A comparison of four techniques to establish intraosseous infusion.

This study was designed to determine whether the success rate in establishing intraosseous infusion (IOI) varied with four different types of needles--standard hypodermic, spinal, bone marrow, and Turkel intraosseous infusion needle. Twenty-four second-year residents from various specialties, without prior training or experience in the technique, participated in the study. Each participant attempted to establish an intraosseous infusion in a randomly assigned limb of an anesthetized piglet, using each needle in a randomly assigned order. The overall success rate was 67.7%. Success ratios varie

Intraosseous route as alternative access for infusion therapy.

The safe and effective use of intraosseous (IO) access is evolving from its historical use only for emergent access for pediatric patients to use in adult trauma patients and, more recently, for adult inpatients. Current data and research support the use of IO route for infusion in patients of any age as a safe, rapid, and equally effective alternative to intravenous access. The article provides an overview of IO access indications, care, and management; describes therapies administered via IO access; and discusses the expanding use of IO access into areas within hospitals during nonemergent c

Use of intraosseous infusion in the pediatric trauma patient.

Intraosseous infusions (IO) are frequently used for gaining rapid vascular access in critically ill children. Few studies exist evaluating the efficacy of this procedure in the injured child. The objective of this study was to describe one pediatric institution's experience with the procedure of IO in young trauma victims. This study evaluated indications, insertion sites, complications, infused pharmacological agents, age, injury severity, and outcome. Fifteen patients received IO placement for cardiopulmonary arrest, seven for hypovolemic shock, and five for neurological compromise. Patient

Intraosseous crystalloid and blood infusion in a swine model.

The technique of intraosseous infusion has attracted increasing interest in recent years, and has proven valuable for drug administration. This study was undertaken to determine whether it was also a potential route for fluid resuscitation. Thirteen- or eighteen-gauge tibial intraosseous needles were placed in eight "large" (mean weight, 14.4 kg) and eight "small" (mean weight, 5.8 kg) swine and the flow rate of blood and saline measured under gravity and 300 mm Hg. Flow was significantly greater using 13-gauge needles in the "large" swine, significantly greater for saline than for blood, and

Intraosseous infusion into the skull: potential application for the management of hydrocephalus.

OBJECT: Hydrocephalus results from abnormal cerebrospinal fluid (CSF) volumes or flow patterns. The absorption of CSF is determined largely by pressures within veins and venous sinuses in the head and adjacent to the spine. Most surgical solutions for hydrocephalus involve diversion of excess CSF into alternative absorption sites, and most of these solutions are still suboptimal. The focus of this work has been to recreate more normal CSF absorption into the dural venous sinuses without having to directly access the superior sagittal sinus (SSS). METHODS: Intraosseous skull infusion for the p

[Intraosseous infusion. An important technique also for paediatric anaesthesia].

Timely establishment of venous access in infants and toddlers can prove a particularly challenging task. Since the 1940s the technique of intraosseous infusion has established itself as a valuable alternative means for rapid, efficient and safe delivery of drugs and fluids to critically ill children. Whereas international guidelines for paediatric emergency medical care have assigned intraosseous infusion a high priority, most anaesthetists utilize this well-proven technique with great reluctance. This article describes the technique of intraosseous infusion, introduces two different cannulati

Resuscitation of hemorrhage with intraosseous infusion of hypertonic saline/dextran.

We resuscitated unanesthetized bled sheep (bled volume = 1.2-1.7 liters) with 200 ml of hypertonic saline/dextran 70 infused either through a peripheral vein (n = 6) or directly into the red marrow of the sternum (n = 6). Intraosseous infusion of the viscous 7.5% NaCl/6% dextran solution required 2-4 min. Plasma sodium was rapidly increased to the same level in both groups demonstrating equally rapid entry into the vascular space. Both regimens provide rapid and sustained normalization of arterial pressure and cardiac output. No significant differences between the two groups were apparent for

[Treatment of acute thrombophlebitis of the lower limbs with long-term intraosseous drug infusions].

Under analysis are the results obtained in 62 patients with acute thrombophlebitis of lower extremities treated by prolonged intraosseous heparin-penicillin blockade. The acute inflammatory phenomena along the veins subsided within 2-6 days. The complete cure was observed 1,5-6,5 days earlier in cases with superficial thrombophlebitis and 4-15 days earlier in profound thrombophlebitis as compared with control.

First-in-human evaluation of a novel subcutaneous intraosseous vascular access device for outpatient infusions.

BACKGROUND: Conventional intraosseous (IO) access is typically reserved for emergency situations and limited to short-term use (⩽48 h). We introduce a novel, fully subcutaneous IO device designed to provide stable, longer-term vascular access in non-emergent settings. This device may benefit patients requiring intravenous therapies with challenging venous access or chronic kidney disease (CKD) who need to preserve peripheral and central veins for future arteriovenous fistula creation for hemodialysis. AIM AND METHODS: This prospective, single-arm, first-in-human study evaluated the feasibilit

Successful use of an intraosseous infusion in an 800 grams preterm infant.

