Transfusion-transmitted cytomegalovirus infection in the era of universal leukoreduction and pathogen inactivation: a contemporary review.

INTRODUCTION: Transfusion-transmitted cytomegalovirus remains a clinically significant risk in immunocompromised individuals, neonates, and transplant recipients. Despite widespread universal leukoreduction and pathogen-reduction technologies, uncertainty persists regarding residual risk and the role of CMV-seronegative blood components. This review summarizes the epidemiology, diagnostics, and prevention of transfusion-transmitted CMV. METHODS: A review of the international literature was conducted using PubMed, Scopus, and transfusion medicine databases. Studies on CMV seroprevalence, trans

Impact of leukoreduction on the metabolome of ovine packed red blood cells during refrigerated storage.

BACKGROUND: Blood transfusion is a life-saving intervention for many species of veterinary interest, including sheep. Despite extensive research on the impact of refrigerated storage of packed red blood cells (pRBC) in humans, research on the quality of stored ovine blood is limited and storage guidelines are mostly informed by studies in humans. Human pRBC are currently stored without residual white blood cells, following selective removal of the leukocytes by filtration (leukoreduction). This process delays the onset and mitigates the progression of the storage lesion, a series of molecular

Measurements of human herpesvirus 8 viral load in blood before and after leukoreduction filtration.

BACKGROUND: Human herpesvirus 8 (HHV-8) is likely transmitted through blood transfusion in high-prevalence areas. The efficacy of leukoreduction filtration for reducing HHV-8 in blood has not been reported. STUDY DESIGN AND METHODS: Blood was drawn from 45 human immunodeficiency virus-positive men either with Kaposi's sarcoma (KS; n=21) or without KS (n=24) and subject to leukoreduction filtration. HHV-8 viral load was measured in plasma and in blood before and after filtration. RESULTS: Twelve subjects, all with KS, had detectable HHV-8 viremia before filtration with viral loads of 10(2) to

Blood donor white blood cell reduction filters as a source of human peripheral blood-derived endothelial progenitor cells.

BACKGROUND: Neovascularization in tumors, wounds, and sites of ischemic injury occurs by both angiogenesis (proliferation from existing vessels) and by vasculogenesis (differentiation into endothelial cells from circulating endothelial progenitor cells [EPCs]). EPCs can be obtained from marrow, from cord blood, or by ex vivo expansion of human peripheral blood (PB). The ease of obtaining human PB EPCs has led many recent studies to utilize PB EPCs. The ability to obtain large numbers of PB EPCs would greatly facilitate characterization to further our understanding of EPC biology and their appl

In vitro and in vivo evaluation of leukoreduced red blood cells after automated separation of whole blood.

BACKGROUND: Leukoreduced red blood cells (LR-RBCs) prepared from whole blood (WB) with the Reveos™ Automated Blood Processing System (Reveos System) are required to meet US Food and Drug Administration (FDA) criteria for RBC component in vitro quality and in vivo 24-h RBC recovery after 42-day storage. STUDY DESIGN AND METHODS: For the in vitro study, 65 Reveos-derived LR-RBC units were stored for 42 days in citrate phosphate dextrose (CPD)/Additive Solution Formula 5 (AS-5). Units were evaluated for post-filtration residual white blood cell (rWBC) content, post-filtration RBC recovery, and p

Sickle cell trait results in a high leukoreduction quality control failure rate for whole blood donations.

BACKGROUND: Prior studies have shown that sickle cell trait (SCT) is the most common reason attributed to leukoreduction (LR) filter failure due to physical blockage. However, current Food and Drug Administration (FDA) guidelines do not require blood collectors to take a specific action to mitigate inadequate LR that may occur among donors with SCT. We sought to determine the scope of inadequate LR among whole blood (WB) donations collected from individuals with SCT and processed under standard manufacturing conditions. STUDY DESIGN AND METHODS: Between 8/2021 and 2/2022, a total of 40 red bl

Managing leukocytosis in malignant pertussis with white blood cell depletion: An effective alternative way to exchange transfusion.

Pertussis is a common infectious disease affecting both children and adults. In young infants, it can cause severe respiratory illness associated with prolonged hospitalization and high mortality, particularly in cases complicated by severe leukocytosis and lymphocytosis, which increase the risk of pulmonary hypertension and multiorgan failure. Exchange transfusion remains the traditional first-line treatment for severe pertussis-associated hyperleukocytosis. However, in centers with appropriate resources and expertise, leukapheresis may be considered as an alternative therapeutic option. Leuk

Guideline development for prevention of transfusion-associated graft-versus-host disease: reduction of indications for irradiated blood components after prestorage leukodepletion of blood components.

