Pulsed field ablation (PFA) is emerging as a major advance in cardiac ablation because it induces irreversible electroporation rather than relying primarily on thermal injury. This article provides an overview of the core biophysical principles underlying PFA, focusing on concepts broadly relevant across systems and clinical applications. Externally applied pulsed electric fields interact with biological tissues and affect their transmembrane potential. When a critical threshold is exceeded, nanoscale aqueous pores form in the lipid bilayer, initiating electroporation. Depending on pulse ampli

Pulsed field ablation (PFA) is being adopted into widespread use worldwide for the treatment of a wide range of arrhythmias, with attractive tissue selectivity and safety profiles. While single-shot PFA remains the mainstay of atrial ablation at present, focal applications of PFA are emerging as a highly effective means of delivering effective ablation. The present case illustrates transient atrioventricular block following PFA, highlighting potential for conduction system damage with PFA and supporting a cautious approach in close proximity to conduction system structures.

Pulsed field ablation (PFA) is a novel nonthermal therapy for cardiac arrhythmias. The influence of contact force (CF), contact status, and application count on lesion characteristics in bipolar biphasic circular PFA remains uncertain. This study investigated lesion dimensions and acute histopathological changes using potato and porcine myocardial models. PFA was delivered in potato models under non-contact conditions or stratified contact forces (0–25 g, 25–50 g, > 50 g; 5 applications each). Porcine myocardium lesions were created with 1, 3, or 5 applications under contact or non-contact. Co

Irreversible electroporation (IRE) can be considered as an alternative therapy for pancreatic tumors. However, a previous study revealed that the difference in electrical conductivity between the liver parenchyma and vessels was reported to result in local changes in electric field distribution during IRE, which may contribute to incomplete ablation and tumor recurrence. Therefore, finite element method (FEM) was conducted to examine whether such local changes could also occur in pancreatic tissue. FEM revealed that a similar phenomenon also occurs around the vessels and ducts in pancreatic ti

Irreversible electroporation (IRE) is an emerging tumor ablation modality that employs high-intensity pulsed electric fields to disrupt cell membranes. However, achieving complete tumor destruction while preserving adjacent critical structures remains a major challenge. In this study, we investigated whether high-aspect-ratio silver nanowires (AgNWs) could enhance IRE efficacy in a three-dimensional hydrogel-based cell model. We systematically evaluated cell viability, ablation area, conductivity, and temperature under various pulse parameters and AgNW concentrations. Increased electric field

BACKGROUND: Combining immune checkpoint blockade therapy with operative disruptive immunomodulation using irreversible electroporation may overcome the resistance to systemic therapy found in patients with locally advanced, unresectable pancreatic cancer. We describe the safety profile and efficacy of IRE with nivolumab.
METHODS: In the preclinical phase of study, human pancreatic cell lines were cultured with interferon-γ (10 ng/mL) and murine models of pancreatic cancer were treated with irreversible electroporation and programmed death ligand-1 (PD-L1) expression was measured. In this phas

Pulsed field ablation (PFA) has rapidly emerged as a promising ablation modality for the treatment of cardiac arrhythmias because of its myocardial tissue preferential ablation and favourable safety profile. However, procedural control of lesion formation and reliable assessment of lesion durability remain important challenges. This review summarizes the main determinants of PFA efficacy and current approaches for intraprocedural lesion assessment. Experimental and clinical studies indicate that PFA lesion formation results from the interaction between delivered electrical energy, catheter des

Pulsed field ablation (PFA) is emerging as an alternative to thermal ablation for pulmonary vein isolation (PVI) in atrial fibrillation, although PFA-specific complications have arisen. This review provides an overview of the efficacy and safety of single-shot PFA systems. Of 804 PubMed studies (January 14, 2026), 47 primary PVI studies were included. All PFA systems reported almost 100% acute PVI. The multispline, circular, and spherical catheters showed significant changes in hemolysis biomarkers. However, acute kidney injury occurred only with the pentaspline (n = 39 of 1307) and variable-l

Irreversible electroporation (IRE) utilizes high-voltage pulses to permeabilize cell membranes and induce cancer cell death, a process that releases damage-associated molecular patterns (DAMPs) and triggers immune responses. This study investigated the impact of intracellular DAMPs, extracted following cell treatment with electric pulses and termed electroporation supernatant (EP SN), on the viability of cancerous (4T1) and noncancerous (CHO) cells using an in vitro model. Viability was assessed via MTT, flow cytometry, and clonogenic assays. Protein, RNA, and DNA extraction levels were quanti

Percutaneous irreversible electroporation (IRE) is a nonthermal ablation technique increasingly used for pancreatic malignancies, though rarely reported in pancreatic neuroendocrine tumors (pNETs). We present the case of a 75-year-old man with a well-differentiated grade-1 nonfunctioning pNET treated by percutaneous IRE after refusing surgery. Posttreatment somatostatin receptors PET/CT showed a marked reduction in 68 Ga-DOTATOC uptake at 1 month, followed by stable residual activity at 1 year, despite complete devascularization on contrast-enhanced CT. This case illustrates the potential of I

PURPOSE: Atherosclerotic vascular disease remains a leading cause of morbidity and mortality worldwide. Current treatments such as angioplasty, stenting, and atherectomy are invasive and limited by restenosis, thrombosis, and incomplete long-term efficacy. Pulsed field ablation (PFA), a nonthermal electroporation-based modality, has demonstrated safety in other cardiovascular applications, but it has not been applied for the treatment of endoluminal vascular diseases. We investigated whether pulsed electric fields could be delivered within the coronary artery and if PFA could selectively ablat
