Blood pressure variability and ambulatory monitoring.

The inherent variability of blood pressure means that the accuracy of a small number of clinic readings in estimating the true blood pressure is limited. In many patients, a clinic visit provokes an increase in blood pressure such that they may be misclassified as being hypertensive (white coat hypertension). This applies to about 20% of hypertensives. Ambulatory monitoring has the potential to overcome these limitations. The white coat effect may still occur in patients taking antihypertensive medications, although in some cases medication may lower the clinic pressure without affecting the a

Ambulatory blood pressure monitoring: practical considerations.

Ambulatory blood pressure monitoring (ABPM) allows one to evaluate the blood pressure (BP) profile over a 24-hour period in the patient's natural environment. Casual pressure measurements in the physician's office can be affected by alarm reactions, thus causing "white coat" hypertension. ABPM allows one to evaluate these reactions and determine the average pressure and variability of BP along with the effects of physical activity and emotional arousal on BP patterns while at work, at home, and during sleep. Average pressures determined by ABPM are more predictive of target organ involvement a

Sudden death during longterm ambulatory monitoring.

Ten instances of sudden cardiac death documented with longterm ambulatory electrocardiography are described. All had coronary artery disease. The lethal arrhythmia was ventricular fibrillation in seven, bradyarrhythmia ending in asystole in two, and electromechanical dissociation in one. There was no specific arrhythmia pattern in the preceding hour of the terminal event and no predictable electrical trigger for the lethal arrhythmia. The literature on the subject has been reviewed. It appears from the information available that larger series of documented sudden death cases are needed if one

Immediate Ambulatory Electrocardiographic Monitoring in Syncope.

BACKGROUND: In patients with syncope who present to the emergency department for evaluation, diagnosing an underlying cardiac arrhythmia remains difficult. METHODS: We conducted an open-label, randomized, controlled trial at 45 hospitals in the United Kingdom to assess the effects of 14-day cardiac monitoring on diagnosis, treatment, and outcomes among patients with syncope. Adults with syncope that remained unexplained after an evaluation in the emergency department were assigned in a 1:1 ratio to undergo 14-day ambulatory electrocardiographic (ECG) monitoring (the intervention group) or to

Ambulatory blood pressure monitoring: which arm?

To determine the effects of routinely selecting the nondominant arm for ambulatory blood pressure monitoring (ABPM) on estimates of patients' blood pressure (BP) and to evaluate the practise of using manual BP from one arm and ambulatory BP from the other on the estimation of white coat effect (WCE), an observational study was conducted in 10 volunteers, exhibiting an interarm resting clinic systolic BP (SBP) difference > or =10 mm Hg. The main outcome measures were: (i) average ambulatory SBP measured on right and left arm simultaneously during 24 h, and (ii) estimate of WCE derived, by curre

Ambulatory blood pressure monitoring in hypertension.

Compared with blood pressure measurements in the office setting, ambulatory blood pressure monitoring offers a more accurate and reliable method of diagnosing and managing hypertension. Indications for ambulatory blood pressure monitoring include borderline hypertension, a discrepancy between home and office blood pressure measurements, persistent blood pressure elevation in the office with no end-organ damage, episodic elevations of blood pressure or suggestive symptoms, hypertension resistant to treatment, episodic angina not related to exercise, end-organ damage but normal office blood pres

[Reproducibility of ambulatory blood pressure monitoring].

Short-term blood pressure variability has been studied extensively with monitoring techniques. To assess whether or not and to what extent average 24-hour blood pressure varies when repeatedly recorded, 12 mild to moderate hypertensive subjects underwent 3 non-invasive blood pressure monitorings at monthly intervals. When the average 24-hour blood pressure of the whole group was evaluated no substantial differences were found between the 3 recordings, even though slightly lower blood pressure values were recorded at the first monitoring. However, when the within-period comparison was studied i

Sudden death during ambulatory Holter monitoring.

Four cases of sudden cardiac death during ambulatory Holter monitoring are described. All had coronary arterial disease. Two patients were on antiarrhythmic drug therapy and both had a prolonged QTc-interval on their resting electrocardiogram. The predominant rhythm was sinus rhythm in all. In one patient, severe bradycardia terminated in asystole. In the remaining 3 patients, ventricular flutter (which was initiated in 2 instances by a short run of polymorphous ventricular tachycardia) degenerated into ventricular fibrillation. The lethal event was triggered once by an early cycle ventricular

Indications and techniques for ambulatory electrocardiogram monitoring.

The development of the ability to record a patient's ECG during a full 24-hour day of normal activities has resulted in improved arrhythmia detection and potential for treatment. Improved methods of ECG data processing, using computer techniques, now allows quantitation of PVC frequency and improved correlation of the arrhythmia with events occurring during the period of recording. Knowledge of the natural occurrence and variability of ambulatory arrhythmias permits appropriate choice and timing of antiarrhythmic therapy.

Ambulatory blood pressure monitoring in arterial hypertension.