Transfusion-associated graft-versus-host disease (TA-GVHD) is a rare, commonly fatal complication of transfusion preventable by irradiation of blood units. The revision of the Dutch transfusion guideline addressed the question whether irradiation is still necessary if blood components are prestorage leukodepleted. We searched for published cases of TA-GVHD following transfusion of prestorage leukodepleted blood and through contacting haemovigilance systems. Six presumed cases were found, dating from 1998 to 2013. Four out of six patients had received one or more non-irradiated units despite re

Molecular surgery for the treatment of malignant tumors: bioincompatible material apheresis for cancer therapy?

Controlled immunological shock, induced by bioincompatible material apheresis for cancer (BIC MAC) therapy, produces an immunoactive status in experimental subjects. However, in order to provide a safe, painless, effective, and reproducible BIC MAC therapy, it is mandatory to provide general anesthesia with endotracheal intubation not only during apheresis procedures of 1-h duration but also for an additional 5 h. Using this procedure, there was no mortality experienced during animal experiments. Also, there were no procedurally related physical or sensory abnormalities demonstrated. This gene

Comparison of Hemostatic Activity in Canine Leukoreduced Cryoprecipitate, Cryopoor Plasma, and Fresh Plasma.

BACKGROUND: To date, no studies have reported the evaluation of hemostatic activity in canine leukoreduced cryoprecipitate (LR-CRYO) and leukoreduced cryopoor plasma (LR-CPP). OBJECTIVES: We aimed to compare the hemostatic activity of LR-CRYO and LR-CPP to leukoreduced fresh plasma (LR-FP) and to evaluate the preservation of LR-CRYO by refrigeration and refreezing after thawing. METHODS: Four hundred fifty milliliters of fresh blood was collected from ten donor dogs, leukoreduced, and separated into LR-FP, then frozen (-20°C) to obtain leukoreduced fresh frozen plasma (LR-FFP). LR-FFP was fu

A positive clinical experience of a long-term treatment pattern with leukocyte apheresis and Adacolumn in ulcerative proctitis.

Ulcerative proctitis (UP) is often involved in the diagnosis of inflammatory bowel disease (IBD). The increase of leukocytes and pro-inflammatory factors in peripheral blood and in the active forms, as well as the infiltration of neutrophils and monocytes/macrophages in the intestinal mucosa is known to occur in this entity. This infiltration of cells damages the mucosa due to the liberation of proteases, oxidation radicals and cytokines, among others. Apheresis techniques such as leukocyte apheresis may be used among the different therapeutic options such as steroids, sulfasalazine, 5-aminosa

A new preparation method for red blood cells for intrauterine transfusion enabling reduction of donor exposure.

BACKGROUND: Severe hemolytic anemia of the fetus, caused by maternal red blood cell (RBC) alloantibodies, is treated with intrauterine transfusion (IUT) of RBCs. Because IUT is associated with additional antibody formation, RBCs with the closest match between donor and mother are preferred. Because one fetus needs a median of three IUTs, finding such RBCs is complicated. Collection of repeated low-volume donations from one selected donor during the entire IUT treatment period would reduce donor exposure and possibly IUT-associated alloimmunization. STUDY DESIGN AND METHODS: Whole blood (WB) d

Leukoreduction and blood-borne vCJD transmission risk.

PURPOSE OF REVIEW: Risk assessments for transmission of variant Creutzfeldt-Jakob disease predicted that leukocyte reduction would be inefficient at preventing transmission of the disease by blood transfusion. Nevertheless, approximately 14 years ago, a significant proportion of European countries decided to implement leukocyte reduction treatment within their human blood supplies in order to mitigate the risk of human prion disease transmission. RECENT FINDINGS: Current epidemiological studies seem to indicate that leukocyte reduction has had a positive impact on reducing the risk. In additi

Introduction of universal prestorage leukodepletion of blood components, and outcomes in transfused cardiac surgery patients.

