[Acute respiratory failure--support of gas exchange using extracorporeal or implanted oxygenators--present status and future development].

In acute respiratory failure gas exchange can be supported or even maintained in an "alternative" way to mechanical ventilation using extracorporeal techniques (extracorporeal membrane oxygenation ECMO, extracorporeal CO2-removal ECCO2R), or intravenacaval oxygenators (IVOX). These techniques, which are currently in use in neonatology, pediatrics, and adult intensive care medicine, or techniques at present in clinical evaluation (IVOX), are reviewed with their indications, contraindications, differences, problems, worldwide results, and possible future applications.

Hypobaric type oxygenators during cardiopulmonary bypass for cardiac surgery to reduce gaseous microemboli: A randomized controlled trial.

OBJECTIVES: Neurological complications after cardiac surgery have been associated with gaseous microemboli generated during cardiopulmonary bypass. We undertook a trial to assess whether a novel dual-chamber oxygenator, designed to denitrogenate the blood, could reduce cerebral gaseous microemboli compared with conventional cardiopulmonary bypass. METHODS: In this single-center, single-blind, parallel-group randomized controlled trial, adults undergoing elective or urgent coronary artery bypass grafting with or without aortic valve replacement (±coronary artery bypass grafting) were assigned

Prolonged Use of Extracorporeal Membrane Oxygenators for COVID-19-Associated Acute Respiratory Distress Syndrome: A Retrospective Analysis.

Whether an anticoagulation strategy combining bivalirudin and aspirin during extracorporeal membrane oxygenation (ECMO) would prolong oxygenator use is unknown. No clear data exist on oxygenator life span during prolonged ECMO use. We evaluated 70 adult patients who received ECMO due to coronavirus disease 2019 (COVID-19)-associated acute respiratory distress syndrome for at least 7 days and who required no or at least one ECMO oxygenator replacement due to ECMO-circuit thrombosis. Anticoagulation parameters mainly included activated partial thromboplastin time (aPTT), with monitoring of inter

Accumulations of von Willebrand factor within ECMO oxygenators: Potential indicator of coagulation abnormalities in critically ill patients?

Clot formation within membrane oxygenators (MOs) remains a critical problem during extracorporeal membrane oxygenation (ECMO). The composition of the clots-in particular, the presence of von Willebrand factor (vWF)-may be an indicator for prevalent nonphysiological flow conditions, foreign body reactions, or coagulation abnormalities in critically ill patients. Mats of interwoven gas exchange fibers from randomly collected MOs (PLS, Maquet, Rastatt, Germany) of 21 patients were stained with antibodies (anti-vWF and anti-P-selectin) and counterstained with 4',6-diamidino-2-phenylindole. The ext

A narrative review of the technical standards for extracorporeal life support devices (pumps and oxygenators) in Europe.

This review summarizes the European rules to control the market when introducing new products. In particular, it shows all the steps to achieve the European Conformity (CE Mark), a certification that all new medical products must achieve before being used in Europe. Extracorporeal membrane oxygenation (ECMO) devices are exposed to the same procedures. Hereby, we present some regulatory issues regarding pumps and oxygenators, providing technical details as released by the manufacturers on their websites and information charts.

Successful Management of Acute Type A Aortic Dissection in a 305 kg Patient Using Two Oxygenators in Parallel.

Patient factors can present formidable challenges in emergent aortic surgery. We report successful mechanical aortic root, ascending aorta, and Zone 2 arch replacement under hypothermic circulatory arrest in a 305 kg patient with acute type A aortic dissection. A dual-oxygenator bypass circuit, proactive metabolic management, bilateral fasciotomies, and early renal replacement therapy were critical to a favorable outcome.

A Validated CFD Model for Gas Exchange in Hollow Fiber Membrane Oxygenators: Incorporating the Bohr and Haldane Effects.

Chronic respiratory diseases claim nearly four million lives annually, making them the third leading cause of death worldwide. Extracorporeal membrane oxygenation (ECMO) is often the last line of support for patients with severe lung failure. Still, its performance is limited by an incomplete understanding of gas exchange in hollow fiber membrane (HFM) oxygenators. Computational fluid dynamics (CFD) has become a robust oxygenator design and optimization tool. However, most models oversimplify O2 and CO2 transport by ignoring their physiological coupling, instead relying on fixed saturation cur

Water Condensation and Gas Exchange Correlation in Different Models and Fibers of Blood Oxygenators: "How Can We Improve Performance?".

Creation of water condensation in blood oxygenators is a phenomenon that is constantly present during cardiopulmonary bypass and in medium- to long-term extracorporeal life support. Clinical observation of condensation at the oxygenator exit is still a common event normally associated with sudden cooling of the gas flow proximal to the outlet cover (after exiting the fiber bundle), where the warming effect of blood is no longer present. Condensation could progressively obstruct a certain number of fibers, reducing the efficiency of gaseous exchange in the membrane of the oxygenator surface. Th

Incidence of neutrophil extracellular traps (NETs) in different membrane oxygenators: pilot in vitro experiments in commercially available coated membranes.