The latest guidelines from both the European Society of Hypertension (ESH) and the European Society of Cardiology (ESC) emphasize ambulatory 24-h ambulatory blood pressure monitoring (ABPM) as a complementary and additive tool for the diagnosis and management of arterial hypertension. ABPM offers a comprehensive assessment of various blood pressure (BP) parameters, making it a crucial tool for clinicians. It enables them to develop tailored treatment strategies, improve BP control, and reduce cardiovascular risk. However, interpreting ABPM results can be challenging, especially for clinicians

Heart Failure Transitional Care and Ambulatory Monitoring.

Chronic heart failure is characterized by periods of stability punctuated by clinical decline often requiring hospitalization. Following hospitalization, patients remain at an increased risk of morbidity and mortality, often requiring rehospitalization due to recurrent deterioration. Transitional care interventions aim to improve the movement from the inpatient to ambulatory setting. Promising interventions, such as patient education, novel uses of loop diuretics, and invasive hemodynamic monitoring, have been developed over the past 20 years all with varying degrees of success.

Ambulatory blood pressure monitoring profiles in Asia.

High blood pressure (BP) remains a major health problem globally, with a proportion of hypertension-mediated organ damage (HMOD) increasing worldwide including in Asia region. Cardiovascular (CV), cerebral and kidney diseases related to hypertension were reported to be closely associated with morning surge and nocturnal hypertension-a subset of BP variability-which can be detected by out-of-office BP measurement. Ambulatory BP monitoring (ABPM) and Home BP monitoring (HBPM) have been recommended by major guidelines to be used in the evaluation of BP variability and outcomes' prediction of hype

Twenty-Four-Hour Ambulatory Blood Pressure Monitoring.

The diagnosis, management, and estimated mortality risk in patients with hypertension have been historically based on clinic or office blood pressure readings. Current evidence indicates that 24-hour ambulatory blood pressure monitoring should be an integral part of hypertension care. The 24-hour ambulatory monitors currently available on the market are small devices connected to the arm cuff with tubing that measure blood pressure every 15 to 30 minutes. After 24 hours, the patient returns, and the data are downloaded, including any information requested by the physician in a diary. The most

Ambulatory blood pressure monitoring in clinical practice.

Being one of the most widely prevalent diseases throughout the world, hypertension has emerged as one of the leading causes of global premature morbidity and mortality. Hence, blood pressure (BP) measurements are essential for physicians in the diagnosis and management of hypertension. Current American College of Cardiology/American Heart Association (ACC/AHA) guidelines recommend initiating antihypertensive medications on the basis of office BP readings. However, office BP readings provide a snapshot evaluation of the patient's BP, which might not reflect patient's true BP, with the possibili

Managing hypertension with ambulatory blood pressure monitoring.

There has been a dramatic shift in the manner in which blood pressure (BP) is measured to provide far more comprehensive clinical information than that provided by a single set of office BP readings. Extensive clinical and epidemiological research shows an important role of ambulatory BP monitoring (ABPM) in the management of hypertensive patients. A 24-h BP profile helps to determine the absence of nocturnal dipping status and evaluate BP control in patients on antihypertensive therapy. The ability to detect white-coat or masked hypertension is enhanced by ambulatory BP monitoring. In 2001, t

Technologies for developing ambulatory cough monitoring devices.

Cough is a prevailing symptom in most lung diseases. While cough sounds themselves can be very instrumental in the diagnosis of certain diseases, their intensity and frequency also infers the intensity of the particular illness. There is an imperative need for a robust system for identifying and analyzing cough sounds. In implementing such systems, researchers are confronted with technical challenges such as the choice of sensors and methods of signal acquisition, the real time analysis of the acquired signals, and the accurate identification of cough events, distinguishing them from similar s

Side effects of ambulatory blood pressure monitoring.

OBJECTIVE: To study the experiences and complaints of patients who underwent 24 h blood pressure monitoring. METHODS: Two groups of hypertensive patients of a tertiary outpatient clinic were asked to fill in a nine-item questionnaire about the side effects of ambulatory blood pressure monitoring (ABPM). The first group (n=75) used the auscultatory Oxford Medilog device (OM) and the second group (n=110) the oscillometric Spacelabs 90207 (SL). RESULTS: The OM showed significantly less error readings (OM versus SL, 9 versus 19%; P=0.005) but was also less tolerated by the subjects. Sleep distur

Current status of ambulatory blood pressure monitoring.

During the past few years, 24 h ambulatory blood pressure (BP) monitoring has become an essential tool in the management of hypertensive patients. Individuals who exhibit increases in BP only in the office setting (white coat hypertension) can now avoid unnecessary drug therapy. For patients with treated hypertension, ambulatory BP monitoring has provided a means to detect individuals at increased risk of experiencing a cardiovascular event independent of the office BP reading. Alternatives to BP monitoring, such as self-measurement in the home, do exist, but the interpretation of findings rep