OBJECTIVE: To assess whether introduction of universal leukodepletion (ULD) of red blood cells (RBCs) for transfusion was associated with improvements in cardiac surgery patient outcomes. METHODS: Retrospective study (2005-2010) conducted at 6 institutions. Associations between leukodepletion and outcomes of mortality, infection, and acute kidney injury (AKI) were modeled by logistic regression, and intensive care unit length of stay (LOS) in survivors was explored using linear regression. To examine trends over time, odds ratios (ORs) for outcomes of transfused were compared with nontransfus

Leukoreduction filtration of whole-blood units from sickle trait donors: effects of a metered citrate anticoagulant system.

BACKGROUND: Polymerization of hemoglobin (Hb) S is exacerbated by acidic and hyperosmotic citrate anticoagulant solutions and often results in occlusion of leukoreduction filters by red blood cells (RBCs) from sickle cell trait (Hb AS) donors. This study evaluates a blood collection instrument that adds citrate anticoagulant in a metered fashion, thus mitigating adverse citrate effects. STUDY DESIGN AND METHODS: Collection of whole blood by a metered anticoagulant system was compared to traditional phlebotomy in 12 Hb AS and 12 non-sickle trait (Hb AA) donors. Each donated twice; on one occas

Prevention of ischemia/reperfusion-induced pulmonary dysfunction after cardiopulmonary bypass with terminal leukocyte-depleted lung reperfusion.

OBJECTIVE: Pulmonary ischemia and reperfusion during routine open heart surgery with cardiopulmonary bypass can lead to pulmonary dysfunction and vasoconstriction, resulting in a high morbidity and mortality. We investigated whether ischemia/reperfusion-induced pulmonary dysfunction after full-flow cardiopulmonary bypass could be prevented by the infusion of leukocyte-depleted hypoxemic blood during the early phase of reperfusion (terminal leukocyte-depleted lung reperfusion) and whether the benefits of this method were nullified by using hyperoxemic blood for reperfusion. METHODS: Twenty-one

Hyperleukocytosis in infant acute leukemia: a role for manual exchange transfusion for leukoreduction.

BACKGROUND: Hyperleukocytosis is a serious, life-threatening complication of pediatric acute leukemia that can cause neurologic injury, pulmonary leukostasis, metabolic derangements, and coagulopathy. Acute leukemia has the highest risk of mortality and morbidity at presentation when associated with hyperleukocytosis. Infant leukemia presents unique challenges and treatment considerations due to the disease itself and size and overall health of the patient. While medical management of hyperleukocytosis in older patients with acute leukemia has been described, including cytoreductive procedures

Removal of exogenous (spiked) and endogenous prion infectivity from red cells with a new prototype of leukoreduction filter.

BACKGROUND: Two recent probable cases of transmission of a variant of human Creutzfeldt-Jakob disease (vCJD) through blood transfusion suggest that the disease can be transmitted through transfusion of blood components from presymptomatic blood donors. In the absence of a preclinical screening test, removal of the infectious agent by processing is the only means by which risk to recipients of blood from donors with inapparent vCJD infections can be eliminated. STUDY DESIGN AND METHODS: In the endogenous infectivity study, a pool of 500 mL of whole blood was collected into CP2D anticoagulant f

Comparison of a novel viscous platelet additive solution and plasma: preparation and in vitro storage parameters of buffy-coat-derived platelet concentrates.

BACKGROUND AND OBJECTIVES: We developed a viscous platelet additive solution (PAS) based on MacoPharma's SSP+ but containing hydroxyethyl starch to address the poor osmotic balance and low yield associated with conventional PAS for the storage of buffy-coat platelet concentrates (PC). MATERIALS AND METHODS: Pools of four buffy-coats were made into leucoreduced PCs (n = 5) suspended either in plasma or viscous PAS. After determination of platelet recoveries, the PCs were stored under standard conditions. On days 1, 2, 3, 5, 7 and 9, PCs were tested for mean platelet volume, platelet concentrat

Blood donor and component management strategies to prevent transfusion-related acute lung injury (TRALI).

OBJECTIVE: Discuss the pros and cons of using donor and blood product-management strategies to prevent transfusion-related acute lung injury (TRALI). DATA SOURCE: A review of the literature was performed. RESULTS: Despite therapeutic advances in pulmonary and critical care medicine, TRALI is now considered to be one of the leading causes of transfusion-associated mortality, and thus determining how to prevent TRALI is extremely important. Donor and product-management strategies to prevent this life-threatening condition have been suggested, but because of gaps in our understanding of TRALI,
