BACKGROUND: Management of superior sulcus non-small cell lung cancer (NSCLC) continues to evolve, with preoperative chemoradiotherapy (CXRT) followed by surgical intervention supported by several studies. Rates of pathologic complete response (pCR) have varied, and previous attempts to identify clinical predictors of pCR have been unsuccessful. We analyzed our experience with resected superior sulcus NSCLC to elucidate clinical features suggestive of pCR among patients after preoperative treatment.
METHODS: A multiinstitutional retrospective review was performed in patients who underwent rese

Pathologic complete response (pCR) to neoadjuvant chemoradiation (CRT) in patients with rectal cancer is associated with improved prognosis, whereas postoperative surgical complications have been linked with poor oncologic outcomes. Our objective was to examine the association between postoperative complications and pCR. We analyzed 127 patients enrolled in a prospective multicenter study investigating rectal cancer response to CRT. Surgical complications were scored according to the Clavien-Dindo scale (Grade 1 to 4). Among the 127 patients analyzed, 28 (22%) patients had a pCR. In the pCR gr

A mediastinal mass can present a diagnostic challenge, especially when symptoms necessitate urgent treatment. Mediastinal seminoma, a rare extragonadal germ cell tumour, shares clinical features with testicular seminoma, such as slow growth and high sensitivity to chemotherapy and radiotherapy. This case is of a man in his 40s presenting with cough and chest discomfort, with imaging revealing a large anterior mediastinal mass that was causing left diaphragmatic paralysis due to phrenic nerve involvement. Urgent CT of chest, abdomen and pelvis and tissue biopsy supported the diagnosis of medias

RATIONALE AND OBJECTIVES: Pretreatment prediction of pathologic complete response (pCR) is clinically important for guiding neoadjuvant chemotherapy (NAC), but robust multi-center benchmarks with standardized evaluation protocols remain limited. We evaluated a Vision Transformer (ViT) approach for pCR prediction from breast dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI).
MATERIALS AND METHODS: We used BreastDCEDL, a public multi-center dataset comprising 2070 patients from the I-SPY1, I-SPY2, and Duke cohorts with predefined splits. Pre-contrast, early post-contrast, and late

Tumor lysis syndrome (TLS) is an oncologic emergency characterized by metabolic abnormalities resulting from rapid tumor cell breakdown. It is most frequently observed following cytotoxic therapy in hematologic malignancies and is rarely encountered spontaneously in solid tumors. Spontaneous TLS associated with ovarian carcinoma remains exceptionally uncommon. A 53-year-old woman with no pertinent medical history presented with progressive abdominal distension, dyspnea, and oliguria. Laboratory evaluation demonstrated acute kidney injury with a serum creatinine of 4.09 mg/dL, accompanied by hy

BACKGROUND: Pathologic complete response after neoadjuvant treatment in pancreatic ductal adenocarcinoma is a rare occurrence. Similar to other malignancies, achieving a pathologic complete response in pancreatic ductal adenocarcinoma seems to correlate with improved survival. However, because of the rarity of such events, the true significance of pathologic complete response in pancreatic cancer remains unclear. The aim of the present study was to investigate the impact of pathologic complete response on survival and recurrence.
METHODS: In a single-center retrospective study, pathologic com

A 77-year-old female patient presented with a medical history of 4 cancerous lesions, each with a surgical history. She was referred to our hospital due to anemia. Upon examination, she was diagnosed with transverse colon cancer. Duodenal invasion was suspected, which made performing R0 surgery difficult; therefore, the NAC approach was chosen. Three courses of CAPOX were administered, resulting in tumor obstruction, leading to the formation of an ileum stoma. MSI testing revealed MSI-H, and pembrolizumab treatment was initiated. CT scans showed tumor shrinkage, and PET scans indicated no accu

BACKGROUND: Recent trials have suggested the feasibility of performing a sentinel lymph node biopsy (SNB) following neoadjuvant chemotherapy (NAC). The selection of suitable patients for this approach remains controversial. We developed a predictive model to identify patients most likely to benefit from SNB following NAC.
METHODS: The National Cancer Data Base was used to identify patients with clinically node positive (cN+) breast cancer undergoing NAC followed by breast surgery and axillary lymph node dissection (ALND). Patients were randomly assigned to a 70% testing or 30% validation coho

