Esketamine combined with hydromorphone versus hydromorphone for post-operative patient-controlled intravenous analgesia in children: a randomized controlled trial.

BACKGROUND: Postoperative pain and opioid-related adverse effects remain significant concerns in pediatric surgical patients. Esketamine, an NMDA receptor antagonist, may enhance analgesia and reduce opioid consumption at low doses. This study aimed to evaluate the effects of low-dose esketamine combined with hydromorphone for PCIA on postoperative pain control, bowel recovery, and safety in children undergoing abdominal surgery. METHODS: This randomized controlled clinical trial enrolled children aged from 3 to 12 years, who were scheduled to undergo abdominal surgery under general anesthesi

Morphine and hydromorphone-induced hyperalgesia in a hospice patient.

Opioids including morphine and hydromorphone are widely used for control of moderate to severe pain and dyspnea in hospice and palliative care patients. Accumulation of the active morphine-3-glucuronide (M3G) and hydromorphone-3-glucuronide (H3G) metabolites is one proposed mechanism for the development of neuroexcitatory effects including allodynia and opioid-induced hyperalgesia (OIH). We report the case of a 43-year-old female hospice patient with metastatic non-small cell lung cancer who initially developed allodynia following morphine administration and again following administration of h

The role of OROS hydromorphone in the management of cancer pain.

The vast majority of cancer patients experience pain, and treatment with opioids offers the most effective option for pain management. Long-lasting opioid formulations are usually used as cancer pain management strategies. This review surveys the available literature on the only available once-daily sustained-release formulation of hydromorphone, and its use in cancer pain management. Sustained-release (SR) formulations have a more consistent opioid plasma concentration, thereby minimizing the peaks and troughs associated with immediate-release opioid formulations. OROS hydromorphone (Jurnista

Pharmacokinetic and pharmacodynamic evaluation of intravenous hydromorphone in cats.

This study describes the pharmacokinetics of intravenous hydromorphone in cats and the simultaneous measurement of antinociceptive pharmacodynamic effects using a thermal threshold testing system. Following establishment of a baseline thermal threshold, six adult cats were administered 0.1 mg/kg of hydromorphone intravenously. Thermal threshold testing and blood collection were conducted simultaneously at predetermined time points. Plasma hydromorphone concentrations were determined by a liquid chromatographic-mass spectral method and pharmacokinetic analysis was performed by nonlinear least s

A systematic review of hydromorphone in acute and chronic pain.

While morphine is historically the gold standard for the management of severe cancer pain, some patients either do not achieve adequate analgesia, or suffer intolerable side effects from this drug. For these patients, alternatives such as hydromorphone are recommended. This review explores the evidence for the efficacy of hydromorphone in the management of pain. A systematic search, from 1966 to 2000, of published and unpublished randomized trials that involved the administration of hydromorphone for both acute and chronic pain conditions in adults and children, was conducted. Forty-three stud

Buprenorphine: duration of blockade of effects of intramuscular hydromorphone.

Six opioid-dependent in-patients were maintained on daily sublingual doses of buprenorphine at 2, 6, and 12 mg/day for five days at each dose in a randomized, balanced sequence. Placebo buprenorphine was substituted for the next three days, and challenge doses of the mu agonist hydromorphone were administered on the three days. Planned comparisons in a 3-factor ANOVA showed dose-dependent hydromorphone effects, significant blockade of 'high' by the 12 mg buprenorphine dose, and no differences on hydromorphone-induced 'high' across 72 h of active buprenorphine. The data suggest that the blockad

Retrospective Review of Intrathecal Hydromorphone Dose Range and Complications.

BACKGROUND: Optimal intrathecal dosing regimens for hydromorphone are not well established for analgesia after abdominal surgery. OBJECTIVES: We reviewed intrathecal hydromorphone doses and complications because dosing variability has been observed among anesthesiologists. We hypothesized that increasing doses of intrathecal hydromorphone would be associated with improved postoperative analgesia, but with increased rates of opioid-related adverse events. STUDY DESIGN: Retrospective analysis. SETTING: A high-volume academic referral center in the United States. METHODS: A retrospective stud

[New Opioid Options in Japan - Methadone, Tapentadol and Hydromorphone].

