From caregiving burden to income transformation: intergenerational employment effects of long-term care insurance.

BACKGROUND: Long-term care insurance (LTCI) has been introduced in China to address the challenges posed by an aging population. This study examines its impact on the income of older adult households, using data from the China Health and Retirement Longitudinal Study (CHARLS) from 2011 to 2020. The analysis further explores heterogeneity and implications for income inequality. METHODS: A Difference-in-Differences (DID) approach is employed, with 12 pilot cities as the treatment group and non-pilot cities as the control group. The analysis is based on 33,435 valid observations of individuals a

Factors related to unmet nursing care needs for home-visit nursing among long-term care insurance beneficiaries.

This retrospective, population-based study aimed to identify factors associated with unmet nursing care needs among long-term care insurance (LTCI) beneficiaries for whom home-visit nursing (HVN) was recommended during care planning. We analyzed 1,445,835 LTCI beneficiaries using home care services between 2018 and 2022 in South Korea. Among those recommended for HVN, 91.4% did not utilize these services. The most frequent unmet needs were dialysis nursing, cancer pain control, and diabetic foot care. Logistic regression revealed that unmet needs were significantly associated with living alone

Does long-term care insurance promote assistive device utilization? Evidence from pilot regions in China.

BACKGROUND: As China's population aging rapidly, long-term care insurance (LTCI) has emerged as a critical policy instrument to address the care needs of older adults. While previous studies have examined the effects of LTCI on service utilization, less is known about its effect on assistive device utilization-a key component in supporting functional independence and reducing caregiver burden. This study examines the association between LTCI benefit design and assistive device use among older adults in China. METHODS: We use cross-sectional data from 1,460 older adults residing in elderly car

Long-term care insurance and intergenerational transfers in families with disabled older adults in China.

Intergenerational transfers constitute an important dimension of later-life quality of life and economic security and serve as a key margin through which social insurance policies affect family welfare. This study examines the effect of long-term care insurance (LTCI) on intergenerational transfers within elderly families in China. We exploit a staggered difference-in-differences approach, complemented by an event-study design, to assess the impact of LTCI on intergenerational transfers, including downward transfers from older parents to their adult children and upward transfers in the opposit

Why are people with dementia overlooked in long-term care insurance policy in Guangzhou, China?

BACKGROUND AND OBJECTIVES: There is a growing demand for dementia care to be funded by long-term care insurance (LTCI). However, evidence indicates that people with dementia are overlooked in China's LTCI policy and empirical research on this issue is notably scarce. Among the first seven LTCI pilot cities that officially enrolled people with dementia, Guangzhou is unique for roll-back LTCI policies related to eligibility criteria and benefits. This study aims to explore why people with dementia are overlooked in China's LTCI policy, using Guangzhou as a case study. METHODS: Semi-structured i

Frailty Status Predicts New Long-term Care Insurance Certification in Hepatitis C Patients Receiving Antiviral Therapy.

BACKGROUND/AIM: Direct-acting antiviral (DAA) therapies for patients with hepatitis C virus (HCV) infection deliver higher cure rates and lower frequencies of adverse events than existing therapies, though DAA treatment costs $45,000-64,000 in Japan. The prognosis of patients who require new long-term care insurance (LTCI) certification is inferior to that of patients who do not. Here, we clarify the factors associated with new LTCI certification in elderly patients with HCV infection who undergo DAA therapy. PATIENTS AND METHODS: We retrospectively surveyed 53 patients aged ≥70 years who wer

Compensatory Effects of Long-Term Care Insurance on Mental Health Among Rural Older Women: Evidence from China.

Background: Rural older women in China face intersecting disadvantages including eroded family support, limited pension coverage, and high social isolation that elevate depressive symptoms. Although Long-Term Care Insurance (LTCI) has expanded rapidly since 2016 to provide both financial protection and formal care services for older adults, whether it can compensate for depleted informal support and protect mental health among this vulnerable population remains unknown. Methods: We used five waves of CHARLS (2011-2020) with 2193 rural women aged ≥60. We employed a staggered difference-in-diffe
