The EEA instrument was used to perform ileostomy in three patients. Two female patients with complications of Crohn's colitis underwent total colectomy and proctocolectomy, respectively. The first patient was operated upon because of toxic megacolon and the second patient for severe perianal disease. A third male patient underwent total colectomy for severe ulcerative colitis that failed to respond to medical therapy. In all patients, ileocutaneous anastomosis was performed with the EEA stapler (ILS, Ethicon). No serious complications were observed except for slight retraction of the ileostomy

INTRODUCTION: A loop ileostomy is a suitable procedure for faecal diversion. A number of technical improvements and advancement in stoma management have made its creation a suitable alternative to a loop colostomy. We describe an alternative technique for securing a loop ileostomy and perform a retrospective review of this technique.
PATIENTS & METHOD: 40 patients who had a loop ileostomy performed as part of an abdominal procedure were reviewed. The loop of ileum was secured to the stoma site with a novel 'suture bridge' technique.

Ogilvie's syndrome (acute pseudoobstruction of the colon) is characterized by acute massive dilatation of the caecum and right colon without organic obstruction. If untreated, distension may cause perforation, peritonitis, and death. Colonoscopic decompression of the colon is the treatment of choice and should be attempted cautiously as soon as the diagnosis is made. If colonoscopy is unavailable or unsuccessful, double-tubed ileostomy and enemas in the anal tube of the stoma is a safe and effective form of treatment.

PURPOSE: Restorative proctocolectomy (RPC) by abdominal colectomy and ileal pouch-anal anastomosis (IPAA) in the setting of chronic ulcerative colitis (CUC) and familial adenomatous polyposis (FAP) has gained widespread popularity among surgeons and patients. Ileoanal anastomosis is usually performed with covering ileostomy. This is primarily done because of fear of pelvic sepsis. Temporary ileostomy may, however, be a source of significant complications. We report a case of a 59-year-old white male who presented with right-sided abdominal pain of 6 months duration. The patient had a total colectomy and Brooks continent ileostomy 20 years ago for severe ulcerative colitis. A CT scan of the abdomen and pelvis revealed large number of pills (pharmacobezoar) in the small bowel, just proximal to the ileostomy pouch. The patient underwent a laparotomy and enterotomy, which resulted in extraction of more than 50 pills from the small bowel. He had no symptoms during the subsequent follow up visits.

Early and late morbidity was studied in 45 consecutive patients submitted to the Kock continent ileostomy reservoir procedure protected by a temporary loop ileostomy. In 24 patients the nipple valve was made by simple intussusception of the ileal segment after stripping of its mesenteric peritoneum and fat, whereas in 21 patients the nipple valve was stapled in addition. The early complication rate was low with necrosis of the nipple valve occurring in 3 patients and a nipple valve fistula in 1. These complications were easily dealt with and revision was done subsequently on an elective basis.
