The international normalized ratio does not reflect bleeding risk in esophageal variceal hemorrhage.

BACKGROUND/AIMS: The international normalized ratio (INR) has not been validated as a predictor of bleeding risk in cirrhotics. The aim of this study was to determine whether elevation in the INR correlated with risk of esophageal variceal hemorrhage and whether correction of the INR prior to endoscopic therapy affects failure to control bleeding. PATIENTS AND METHODS: Patient records were retrospectively reviewed from January 1, 2000 to December 31, 2010. Cases were cirrhotics admitted to the hospital due to bleeding esophageal varices. Controls were cirrhotics with a history of non-bleeding

International normalized ratio patient self-management for mechanical valves: is it safe enough?

PURPOSE OF REVIEW: People with mechanical heart valve replacement depend on lifelong anticoagulation. Since a few years, patients can control this themselves with the assistance of a portable anticoagulation monitor. If the patient performs the complete self-testing and self-adjustment by himself, the method is called self-management. Recently completed studies concerning international normalized ratio (INR) self-management in mechanical heart valve patients are reviewed in this article. RECENT FINDINGS: Large randomized prospective studies have demonstrated that the INR self-management conce

Thrombin generation and international normalized ratio in inherited thrombophilia patients receiving thromboprophylactic therapy.

BACKGROUND: Thrombin generation assay (TGA) is useful as a global functional test for assessing bleeding or thrombotic risk and its modification with therapy. We investigated TGA to assess anticoagulation status compared with the international normalized ratio (INR) system in patients with primary thrombophilia receiving and not undergoing thromboprophylaxis. MATERIALS AND METHODS: We studied 50 patients with at least one thrombotic event and a confirmed diagnosis of inherited thrombophilia. Thrombin generation was measured in platelet-poor plasma by calibrated automated thrombography (CAT).

Relationship Between International Normalized Ratio to Albumin Ratio and Mortality in Critically Ill Patients With Myocardial Infarction.

BackgroundMyocardial infarction (MI) continues to be one of the leading causes of morbidity and death worldwide. The international normalized ratio (INR) to albumin ratio (PTAR), calculated as INR divided by serum albumin, is a composite biomarker reflecting both nutritional status and coagulation abnormalities. While PTAR has shown prognostic relevance across various severe clinical scenarios, its role in forecasting mortality among critically ill MI patients remains insufficiently defined. The objective of this study was to examine the relationship between PTAR and all-cause mortality, and t

Prognostic value of the international normalized ratio to albumin ratio in acute pancreatitis: insights from MIMIC-IV database.

BACKGROUND: While the prognostic value of the international normalized ratio - to - albumin ratio (PTAR) has been established in several diseases, its predictive value in acute pancreatitis (AP) mortality remains uncertain. This study aimed to investigate the association between PTAR and survival outcomes in AP patients. METHODS: Initial screening using univariate and multivariate regression analysis identified significant prognostic variables. Survival distributions according to different PTAR levels were compared using the Kaplan - Meier (K - M) method. Receiver operating characteristic (RO

Torsemide and warfarin: A cautionary case of altered international normalized ratio and bleeding risk.

Warfarin is a widely used oral anticoagulant with a narrow therapeutic index and multiple drug-drug interactions that may significantly alter its anticoagulant effect. Torsemide, a loop diuretic commonly prescribed for heart failure and fluid overload, shares metabolic pathways with warfarin, raising the possibility of clinically significant interactions. However, the evidence regarding this interaction remains limited and inconsistent. A 66-year-old male with a history of type 2 diabetes mellitus, hypertension, chronic kidney disease, and prior aortic valve replacement on chronic warfarin the

Elevated international normalized ratio is correlated with large volume transfusion in pediatric trauma patients.

BACKGROUND: Pediatric trauma patients may benefit from a balanced transfusion strategy, however, determining when to activate massive transfusion protocols remains uncertain. The purpose of this study was to explore whether certain scoring systems can predict the need for large volume transfusion. METHODS: We conducted a retrospective review of pediatric trauma patients who presented to our center and required a transfusion of packed red blood cells. Baseline laboratory and clinical data were used to calculate Trauma Associated Severe Hemorrhage (TASH) score and a previously reported composit

Comparison of Sampling Methods for International Normalized Ratio Monitoring in Haemodialysis Patients (INRHaemo Study).

BACKGROUND: Haemorrhagic and thrombotic complications are common in dialysis patients on warfarin; thus, accurate international normalized ratio (INR) monitoring is critical. For expediency and patient comfort, blood sampling from the haemodialysis access or circuit is commonly performed. Point-of-care (POC) INR machines allow both peripheral vein preservation and rapid results, yet are not validated in the haemodialysis population. METHODS: A prospective cohort study in haemodialysis patients taking warfarin was undertaken. Three paired samples were drawn over a single session: peripheral bl

Warfarin dose reduction vs watchful waiting for mild elevations in the international normalized ratio.

