograph?

Hemodialysis (HD) is often administered in critical care areas to patients with chronic renal failure as a continuation of the HD they are receiving on an ambulatory basis, and to patients who develop such a need for the first time or may require HD only transiently. The double-lumen Shiley central venous catheter (SCVC), inserted via the brachiocephalic veins, is often employed for HD, and it is customary to obtain a chest radiograph to ensure proper positioning of the tip of the SCVC within the superior vena cava (SVC) or high right atrium (RA). This practice is implemented to evaluate for c

Concurrent Control of Sodium and Bicarbonate Serum Concentrations Using a Four-Stream Hemodialysis Fluid Delivery System.

BACKGROUND: Previously, two reports proposed a four-stream dialysis fluid delivery system consisting of an acid concentrate, a base concentrate, a sodium chloride concentrate, and product water for correcting dysnatremias and metabolic acid-base disorders separately, by hemodialysis. METHODS: This report describes a new method for the clinical use of the previously reported four-stream dialysis fluid delivery system to treat concurrently dysnatremias and metabolic acid-base disturbances by hemodialysis. Pumps attached to each concentrate are designed to control its flow rate. Formulas were de

[Non-pharmacological calcium metabolism control in patients undergoing hemodialysis].

Calcium overload has been claimed to be involved in the increased vascular morbidity and mortality of dialysis patients. Conversely, calcium depletion can worsen secondary hyperparathyroidism and reduce bone mass. When residual renal function is null or negligible, the calcium balance is mainly determined by the calcium absorbed from the intestine, and that gained or lost from or into the dialysate. This article aims to review the assessment and mechanisms of calcium flux during hemodialysis (HD) and hemodiafiltration (HDF). Calcium mass transfer in HD is mainly dependent on the ionized calciu
