Defining technical competence in olecranon fracture fixation: an international Delphi consensus on tension band wiring assessment.

BACKGROUND: Tension band wiring (TBW) is a widely used technique for displaced olecranon fractures but carries a substantial risk of complications, often due to technical errors. Despite being a designated core competency in orthopedic curricula, no standardized, procedure-specific tool exists to assess technical performance in TBW. This study aimed to establish international expert consensus on assessment parameters for evaluating surgical competence in TBW of simple transverse olecranon fractures. METHODS: A modified 4-round Delphi process was conducted with 98 orthopedic trauma surgeons, p

Olecranon Fractures: A Critical Analysis Review.

»: Olecranon fractures account for 10% of all elbow fractures and are more likely to result from a low-energy injury. A displaced fracture with a stable ulnohumeral joint (Mayo type 2) is the most common type of injury. »: The management of an isolated olecranon fracture is based on patient factors (age, functional demand, and if medically fit to undergo surgery) and fracture characteristics including displacement, fragmentation, and elbow stability. »: Nonoperative management can be successfully used in undisplaced fractures (Mayo type 1) and in displaced fractures (Mayo type 2) in frail pa

Modified tension band wiring technique by safely inserting K-wires in olecranon fracture osteosynthesis.

AIM: The present study presents the results of a modified tension band technique by surgically inserting K-wires to treat olecranon fractures. MATERIALS AND METHODS: The modification includes inserting the K-wires from the olecranon's upper tip and directing them to the ulna's dorsal surface. Twelve patients (three males and nine females) from 35 to 87 years of age were operated for olecranon fracture. After the standard approach, the olecranon was reduced and fixed with two K-wires from the tip to the dorsal ulnar cortex. Then the standard tension band technique was carried out. RESULTS: Th

Biomechanical study on different internal fixation methods for treating Mayo type IIA olecranon fractures of the ulna.

The aim of this experiment is to compare the biomechanical strength of six distinct internal fixation techniques for Mayo type IIA olecranon fractures using biomechanical analysis. This study utilized tensile tests on artificial, shape-mimicking olecranon bones to assess their biomechanical properties. A tensile test was performed on the artificial, shape-mimicking olecranon bone at a 90° angle, with the tensile load applied at a rate of 2 mm/min until the test displacement reached 2 mm, at which point the test was halted. Throughout the test, the testing system was able to collect load and di

Comparison of intramedullary fixation techniques in the maintenance of functional anatomy of the elbow after fixation of the proximal ulna.

The objective of this study was to compare intramedullary (IM) nail and IM screw fixation for reattachment of the proximal ulna. Preserved elbow anatomy served as the primary outcome and was defined as the distance between the coronoid process and the olecranon. A retrospective cohort study of 31 patients treated with IM fixation of the proximal ulna was performed. Radiographs were used to compare displacement distances between the coronoid process and the olecranon, with average follow-up of approximately 5 months. IM nail fixation corresponded to a mean displacement of -0.65 mm between the o

Anatomical variations of the semilunar notch in elbow dislocations.

The anatomical relations of the semilunar notch of the ulna were studied in radiographs, taken in a strict lateral view, from 100 patients with elbow dislocations. The results were compared with those from a control group of 150 patients without dislocation or other radiographic pathology. We found that the coronoid process and an angle (a) formed between the axis of the ulna and a line going through the tip of the coronoid process and the tip of the olecranon were significantly smaller than those in the control group (P = 0.02). The central angle of the semilunar notch was significantly large

Cable versus cerclage wire in weber tension band fixation for transverse olecranon fractures: a biomechanical study in a synthetic ulna model.

Stable fixation of displaced simple transverse olecranon fractures is required to maintain reduction under triceps traction and enable early elbow motion. Although Weber tension band fixation is widely used, the biomechanical influence of tension band material (cable vs cerclage wire) across different pin/anchorage designs remains unclear. This study compared resistance to fracture-line separation among six Weber-based constructs in a standardized synthetic ulna model. Sixty commercially available synthetic ulnae were osteotomized to create a transverse olecranon fracture model (Mayo type IIA)

Olecranon Stress Fracture.