BACKGROUND: Whether clinicians should decrease the warfarin dose in response to a mild, asymptomatic elevation in the international normalized ratio (INR) is unknown. OBJECTIVES: The study objectives were as follows: (1) to evaluate the safety of an anticoagulation service (ACS) policy advocating that the warfarin dose not be changed for isolated, asymptomatic INRs of < or = 3.4; (2) to compare the dosing strategies of an ACS and primary care providers (PCPs); and (3) to quantify the relationship between reduction of the warfarin dose and the subsequent fall in the INR. Prothrombin Time-International Normalized Ratio Predicts the Outcome of Atrial Fibrillation Patients Taking Rivaroxaban.

Background: Although direct oral anticoagulants (DOACs) for patients with atrial fibrillation (AF) are considered to be safe, over or under anticoagulation and increased bleeding or thromboembolic risk are still considered individually. We aimed to investigate whether there is an association between prothrombin time and international normalized ratio (PT-INR) or activated partial thromboplastin time (aPTT) ratio, and the risks of ischemic stroke/systemic embolism (IS/SE) and major bleeding among AF patients taking rivaroxaban or dabigatran. [Effects of Tramadol Coadministration on Prothrombin Time-International Normalized Ratio in Patients Receiving Warfarin].

Several case studies have reported a possible drug interaction between warfarin and tramadol where tramadol coadministration enhanced the antithrombotic effects of warfarin. To assess this drug interaction, changes in prothrombin time-international normalized ratio (PT-INR) before and after tramadol coadministration were investigated in patients receiving warfarin. For this study, we examined 54 patients (male/female: 22/32, 68.4±12.7 years) who were being treated with warfarin for deep vein thrombosis, atrial fibrillation, arteriosclerosis obliterans, congestive heart failure, and other vascu

Elevated admission international normalized ratio strongly predicts mortality in victims of abusive head trauma.

BACKGROUND: Victims of abusive head trauma have poor outcomes compared with other injured children. There is often a delay in diagnosis because these young patients are unable to communicate with health care providers. These critically injured patients would benefit from early identification and therapy. METHODS: We performed a retrospective review of our single hospital trauma registry from 2005 to 2014. All Level 1 pediatric (age 0-17 years) trauma patients who sustained abusive head trauma were included. Exclusion criteria included no admission coagulation studies, prehospital product tran

Primary breast angiosarcoma associated with abnormalities in international normalized ratio platelet count and anemia.

Angiosarcomas are rare malignancies with poor prognosis and could affect any organ in the body. Breast angiosarcomas mostly affect adolescent and young adult females. Here, we present the case of a 19-year-old female with low grade primary breast angiosarcoma associated with international normalized ratio (INR) abnormalities, thrombocytopenia and anemia. Patient experienced severe bleeding on biopsy. INR and count of blood cells turned to normal levels after mastectomy. Patient refused chemotherapy. Three months later, computed tomography showed liver metastasis.

Thrombin generation and coagulation factor activities: evaluation and comparison with the international normalized ratio.

Oral anticoagulation therapy is monitored effectively by the international normalized ratio (INR). However, INR is perhaps not optimal in predicting risk of complications. Other more sensitive methods have been suggested. The aims of this study were to estimate the variability and association of calibrated automated thrombin generation (CAT) and clotting activity of coagulation factors II, VII, IX and X and to compare these parameters with the INR measured in a central laboratory as well as using portable coagulometers. Twenty-four patients on stable oral anticoagulation therapy with coumarins

Prognostic Value of International Normalized Ratio-to-Albumin Ratio and Ferritin Level in Chronic Liver Patients with Hepatocellular Carcinoma.

OBJECTIVE: Hepatocellular carcinoma (HCC) is the sixth among the most common cancers and the fourth among cancer-related causes of death in the world. In the evaluation of liver function in HCC patients, parameters such as albumin-bilirubin, prothrombin time-international normalized ratio (PT-INR) to albumin ratio (PTAR) are used among new methods other than Child-Pugh and MELD scores. Biomarkers are widely used in clinical practice in cases such as diagnosing various diseases, evaluating treatment response and predicting prognosis. We aimed to evaluate the prognostic role of serum ferritin an

The influence of antiphospholipid antibodies on prothrombin time and international normalized ratio: a scoping review.

Antiphospholipid antibodies (aPL) may interfere with prothrombin time (PT) and international normalized ratio (INR) assays. Patients with antiphospholipid syndrome (APS) are often treated with vitamin K antagonists and require therapeutic monitoring with the INR. However, it remains unclear to what extent these assays are influenced by aPL and the consequent clinical relevance. This scoping review aimed to map the available evidence on the impact of APS and aPL on PT/INR assays, describe the methods used in research in this area, and identify gaps to guide future investigations.
