Neurologic Deficits Following Oral Misuse of the Nasal Decongestant Propylhexedrine.

INTRODUCTION: Propylhexedrine is an over-the-counter nasal decongestant used recreationally for amphetamine-like effects. Prior reports have associated intravenous misuse with brainstem dysfunction and diplopia. This is a case of propylhexedrine ingestion resulting in neurological impairment with associated MRI findings. CASE REPORT: A 21-year-old man presented with neurologic symptoms after ingesting propylhexedrine extracted from a 250 mg nasal inhaler into a lemon juice solution following recommendations from an online forum. He also followed recommendations to simultaneously ingest 1 g L-

Phenylpropanolamine's decongestive effect on the nasalmucosa of pregnant women with nasal stuffiness.

Pregnancy rhinitis is common and very troublesome for many women. Today, no safe and effective treatment is available for this condition. The aim of this placebo-controlled double-blind study was to evaluate the decongestive effect of phenylpropanolamine (PPA 50 mg) twice daily for seven days in 38 women with pregnancy rhinitis. In the morning, before starting the course of treatment, and two to three hours after taking the last dose of the study-medicine in the morning on the eighth day, recordings of the position of the nasal mucosal surface were made with rhinostereometry. Every evening, th

Relationship between rhinitis duration and response to nasal decongestion test.

OBJECTIVES/HYPOTHESIS: Nasal obstruction depends on allergic inflammation. Decongestion tests evaluate the reversibility of nasal airflow limitation. It has been previously reported that duration of persistent allergic rhinitis (PER) may involve important functional consequences. The purpose of the study was to evaluate the impact of the duration of rhinitis on the response to nasal decongestion test in a cohort of patients with PER. METHODS: A total of 312 patients with moderate-severe PER were prospectively and consecutively evaluated: 234 males and 78 females, mean age 23.6 years. A detail

Efficacy of a topical nasal decongestant in different formulations: the effect of viscosity.

Factors affecting the absorption of drugs through the nasal mucosa are increasingly being studied. Dosage form factors, i.e. the viscosity of the excipients, have been claimed to be of importance for enhanced nasal absorption due to prolongation of contact time between drug and mucosa. In the present study, a comparison was made of the effect of a local nasal sympathomimetic when administered intranasally in formulations with different viscosities. Volunteers received the test drugs as nasal sprays in a randomized cross-over design by a rhinomanometric technique, the effect of the drug being r

Assessment of the Prevalence of the Use of Nasal Decongestants Among the General Population in Saudi Arabia.

Background A topical nasal decongestant (NDC) is widely prescribed in ENT practice and used as self-medication because it is available over the counter, which makes it an easily accessible medication. Due to its common and long-term use, it is associated with serious nasal complications. It is commonly self-administrated in many otolaryngology diseases like the common cold, sinusitis, and acute or chronic rhinitis. The long-term usage of nasal decongestants is associated with significantly increased side effects. Aim To assess the prevalence of the usage of nasal decongestants among the genera

Efficacy of a timed-release antihistamine/decongestant tablet for symptoms of nasal allergy.

In a double-blind study, a timed-release tablet containing carbinoxamine maleate 8 mg and pseudoephedrine hydrochloride 120 mg was compared with placebo for the treatment of signs and symptoms of nasal allergy. Ninety-four adults with rhinitis caused by grass or ragweed allergy were paired according to severity of symptoms and nasal congestion, then assigned randomly to drug or placebo. After baseline measurements were taken, three doses of drug or placebo were given at 12-hour intervals. The active drug was significantly better than placebo in relieving the following symptoms: nasal congestio

Topical Nasal Decongestant Oxymetazoline: Safety Considerations for Perioperative Pediatric Use.

The over-the-counter nasal decongestant oxymetazoline (eg, Afrin) is used in the pediatric population for a variety of conditions in the operating room setting. Given its vasoconstrictive properties, it can have cardiovascular adverse effects when systemically absorbed. There have been several reports of cardiac and respiratory complications related to use of oxymetazoline in the pediatric population. Current US Food and Drug Administration approval for oxymetazoline is for patients ≥6 years of age, but medical professionals may elect to use it short-term and off label for younger children in

Underwater nasal decongestant use: a novel approach to middle ear equalization.

Middle ear barotrauma is the most common diving-related injury. It is estimated to occur in more than 50% of experienced divers. Although divers learn how to effectively equalize their ears with various maneuvers, airway congestion may impede the ability to equalize the middle ear space via the Eustachian tube. In this case, one may have to avoid diving or abort a dive due to inability to descend. If difficulty with middle ear equalization occurs during the bottom phase of the dive, which may transpire during a multilevel cave or wreck dive, a diver may need to descend before he can exit the w

Decongestion test in patients with allergic rhinitis: functional evaluation of nasal airflow.