An intravascular access line for the administration of life support drugs and volume expanders may be particularly difficult, especially in very small premature babies. We report on the successful use of an intraosseous accessline in an 800 grams preterm infant for the administration of drugs and fluid. The use and technique of an intraosseous access is an important emergency alternative which may be lifesaving, even in very preterm babies, when other methods fail.

Set-Up Times, Infusion Times, and User Evaluations of Three Pressurized Intraosseous Infusion Devices.

INTRODUCTION: Hemorrhage is the leading cause of preventable battlefield death, and rapid whole blood transfusion is lifesaving. IV access is often difficult or unobtainable in hemorrhagic shock. Tactical Combat Casualty Care (TCCC) currently recommends intraosseous (IO) access for difficult vascular access scenarios and use of a pressure bag for IO blood infusion. However, comparative data on IO infusion speed and user experience with different IO infusion techniques is lacking. METHODS: This study evaluated 64 military medical personnel (32 Corpsmen, 32 Special Operations medics) in a manne

Intraosseous infusion is unreliable for adenosine delivery in the treatment of supraventricular tachycardia.

Supraventricular tachycardia (SVT) is a common tachyarrhythmia in the pediatric population that can necessitate immediate treatment. Adenosine has been well studied as a mainstay treatment, but the methods of adenosine administration have not been very well delineated. The intraosseous technique has presented itself as a possible method of administration. We describe 2 cases in which adenosine was administered through bone marrow infusion to convert SVT without success. The cases we describe show that intraosseous is not a reliable method of administering adenosine to stop SVT. Both patients p

Finding an ideal site for intraosseous infusion of the tibia: an anatomical study.

Intraosseous infusion is a technique used for the administration of fluids to a hemodynamically shocked child in whom attempts to access the vascular system have been unsuccessful. Although few complications are seen, injury to the epiphyseal growth plate during the performance of this technique remains a serious problem. This study investigates the relationship between the site of insertion of the intraosseous needle and the epiphyseal growth plate, and the ease of needle insertion into various locations of the tibia in newborn infants. Fourteen newborn infant cadavers (28 tibias in total) we

[Heart arrest: septic and hypovolemic shock--intraosseous infusion technique].

Cardiorespiratory arrests in infants are most often caused by generalized hypoxia. This leads to the well-known fact that the outcome of arrests in the pediatric age group is poor. On the other hand we have the chance to recognize the ongoing hypoxia at early stages. Prevention of this dangerous situation is the aim of treatment. Clinical manifestations of shock states in children differ considerably from those seen in adult patients. Causes and treatment of hypovolemic and septic shock and the technique of intraosseous puncture and infusion are described in detail.

A Novel Method of Intraosseous Infusion of Adenosine for the Treatment of Supraventricular Tachycardia in an Infant.

Supraventricular tachycardia is a common arrhythmia faced by emergency physicians in the pediatric population. In most cases, antecubital intravenous access can be established, and adenosine can be administered in a rapid and timely fashion. The role and administration of intraosseous adenosine are poorly established. We describe a case where the administration of adenosine was successful via a mixed method administration.

Intraosseous infusion of blood products and epinephrine in an adult patient in hemorrhagic shock.

A 79-year-old woman presented in the postanesthesia care unit with hematemesis following replacement of a jejunostomy tube. Her medical history included recurrent stage IIIC ovarian cancer. The patient rapidly decompensated despite blood products administered through the patient's implanted medication port. The anesthesia service was consulted for resuscitative support. Examination revealed an alert, hypotensive elderly female in hemorrhagic shock. While peripheral intravenous (IV) access was sought, her condition further deteriorated. Attempts at peripheral access were determined futile and c

The Use of Intraosseous Infusion in the Early Resuscitation of Patients With Extremely Severe Burns.

According to research, shock, the most common complication of extremely severe burns, is also the leading cause of mortality among patients with such burns. The case fatality rate reaches 83.45% when the total burn area exceeds 90%. The American Heart Association in 2020 recommended the intraosseous (IO) access after the peripheral access and prior to the central venous access when venous cannulation is either difficult or delayed. The use and experience with intraosseous infusion in extremely severe burns are still limited. We report efficacy and safety results from 19 burn patients treated w

Fluid resuscitation using the intraosseous route: infusion with lactated Ringer's and hetastarch.

OBJECTIVE: The effectiveness of the intraosseous (IO) route for fluid resuscitation remains uncertain. This study compares IO infusion rates and estimated volume expansion using clinically relevant infusion pressures with lactated Ringer's (LR) and hetastarch (HES). METHODS: IO needles were placed in the sternum or tibia for infusion of HES and LR in 8 Yorkshire pigs. Pressures were recorded at variable flow rates and linear regression used to identify flow rates at 100 and 400 mm Hg. Volume expansion was calculated for a 10-minute infusion. RESULTS: Mean LR flow rates (mL x min(-1)) were 24
