BACKGROUND AND OBJECTIVES: Older kidney transplant recipients are susceptible to cognitive impairment, frailty, comorbidities, immunosuppression-related complications, and chronic graft failure, however, there has been limited focus on their concerns and expectations related to transplantation. This study aims to describe the perspectives of older kidney transplant recipients about their experience of kidney transplantation, self-management, and treatment goals to inform strategies and interventions that address their specific needs.

Kidney transplantation is the gold standard for end-stage renal disease, and robotic kidney transplant is becoming increasingly common globally. There have not been large-scale studies, however, assessing national outcomes of robotic kidney transplant compared to open kidney transplant. This was a retrospective cohort analysis of adults undergoing kidney-only transplant from the National Inpatient Sample, 2016–2022. Comparative analysis was performed using log-transformed linear regression for continuous variables and Rao-Scott chi-square testing for categorical variables. Rates of delayed gra

A profound kidney shortage compromises the effective care of renal failure patients in the United States and other nations. This article discusses the need for kidneys, the procurement of live donor and cadaver organs, and strategies to increase organ donation. It suggests that the kidney shortage can be minimized and perhaps eliminated through the use of living related donors whenever appropriate, and through more efficient procurement of volunteered cadaver kidneys. Two other strategies to increase the supply of kidneys-the sale of organs and "contracting out" laws-are rejected. The first is

Determination of the viability of a transplanted kidney is still an unsolved problem. Apart from immunological effects hemodynamic and microcirculatory changes in the kidney are important. Thus in 39 mongrel dogs renal floow flow (electromagnetic and local renocortical oxygen supply) and microcirculation (by meaning local tissue pO2) were determined before nephrectomy, immediately after revascularization and before sacrification in experiments with autologous transplantation. Whole kidney blood flow decreased by 30-40%. Measurements of tissue pO2 revealed distinct changes in renocortical oxyge

The kidney is an important organ for maintenance of homeostasis in the human body. As renal failure progresses, renal replacement therapy becomes necessary. However, there is a chronic shortage of kidney donors, creating a major problem for transplantation. To solve this problem, many strategies for the generation of transplantable kidneys are under investigation. Since the first reports describing that nephron progenitors could be induced from human induced pluripotent stem cells, kidney organoids have been attracting attention as tools for studying human kidney development and diseases. PURPOSE OF REVIEW: Obesity has reached epidemic proportions in the United States. It is a risk factor for developing, among others, heart disease, stroke, type 2 diabetes, and chronic kidney disease (CKD), and thus a major public health concern and driver of healthcare costs. Although the prevalence of obesity in the CKD/end-stage kidney disease population is increasing, many obese patients are excluded from the benefit of kidney transplant based on their BMI alone. For this reason, we sought to review the experience thus far with kidney transplantation in obese patients and associated outcome

Anemia is a frequent complication in pediatric kidney transplant recipients (KTR) with a variable reported prevalence estimated between 20 and 80% depending on how defined. Causes of and risk factors for post-transplantation anemia (PTA) are multifactorial with iron deficiency being the primary cause of early PTA (within the first 6 months after transplantation) and impaired glomerular filtration rate (GFR) commonly responsible for late PTA (after 6 months). Medications, viral infections, chronic inflammation, and comorbidities also play a role. PTA has relevant long-term consequences and is a

This paper describes robot-assisted kidney transplantation (RAKT) from a living donor. The robot is docked between the parted legs of the patient, placed in the supine Trendelenburg position. Kidney allografts are provided by a living donor. Before vascular anastomosis, the kidney allograft is prepared by inserting a double-J stent in the ureter, and the temperature for the anastomosis is lowered by wrapping it in an ice-packed gauze. A 12 mm or 8 mm port for the robotic camera and three 8 mm ports for robotic arms are placed. A peritoneal pouch is created for the kidney allograft by raising t

Two significant policy changes, one in the way people are put forward for kidney transplants and the other in the way in which kidneys are distributed to people on the waiting list, make the question of whether someone is too frail to receive a transplant all the more relevant, particularly in Rhode Island. An executive order signed by President Donald Trump1 stresses that efforts to treat kidney disease need to concentrate on providing more people with kidney transplants and increasing the number of organs transplanted rather than discarded. An effort to decrease waiting times for kidneys in

Kidney transplantation is the best approach for treating patients with end stage renal disease, offering patients the best chance of returning to normal health. While the techniques used in kidney transplantation surgery are mature and highly successful, there is a severe shortage of donor organs. Statistics show a serious imbalance between organ donations and patients on the waiting list for organ transplantation. Moreover, evidence from empirical studies has shown a better transplantation outcome for patients who receive living donor transplantation than for those who receive organs from cad

The kidney serves as the main clearance organ of our body, filtrating and excreting metabolic waste products. Various intrinsic and extrinsic conditions can lead to kidney injury, roughly 0.1% of the population suffer from end stage renal disease. Renal transplantation reinstitutes an almost normal quality of life; again it is cost effective and thus the preferred treatment of terminal renal failure. miRNAs play pivotal roles in immune responses and inflammation, which makes them particularly interesting in the field of transplantation and in understanding the molecular pathways of allograft p

In the era of calcineurin inhibitors, hypomagnesaemia is a very common finding in kidney transplant recipients. Especially the first weeks after transplantation it is the rule rather than the exception. Hypomagnesaemia or low magnesium intake have been associated with a higher mortality or more cardiovascular events in the general population, but this association has never been explored in kidney transplant recipients, despite their increased cardiovascular risk. Kidney transplant recipients with pre- or post-transplant hypomagnesaemia seem to have an aberrant glucose metabolism and develop di

The discovery of novel classes of non-coding RNAs (ncRNAs) has revolutionized medicine. Long thought to be a mere cellular housekeeper, surprising functions have recently been uncovered. MicroRNAs (miRNAs), are a representative of the class of short ncRNAs, play a fundamental role in the control of DNA and protein biosynthesis and activity as well as pathology. Currently, miRNAs are being investigated as diagnostic and prognostic markers and potential therapeutic targets in kidney transplantation for such indolent processes as ischaemia-reperfusion injury, humoral rejection or viral infections

Kidney transplantation is the preferred modality of renal replacement therapy. Long-term patient and graft survival have only improved marginally over the recent decade, mainly because of the development of cardiovascular disease after transplantation. Obesity is a risk factor for cardiovascular disease and is common before and after transplantation. This article reviews the literature assessing the role of pre- and post-transplant obesity on patient and graft survival, discusses the underlying obesity-related mechanisms leading to inferior kidney transplant outcomes, and explores the role of

Owing to the shortage of deceased donors in Japan, since 1989, we have performed ABO-incompatible kidney transplantation (ABO-IKTx) to expand the indication for living donor kidney transplantation. During the past two decades, about 2000 ABO-IKTxs were performed. Since 2001 the success rate for these kidney transplants has reached 96% for 1-year, 91% for 5-year and 83% for 9-year graft survival, similar to outcomes of ABO-compatible kidney transplantation (ABO-CKTx). This dramatic improvement in results means that ABO-IKTx has become accepted as a therapeutic alternative for end-stage renal fa

Cytopenias frequently occur during the first months after solid organ transplantation. Many mechanisms are involved but drugs toxicity and infections are the major causes of cytopenias. Anemia is also related with chronic kidney graft dysfunction. Several drugs are pointed out but antithymocyte globulin, antiproliferative drugs and antiviral drugs are mainly responsible for cytopenias. Infectious causes are mainly viral and can rarely induce macrophage activation syndromes. Passenger lymphocyte syndrome is only described after ABO incompatible transplantations. Thrombotic microangiopathies are
