BACKGROUND/AIM: The risk for nosocomial infections (NIs) is 5-10 times higher in patients hospitalized in the Intensive Care Units (ICUs) than in patients staying in other wards. The higher incidence rates of NIs in the ICUs may be explained by the fact that the patients in the ICUs have more severe underlying disease, and are exposed to the invasive diagnostic and therapeutical procedures. The unreasonable use of antibiotics leads to the selection of multiresistant agents, which have been increasingly recorded as the NIs causative agents. The aim of this study was to investigate the character

OBJECTIVE: To perform an epidemiologic study of artificial nutrition in critically-ill pediatric patients.
PATIENTS AND METHODS: A multicenter, prospective and descriptive study was conducted in 23 Spanish intensive care units (ICU) (18 pediatric ICUs and five pediatric/neonatal ICUs) over a 1-month period. Artificial nutrition (AN) was required by 165 critically-ill patients (21.4 %). Data on diagnosis, severity, treatment, type of nutrition administered and complications were analyzed.
RESULTS: A total of 54.4 % of the participants were younger than 1 year, 19.4 % were aged between 1 and 5

OBJECTIVE: To study antimicrobial use for benchmarking and ensuring quality of antimicrobial treatment and to identify risk factors associated with the high use of antimicrobials in German intensive care units (ICUs) through implementation of the SARI (Surveillance of Antimicrobial Use and Antimicrobial Resistance in ICUs) system.
DESIGN: Prospective, unit-based surveillance on antimicrobial use from February, 2000, until June, 2002. The data are standardised by use of the defined daily dose (DDD) for each antimicrobial defined by the WHO and by calculating use per 1000 patient days.

BACKGROUND: Despite the availability of evidence-based eye-care protocols, compliance among nurses in intensive care units (ICUs) remains inconsistent. Conceptual ambiguity between adherence, compliance, and protocol implementation has limited the development of valid measures and targeted interventions.
OBJECTIVE: To clarify and define the concept of Eye Care Compliance in ICUs through a systematic concept analysis.
METHODS: This concept analysis followed the eight-step framework of Walker and Avant. CINAHL Plus, PubMed, Scopus, Web of Science, and ScienceDirect were searched for studies pu

In the units for difficult patients (UMD) and the intensive psychiatric care units (Usip), patients whose mental pathologies have or could generate violent acts which can go as far as committing homicide are taken care of. If, during the psychiatric care of these patients, isolation and restraint measures can be put in place as a last resort, in general it is the symptomatological and behavioral appeasement of these persons that is sought in an alternative way.

BACKGROUND: As a neuropsychiatric syndrome, delirium worsens the prognosis of critically ill patients in the intensive care unit (ICU). Risk factors such as sleep deprivation increase the incidence of delirium. Targeted light exposure influences the circadian rhythm, and daylight interventions can help to restore the natural daily rhythm. To date, a review of daylight therapy for German-speaking countries is lacking.
OBJECTIVE: This work aims to provide an overview of the currently available evidence on the use of daylight therapy in critically ill patients on the ICU, with a medical focus on

Candidemia is a significant cause of morbidity and mortality among critically ill patients. Early-onset candidemia is characterized by occurring within the first seven days after admission to the Intensive Care Unit and presents several important challenges regarding its management. Risk factors may vary among patients with early- and late-onset infection, while clinical manifestations are generally non-specific and covered by the underlying disease and co-morbidities. Diagnosis and appropriate therapy are frequently delayed, with a high risk of progression to invasive, deep-seated infections,

Immobility of patients in intensive care units (ICU) can lead to long-lasting physical and cognitive decline. During the last few years, bundles for rehabilitation were developed, including early mobilization. The German guideline for positioning therapy and mobilization, in general, recommends the development of ICU-specific protocols. The aim of this narrative review is to provide guidance when developing a best practice protocol in one's own field of work. It is recommended to a) implement early mobilization as part of a bundle, including screening and management of patient's awareness, pai

BACKGROUND: Delirium as a result of hospitalization in an intensive care unit (ICU) is defined by a mental state different from the patients' normal state and an acute fluctuating course. Both morbidity and mortality are increased in relation to delirium. The incidence of delirium has been reported from 16% to 87% in international studies primarily in elderly patients.
AIMS: The purpose of this study was to evaluate the incidence of delirium in adult intensive care patients in Denmark and to identify correlations between delirium, sedatives, opioid analgesics and age.
METHODS: In a prospecti