Olecranon stress fractures are a rare upper extremity fracture that primarily affects throwing athletes. The incidence of olecranon stress fractures are increasing owing to the number of patients playing and the volume of engagement in competitive sports, especially in the pediatric population. However, olecranon stress fractures can present a challenge from a management and a rehabilitation perspective owing to their vague presentation, thereby affecting how these patients are diagnosed and managed. Therefore, it is imperative to further evaluate the disease process, diagnosis, and treatment

Adult Monteggia and Olecranon Fracture Dislocations of the Elbow.

Monteggia fractures and olecranon fracture dislocations represent complex injuries with distinct patterns of bony and soft tissue involvement. Fractures of the proximal ulna and olecranon process may lead to disruption of the proximal radioulnar joint and/or ulnohumeral joint. The keys to treatment are recognition of the pattern of injury and formation of an algorithmic surgical plan to address all components of the injury process. Complications are common and may be related to the injury spectrum itself and/or inadequate fracture alignment or fixation.

Idiopathic or traumatic olecranon bursitis. Clinical features and bursal fluid analysis.

Thirty cases of idiopathic olecranon bursitis were studied. Most had previous local trauma. The process was unilateral and often associated with nontender pitting edema in cases of short duration. Ten patients exhibited a bony spur at the olecranon process, and amorphous calcific deposits were seen in 6. The bursal fluid was hemorrhagic with a xanthochromic supernatant, and the mucin clot test was poor or fair. Leukocyte count averaged 878/mm3, predominantly mononuclears. Many cells contained inclusion bodies. Glucose, total protein, and complement (C3) concentration averaged 80, 60, and 60% o

A novel fragment specific classification of complex olecranon fractures: 3-dimensional model design, radiological validation, and proposed surgical algorithm.

BACKGROUND: Current classifications for proximal ulna fracture patterns rely on qualitative data and cannot inform surgical planning. We propose a new classification system based on a biological and anatomical stress analysis. Our hypothesis is that fragment types in complex fractures can be predicted by the tendon and ligament attachments on the proximal ulna. METHODS: First, we completed a literature review to identify quantitative data on proximal ulna soft tissue attachments. On this basis, we created a 3-dimensional model of ulnar anatomy with SliceOMatic and Catia V5R20 software and det

Olecranon fractures: A biomechanical comparison of three tension band wiring fixation methods on bone models.

PURPOSE: This study aimed to compare the biomechanical pull-out strength of the three different tension band wiring (TBW) methods employed to fix transverse olecranon fractures on bone models. METHODS: Three different fixation models were created in groups of seven synthetic olecranon fractured bone models. The first technique was fixed the olecranon with the traditional TBW method. The second technique was fixed the olecranon with a large intramedullary screw TBW method. The third technique was fixed the olecranon with the double-screw TBW method. The pull-out force needed for the failure of

Ideal pin length and interval in tension band wiring using ring pins for transverse olecranon fractures: a biomechanical study.

BACKGROUND: Several clinical and biomechanical studies on tension band wiring (TBW) using a ring-pin system have been conducted, but no consensus has been reached on the ideal surgical technique. In this study, we aimed to determine the ideal interval and length of ring pins for the treatment of transverse olecranon fractures using TBW with a ring-pin system. METHODS: A biomechanical study was performed using 32 fourth-generation composite ulnae and a ring-pin system specially designed for TBW. Four groups of eight sawbones were created based on the interval and length of the ring pins. A cyc

Locked Tension Band Wiring for Mayo IIA Olecranon Fractures: Modified Surgical Technique and Retrospective Comparative Study of Clinical Outcomes and Cost-effectiveness with Locking Plate.

Background: Tension band wiring (TBW) has traditionally been used for simple olecranon fractures, but due to its many complications, locking plate (LP) is increasingly being employed. To reduce the complications, we developed a modified technique for olecranon fracture repair, locked TBW (LTBW). The study aimed to compare (1) the frequency of complications and re-operations between LP and LTBW techniques, (2) clinical outcomes and the cost efficacy. Methods: We retrospectively evaluated data of 336 patients who underwent surgical treatment for simple and displaced olecranon fractures (Mayo Typ

Controversies in Fractures of the Proximal Ulna.

The olecranon process, coronoid process, and greater sigmoid notch are important components of the complex proximal ulna. Along with providing bony stability to the ulnohumeral joint, the proximal ulna serves as the attachment site of many important muscles and ligaments that impart soft tissue stability to the elbow joint. Management of proximal ulnar fractures continues to evolve as advances in imaging and anatomic and biomechanical studies have led to improvements in available implants; however, controversies remain, as shown in the current relevant literature.
