Mononuclear cells from patients with sarcoidosis respond to Kveim tissue suspensions in vivo and from granulomas. We attempted to develop an in vitro Kveim test by adding Kveim material to monocytes and lymphocytes from patients with sarcoidosis and observing the cells in tissue culture. Intracutaneous Kveim test and measurement of serum mitochondrial antibodies were performed in a group of patients with primary biliary cirrhosis (PBC) and in a group of patients with sarcoidosis. Sarcoidosis is a granulomatous disorder of unknown origin. The sarcoid spleen-derived Kveim-Siltzbach test reagent (KSTR) elicits a sarcoid-specific, granulomatous response and was used to diagnose sarcoidosis. A young man had a mild, slightly progressive pulmonary sarcoidosis. After 4 years he developed an acute disease with splenomegaly, anemia, marked elevation of ESR, hypercalcemia and mild renal insufficiency. The anemia and ESR elevation disappeared after splenectomy, whereas the hypercalcemia still needs corticosteroid treatment. Attempts to withdraw this treatment resulted in recurrence of the hypercalcemia, but no other abnormalities. The immunological aspects of lepra (India, Germany) and Sarcoidosis are presented under consideration of cutaneous reaction to lepromin and Kveim-extracts. Striking differences were observed between the visceral and cutaneous responses after tests with validated Kveim and normal spleen suspensions in a guinea pig model of granulomatous bowel disease. In the Institute for Pulmonary Diseases and Tuberculosis, Novi Sad, a total number of 1500 patients was tested with a Kveim antigen prepared in this institute. Out of 146 cases, 92 (64%) cases with active sarcoidosis showed granulomas in biopsy at 4-6 weeks after Mantoux test (MT). No granuloma was seen in anyone of the 162 sites injected with other allergens in 27 of MT site positive cases, serving as self-control. False positive biopsies were not seen. Another control group of 40 patients with other respiratory disorders like tuberculosis failed to produce any granulomatous response at the site of MT. Mantoux test site biopsy may prove a valuable diagnostic test in sarcoidosis in India.

Sarcoid granulomata were found incidentally in the mammary lobules adjacent to an excised fibroadenoma in a case of sarcoidosis of the breast. The diagnosis of sarcoidosis was established by the radiological finding of bilateral hilar lymphadenopathy, raised concentrations of serum angiotensin converting enzyme and lysozyme, and, finally, by a positive Kveim test.

The authors report a case of inflammatory plasma cell pseudo-tumor in the lung associated with mediastino-pulmonary sarcoid histological lesions. Diffusion of the latter to pulmonary parenchyma, the intensity of the follicular reaction in mediastinal lymph nodes together with a positive Kveim test suggest the fortuitous association of a mediastinal-pulmonary sarcoidosis with a plasma cell granuloma.

In 25 of 35 cases of acne conglobata, a reduction in or lack of delayed reactivity was found by intracutaneous tests with a battery of antigens. On the basis of these findings as well as of normal T-cell function in lymphocyte culture and of negative Kveim-tests (in three cases), the possible immunological mechanisms are discussed.
