Ectopic pregnancy among non-medicated and copper-containing intrauterine device users: a 10-year follow-up.

OBJECTIVE: To compare ectopic pregnancy (EP) rates among users of non-medicated and copper-containing IUDs. STUDY DESIGN: The number of women with non-medicated and copper devices were 3491 and 11,682, respectively. Cumulative woman-months of use (CWMU), Pearl Index (PI) and gross cumulative lifetable EP rates for 1000 women were calculated. RESULTS: The numbers of users at the end of the 10th year were 550 and 569 for non-medicated and copper-containing IUDs, respectively. CWMU for plastic and copper devices was 191,678 and 473,533, respectively, at the 169th month of maximum follow-up. The

Placement of an intrauterine device within 48 hours after second-trimester medical abortion: a randomized controlled trial.

BACKGROUND: International guidelines recommend placement of intrauterine devices immediately after second-trimester medical abortion, but evidence concerning the optimal time for intrauterine device placement is lacking from clinical trials. OBJECTIVE: This study aimed to investigate effectiveness, safety, and acceptability of intrauterine device placement within 48 hours, compared with placement at 2 to 4 weeks after second-trimester medical abortion. We hypothesized that intrauterine device placement within 48 hours would be superior compared with placement at 2 to 4 weeks after the abortio

Immediate placement of intrauterine device after second-trimester medical abortion-Secondary outcomes with one-year follow-up.

INTRODUCTION: Intrauterine devices (IUDs) effectively prevent unwanted pregnancies. Little is known about long-term outcomes of women choosing an IUD after second-trimester medical abortion. MATERIAL AND METHODS: We performed a 12-month follow-up of a multicenter randomized controlled trial comparing IUD placement within 48 h (intervention) with placement at 2-4 weeks (control) after medical abortion with 85 to 153 days' gestation. Secondary outcomes such as proportions of long-term IUD use, subsequent pregnancies and abortions, and participant satisfaction are presented (ClinicalTrials.gov N

Immediate versus delayed insertion of the copper intrauterine device after medical abortion at 17-20 gestational weeks: a randomised controlled trial.

INTRODUCTION: This trial reports on use of the copper intrauterine device (IUD) after immediate compared with delayed insertion following medical abortion at 17-20 gestational weeks (GW). METHODS: This randomised controlled trial was conducted at one tertiary hospital and five community healthcare centres in Cape Town, South Africa. Eligible consenting women were randomised to immediate (within 24 hours) or delayed (3 weeks post-abortion) insertion of the copper IUD. Follow-up was at 6 weeks, 3 months and 6 months. Main outcomes were use of the original IUD and use of any IUD, including repla

Lost levonorgestrel IUD: diagnosis and therapy.

A lost intrauterine device (IUD) is an uncommon event. Recently, a new levonorgestrel-releasing intrauterine system was introduced. In view of several cases of an unusual diagnostic discrepancy in the location of a lost levonorgestrel-releasing IUD in our department, we sought to determine if the extrauterine location of lost levonorgestrel-releasing IUDs differs from that of lost copper IUDs. The medical files of all patients with a diagnosis of lost IUD who were admitted to Rabin Medical Center from 2000 to 2003 were reviewed. Fourteen women were identified, 9 with levonorgestrel-releasing I

Endometrial morphological changes in IUD users: a review.

The introduction of an intrauterine device into the uterine cavity induces a foreign body reaction in the surrounding endometrium which is characterized by the infiltration of polymorphonuclear leucocytes and macrophages into the endometrial stroma and subsequently through the surface epithelium. Leucocyte migration is greater with copper IUDs than with inert IUDs. Ulceration of the surface epithelium, haemorrhage of erythrocytes and microthrombosis of stomach capillaries occur in the functional endometrium in contact with inert and copper IUDs. In endometrium adjacent to, but not in contact w

Management of translocated and incarcerated intrauterine contraceptive devices.

Thirty-seven consecutive patients who reported with absent or snapped strings and failed attempts at removal of intrauterine contraceptive devices (IUCD) by hooks or curette were posted for hysteroscopy/laparotomy following a plain X-ray of the pelvis to exclude unrecognized spontaneous expulsion. Direct visualization of the endometrial cavity was invaluable in locating and removing IUCDs, particularly the embedded and fragmented ones. Translocated medicated devices were associated with dense adhesions, suggesting the need for their prompt removal. There appears to be an increased risk of tran

Improved utilization of spacing methods--intrauterine devices (IUDs) and low-dose combined oral contraceptives (OCs)--through re-orientation training for improving quality of services. Indian Council of Medical Research Task Force on IUD and Hormonal Contraceptives.

A multicentre study was conducted by the Indian Council of Medical Research (ICMR) for improving the utilization of intrauterine devices (IUDs) and oral contraceptives (OCs) at the Post Partum Centres (PPCs) run by District/State Health Authorities. The ICMR's network of Human Reproduction Research Centres (HRRCs) had the technical responsibility for providing re-orientation training to health functionaries (medical and paramedical) of the PPCs for better quality of services and follow-up care for IUD and OC users. The uniform training module was utilized by all HRRCs which specifically assure

Intrauterine devices. The optimal long-term contraceptive method?

OBJECTIVE: To review selected data on the effectiveness, safety, cost and technical ease of intrauterine device (IUD) use compared with Norplant and surgical sterilization. STUDY DESIGN: Literature review. RESULTS: IUDs are highly effective, safe and relatively inexpensive methods of contraception that may offer advantages for some women over other long-term methods, such as sterilization and Norplant. IUDs provide protection against pregnancy comparable to that provided by female sterilization, and they may be more effective than Norplant. IUDs have a long duration of effectiveness: the cop

Diagnostic and therapeutic technology assessment. Intrauterine devices.

The DATTA panelists emphasized the critical importance of patient selection when considering IUDs for contraception. The IUD is an acceptable method of contraception, especially for those women who are in the middle to older reproductive years, unable to take oral contraceptives, in a stable monogamous relationship, and not at risk for sexually transmitted diseases. Within these constraints, the panelists gave overwhelming support to the IUD as a safe and effective method of contraception. The minority opinion (two panelists) that these devices were not established for safety or effectiveness