Neutrophil extracellular traps (NETs) were detected in blood samples and in cellular deposits of oxygenator membranes during extracorporeal membrane oxygenation (ECMO) therapy and may be responsible for thrombogenesis. The aim was to evaluate the effect of the base material of gas fiber (GF, polymethylpentene) and heat exchange (HE) membranes and different antithrombogenic coatings on isolated granulocytes from healthy volunteers under static culture conditions. Contact of granulocytes with membranes from different ECMO oxygenators (with different surface coatings) and uncoated-GFs allowed det

Post-Operative Delirium and Cognitive Dysfunction in Aged Patients Undergoing Cardiac Surgery: A Randomized Comparison between Two Blood Oxygenators.

In elderly patients undergoing cardiac surgery, extracorporeal circulation affects the incidence of post-operative delirium and cognitive impairment with an impact on quality of life and mortality. In this study, a new oxygenator system (RemoweLL 2) was tested against a conventional system to assess its efficacy in reducing the onset of postoperative delirium and cognitive dysfunction and the levels of serum inflammatory markers. A total of 154 patients (>65 y.o.) undergoing cardiopulmonary bypass (CPB) were enrolled and randomly assigned to oxygenator RemoweLL 2 (n = 81) or to gold standard d

α-Fe2O3@Au-PEG-Ce6-Gd Nanoparticles as Acidic H2O2-Driven Oxygenators for Multimodal Imaging and Synergistic Tumor Therapy.

Nanoparticles with visual imaging capabilities and synergistic therapeutics have a bright future in antitumor applications. However, most of the current nanomaterials lack multiple imaging-guided therapeutic capabilities. In this study, a novel enhanced photothermal photodynamic antitumor nanoplatform with photothermal imaging, fluorescence (FL) imaging, and MRI-guided therapeutic capabilities was constructed by grafting gold, dihydroporphyrin Ce6, and Gd onto α-iron trioxide. This antitumor nanoplatform can convert NIR light into local hyperthermia at a temperature of up to 53 °C under NIR li

A Pressure Difference-Based Strategy for Blood Oxygen Control in Membrane Oxygenators: Reduced Modeling, Computational Simulation, and Exploratory In Vivo Evaluation.

PURPOSE: This study investigated whether blood oxygenation in a membrane oxygenator can be regulated by modulating the gas-blood pressure difference. METHODS: This study combined theoretical analysis, reduced computational simulation, and exploratory in vivo animal testing. A conceptual three-compartment framework based on Dalton's law, Fick's law, Henry's law, and the Hill equation was used to organize the main oxygen transfer pathway from the gas phase to plasma and red blood cells. For numerical analysis, this conceptual framework was reduced to a steady-state lumped oxygen balance model t

Oxygen metabolism and oxygenation of the newborn.

The premature infant is to some extent protected from hypoxia, however defense against hyperoxia is poorly developed. The optimal assessment of oxygenation is to measure oxygen delivery and extraction. At the bedside PaO2 and SpO2 are approximations of oxygenation at the tissue level. After birth asphyxia it is crucial to know whether or not to give oxygen supplementation, when, how much, and for how long. Oxygen saturation targets in the delivery room have been studied, but the optimal targets might still be unknown because factors like gender and delayed cord clamping influence saturation le

Impaired hemostasis in mechanical circulatory support systems: Monitoring with T-TAS® 01, in vitro correction with VWF concentrates, and impact of membrane oxygenators.

BACKGROUND: Mechanical circulatory support (MCS) systems assist patients with severe cardiac or respiratory failure, but bleeding linked to acquired von Willebrand disease remains a major complication. The T-TAS® 01 system has emerged as a rapid tool for assessing MCS patients' hemostasis. OBJECTIVES: To evaluate hemostatic changes during and after MCS support. Secondary objectives included assessing the impact of the in vitro addition of von Willebrand factor concentrates and exploring differences according to the presence of a membrane oxygenator (MO). METHODS: This prospective, bicentric

On the pulmonary toxicity of oxygen: III. The induction of oxygen dependency by oxygen use.

Oxygen is central to the development of neonatal lung injury. The increase in oxygen exposure of the neonatal lung during the onset of extrauterine air breathing is an order of magnitude, from a range of 10-12 to 110-120Torr. The contributions of oxygen and the volume and pressure relationships of ventilatory support to lung injury are not easily distinguished in the clinical setting. Sequential changes in inspired air or 100% oxygen were studied in 536 newborn rabbits without ventilatory support. Bilateral cervical vagotomies (BCV) were performed at 24h post natal to induce ventilatory distre