BACKGROUND: Rhinomanometry measures nasal airflow that is frequently impaired in allergic rhinitis. Decongestion tests consist of spraying an intranasal vasoconstrictor drug to evaluate the reversibility of nasal airflow limitation. The aim of this study was to evaluate the decongestion test in patients with seasonal allergic rhinitis (SAR) caused by pollen sensitization, perennial allergic rhinitis (PAR) caused by sensitization to perennial allergens only, or mixed allergic rhinitis (MAR) caused by sensitization to both allergens. METHODS: One hundred twenty-three subjects (112 men and 11 wo

Effects of decongestion on nasal cavity air conditioning efficiency: a CFD cohort study.

Decongestion reduces blood flow in the nasal turbinates, enlarging the airway lumen. Although the enlarged airspace reduces the trans-nasal inspiratory pressure drop, symptoms of nasal obstruction may relate to nasal cavity air-conditioning. Thus, it is necessary to quantify the efficiency of nasal cavity conditioning of the inhaled air. This study quantifies both overall and regional nasal air-conditioning in a cohort of 10 healthy subjects using computational fluid dynamics simulations before and after nasal decongestion. The 3D virtual geometry model was segmented from magnetic resonance im

Use of peak nasal inspiratory flowmetry and nasal decongestant to evaluate outcome of septoplasty with radiofrequency coblation of the inferior turbinate.

Immunotherapy has revolutionized cancer treatment, yet only a minority of individuals respond clinically, necessitating alternative strategies that can benefit these patients. Novel immuno-oncology targets may achieve this through bypassing resistance mechanisms to standard therapies. We introduce Mining Immunotherapy Drug tArgetS (MIDAS), a multimodal graph neural network system for immuno-oncology target discovery. MIDAS leverages gene interactions, multi-omic patient profiles, immune cell biology, antigen processing, disease associations and phenotypic consequences of genetic perturbations.

Visual hallucinations in a toddler: accidental ingestion of a sympathomimetic over-the-counter nasal decongestant.

Hallucinations are uncommonly encountered in the young child. The differential diagnosis of such behavior includes a number of potentially serious syndromes such as central nervous system malignancy, encephalitis, temporal lobe epilepsy, closed head trauma with frontal lobe confusion, hypoglycemia, drug intoxications, and childhood psychiatric syndromes. Organic explanations are most often discovered and frequently involve toxicologic causes. A case is presented of a toddler with hallucinations caused by inappropriately high doses of pseudoephedrine.

[Adverse cardiovascular and central neurologic reactions to sympathomimetics used as nasal decongestants: results of the French National Pharmacovigilance Survey].

Remedies for coughs and colds often contain both an indirect sympathomimetic agent plus another drug (anti-inflammatory, analgesic or antibiotic, etc.). In France, oral sympathomimetics are available over the counter, and preparations for topical use (nasal sprays or drops) require a medical prescription. Using the French Pharmacovigilance database, we performed a retrospective study to identify cardiovascular and neurological adverse drug reactions (ADRs) associated with the use of nasal decongestants containing a sympathomimetic agent. Of a total of 165 ADRs. 15 were strokes or cerebral haem

Cardiovascular and Cerebrovascular Events Linked to Abuse and Misuse of Sympathomimetic Nasal Decongestants: A Narrative Review of Clinical Evidence.

BACKGROUND: Sympathomimetic nasal decongestants such as pseudoephedrine, phenylephrine, oxymetazoline, and xylometazoline are commonly used to relieve nasal blockage caused by allergic rhinitis, infections, or sinus inflammation. They work by activating α-adrenergic receptors, which promote vasoconstriction. However, overuse or inappropriate use of these medications has been linked to serious cardiovascular and cerebrovascular side effects. OBJECTIVE: The purpose of this review is to compile the available clinical evidence on the misuse and overuse of sympathomimetic nasal decongestants and t

Utilization Pattern and Related Knowledge of Nasal Decongestants Among the General Population in the Al-Qassim Region, Saudi Arabia.

BACKGROUND: Nasal congestion, a common symptom of various upper respiratory conditions, is often treated with nasal decongestants, which are effective vasoconstrictors used to relieve blockage in conditions like allergic rhinitis, rhinosinusitis, and nasal polyps. However, frequent use of these decongestants can lead to both local and systemic side effects. METHODS: This cross-sectional study used an online, self-administered questionnaire to collect data from our population as regard on the inclusion and exclusion criteria. Inclusion criteria encompassed all consenting individuals aged 10 ye

A double-blind controlled evaluation of the nasal decongestant effect of Day Nurse in the common cold.

A rhinomanometric technique was used to evaluate the nasal decongestant effect of a single dose of Day Nurse over a 4-hour post-dosing period in human volunteers suffering from nasal congestion due to a common cold. A significant improvement in nasal congestion was obtained at the 1 hour, 2 hour, 3 hour and 4 hour post-dosing interval, from initial values compared to a placebo group. The study therefore demonstrated by an objective technique that Day Nurse provides relief of nasal congestion and related symptoms in cold sufferers.