In last years an increasing number of evidences has been gained that inflammatory response plays a major role in critical illness. The acronym SIRS (Systemic Inflammatory Response Syndrome) has been introduced to define the condition in which the inflammatory reaction exceeds local mechanisms of containment and inflammatory mediators invade the bloodstream causing systemic disturbances. Theoretically, the use of corticosteroids offers a potent tool to control the excess of inflammatory reaction, but initial trials on Adult Respiratory Distress Syndrome (ARDS), head trauma, and septic shock sho

Malaria requiring intensive care is characterized by failure of one or more organ systems and/or development of several metabolic disorders secondary to the presence of Plasmodium faliciparum in the blood. Severe imported malaria in non-immunized adults causes multiple organ failure with variable degrees of altered mental status. Acute pulmonary edema is frequent, jaundice associated with mild disturbance of liver function is consistent, arterial hypertension due to hypovolemia is usual, and acute renal insufficiency is uncommon. Coagulation disorders are generally low-grade, acidosis is an un

Sedation is a technique widely used in intensive care unit patients. The main objective is to ensure a proper level of analgesia and the best physical and psychical comfort possible. For the vast majority of patients a light level of sedation is adequate and the level of sedation can easily be deepened to perform a short but painful procedure. A deeper level of sedation, close to that of a general anesthesia is rarely needed and limited to specific indications: adult respiratory distress syndrome, head trauma, status asthmaticus. Drugs used for sedation are combinations of opioids (fentanyl or

Psychosomatic medicine in intensive care units is essentially characterized by problems arising from the emotional involvement of the intensive care unit team. That is why the functions of a psychosomaticist are not only the diagnostics of psychosyndromes and the psychotherapy of dangerously ill patients, but especially the study of psychological interrelations within the unit team. Hypochondriac depressive psychosyndromes caused by the experience of dread, confusion, exhaustion and communication problems, as well as psychosyndromes characterized by a reduction of consciousness and orientation

OBJECTIVE: To map existing knowledge on palliative extubation in adult patients admitted to intensive care units.
METHOD: Scoping review conducted according to JBI guidelines. Searches were conducted between October and December 2024 in seven databases and four repositories, in addition to a reverse search. The selection was independent, the extraction followed a structured protocol, and the report used PRISMA-ScR. The analysis of the results was based on the PAGER framework (Patterns, Advances, Gaps, Evidence for practice and Research recommendations).
RESULTS: The search identified 1,463 m

Rationale: There is a paucity of data, and no consensus, about the composition of interdisciplinary teams of healthcare workers (HCWs) who provide care in intensive care units (ICUs). Objectives: To delineate the nature and variation of HCW staff composition in U.S. adult ICUs before the COVID-19 pandemic. Methods: A national survey of 574 adult ICUs inquired about ICU staffing. Two sets of survey items asked about 1) "availability to provide care" in ICUs for 11 HCW types, collapsed into six groupings; and 2) the presence in formal ICU clinical rounds of nine HCW types, collapsed into six gro

BACKGROUND: To determine the outcome of children treated for acute limb ischemia (ALI) in pediatric intensive care units (ICUs).
METHODS: A single-center, retrospective cohort study including all patients diagnosed with ALI between 2005 and 2022 in 2 different pediatric ICUs: respiratory and cardiac ICU. Data collected included patients demographics and comorbidities, location and cause of arterial occlusion, and type and duration of therapy. Primary end point was freedom from amputation. Secondary end point was all-cause mortality.
RESULTS: A total of 78 patients (58% male) with ALI were in

BACKGROUND: In intensive care units (ICUs), antiepileptic drugs (AEDs) are used for manifold indications. This is the first study to assess the prevalence of acute AED use in ICUs and to identify associated clinical variables.
METHODS: All patients in seven adult ICUs of a German university hospital in 2016 were retrospectively evaluated. Data were extracted from the computerized critical care information system and manually reviewed. Acute AED treatments were defined as initiated during ICU treatment or ≤ 6 h before ICU admission, excluding benzodiazepines and sedatives.
RESULTS: Among 2335

OBJECTIVE: To know and compare the patterns of antimicrobials use in intensive care units (ICUs) based on the Anatomical Therapeutic Chemical/Defined Daily Dose (ATC/DDD) system.
METHODS: a prospective cohort study was conducted in three medical-surgical intensive care units, two of them in public hospitals and one in a private hospital. Simple random, independent samples of patients admitted from 10/2004 to 09/2005 to the selected intensive care units were used. The antibiotics use was assessed using the ATC/DDD system. The amount of antibacterials used in each intensive care unit, in grams,