In 2010s, several opioids became available in Japan, including methadone, tapentadol and hydromorphone. Methadone was approved in September 2012 by Japanese regulatory authority. Since methadone is positioned as so-calledstep 4 opioidin Japan, it must be prescribed as alternative opioid switched from another of 60mg/day or greater equivalent dose of oral morphine. Diversity of pharmacokinetics among individuals and various drug interactions require close monitoring of adverse events. In spite of these cautions, unique characteristics such as inhibiting N-methyl-D-aspartate(NMDA)and in- ducing

Hydromorphone-3-glucuronide: biochemical synthesis and preliminary pharmacological evaluation.

Hydromorphone-3-glucuronide (H3G) was synthesized biochemically using rat liver microsomes, uridine-5'-diphosphoglucuronic acid (UDPGA) and the substrate, hydromorphone. Initially, the crude putative H3G product was purified by ethyl acetate precipitation and washing with acetonitrile. Final purification was achieved using semi-preparative high-performance-liquid-chromatography (HPLC) with ultraviolet (UV) detection. The purity of the final H3G product was shown by HPLC with electrochemical and ultraviolet detection to be > 99.9% and it was produced in a yield of = 60% (on a molar basis). The

A case of fatal overdose involving both hydromorphone and kratom.

Kratom is a plant originating in Southeast Asia that has been used for its dose-dependent stimulant and opioid effects. The main active compound in kratom is mitragynine, an alkaloid with affinity for the mu-opioid receptor. Toxicity and fatalities related to kratom use have increased substantially in recent years. In this case report, we describe a 44-year-old man who was found deceased in bed. The only significant finding at autopsy was abdominal distension with >4 L of ascites. Toxicology testing was performed on femoral blood which showed 79 ng/mL of hydromorphone, 560 ng/mL of mitragynine

The role of hydromorphone and OPRM1 in postoperative pain relief with hydrocodone.

BACKGROUND: Postoperative pain management remains a challenge for clinicians due to unpredictable patient responses to opioid therapy. Some of this variability may result from single nucleotide polymorphisms (SNPs) of the human opioid mu-1 receptor (OPRM1) that modify receptor binding or signal transduction. The OPRM1 variant with the highest frequency is the A118G SNP. However, previous studies have produced inconsistent results regarding the clinical effects of A118G on opioid response. We hypothesized that measurement of serum opioid concentrations, in addition to determining total opioid c

Antinociceptive effects of epidural administration of hydromorphone in conscious cats.

OBJECTIVE: To evaluate the antinociceptive effects of epidurally administered hydromorphone in conscious, healthy cats. ANIMALS: 7 healthy adult cats. PROCEDURES: An epidural catheter was implanted in each cat. Thermal threshold (TT) was measured by increasing the temperature of a probe placed on the thorax and monitoring the cat's response. Mechanical threshold (MT) was measured by manually inflating a modified blood-pressure bladder affixed to a thoracic limb and monitoring the response. After the baseline TT and MT values were determined, hydromorphone (0.05 mg/kg) or an equal volume of s

Analgesic efficacy of hydromorphone in American alligators (Alligator mississippiensis).

BACKGROUND: American alligators (Alligator mississippiensis) are maintained in zoos, aquaria, and farms for educational, research, and production purposes. The standard of veterinary medical care and welfare for captive reptiles requires managing pain and discomfort under conditions deemed painful in mammals. While analgesic efficacy and pharmacokinetic data for several reptile species are published, data with respect to analgesic efficacy in crocodilians are clearly lacking. OBJECTIVE: The objective of this study was to determine the analgesic efficacy of hydromorphone in alligators. METHOD

Hydromorphone use for acute pain: Misconceptions, controversies, and risks.

Hydromorphone (HM) is a potent opioid analgesic that is commonly administered in the emergency department (ED) and other acute care settings, such as medical surgical wards. In recent years, there has been a significant increase in the ED administration of HM relative to other opioids. Although HM is an effective analgesic, its use has been commonly implicated in adverse drug events and medication errors. In addition, intravenous HM has potent euphoric effects that may contribute to its abuse liability. There are limited data regarding how acute parenteral administration of opioid analgesics i