INTRODUCTION: Factors influencing mortality among pancreatic ductal adenocarcinoma (PDAC) patients who achieve pathologic complete response (pCR) following neoadjuvant therapy have not been previously well studied.
METHODS: Adult PDAC patients (2006-2021) who received neoadjuvant were identified in the National Cancer Database. Tumor response was categorized as pCR if final specimen was pT0N0. Patients with positive margins and stage IV disease were excluded.
RESULTS: Of 13,089 eligible patients, there were 207 patients who achieved pCR. The 3-year overall survival was 75% [69-81] for the en

OBJECTIVE: Accurate prediction of pathological complete response (pCR) to neoadjuvant chemotherapy (NAC) is essential for treatment decision-making in breast cancer. The value of integrated prediction models combining interim magnetic resonance imaging (MRI) findings with clinicopathological factors remains uncertain. This study aimed to compare interim and post-NAC MRI in predicting pCR and to assess the performance of prediction models integrating MRI and clinicopathological variables.
MATERIALS AND METHODS: We retrospectively analyzed 249 patients with early-stage breast cancer who underwe

BACKGROUND/AIM: Enfortumab vedotin plus pembrolizumab (EVP) is the preferred first-line treatment for locally advanced or metastatic urothelial carcinomas. However, the optimal therapy duration and the role of consolidative surgery in patients achieving a complete response remain unclear.
CASE REPORT: A 75-year-old Japanese man presented with intermittent gross hematuria and was diagnosed with a locally advanced lower ureteral carcinoma with regional lymph node metastasis (cT4N2M0). First-line systemic EVP therapy was initiated. After six cycles, radiologic assessment demonstrated a partial r

Vermillion lip squamous cell carcinoma (SCC) is a rare cancer, currently grouped together with the cutaneous lip under the cutaneous squamous cell carcinoma (cSCC) system. Herein, we present a case of an 81-year-old male with locally advanced lower lip SCC involving the vermillion who achieved a complete pathologic response to neoadjuvant cetuximab after the failure of the programed cell death protein-1 (PD-1) inhibitor, cemiplimab. He was followed with clinical observation, with special attention to skin/mucosal surfaces. At the 24-week follow-up visit, he had no signs of recurrence and was v

BACKGROUND: Pathologic complete response after neoadjuvant therapy is an important prognostic indicator for locally advanced rectal cancer and may give insights into which patients might be treated nonoperatively in the future. Existing models for predicting pathologic complete response in the pretreatment setting are limited by small data sets and low accuracy.
OBJECTIVE: We sought to use machine learning to develop a more generalizable predictive model for pathologic complete response for locally advanced rectal cancer.

OBJECTIVES: Nonoperative management (NOM) of locally advanced rectal cancer is an emerging approach allowing patients to preserve their anal sphincter. Identifying clinical factors associated with pathologic complete response (pCR) is essential for physicians and patients considering NOM.
MATERIALS AND METHODS: In total, 412 locally advanced rectal cancer patients were included in this retrospective analysis. Tumor volumes were derived from pretreatment MRI. Clinical parameters such as tumor volume, stage, and location were analyzed by univariate and multivariate analysis, against pCR. A rece

PURPOSE: To assess whether 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors, or statins, might enhance the efficacy of neoadjuvant chemoradiation in rectal cancer.
METHODS AND MATERIALS: Between 1996 and 2001, 358 patients with clinically resectable, nonmetastatic rectal cancer underwent surgery at Memorial Sloan-Kettering Cancer Center after neoadjuvant chemoradiation for either locally advanced tumors or low-lying tumors that would require abdominoperineal resection. We excluded 9 patients for radiation therapy dose <45 Gy or if statin use was unknown, leaving 349 evaluable patien

OBJECTIVE: To review whether pathological complete response (pCR) in esophageal adenocarcinoma (AC) may be regime-dependent and to outline its implications for surveillance, adjuvant therapy and trial design.
BACKGROUND: ESOPEC established perioperative FLOT as superior to preoperative CROSS for survival and distant disease control in resectable esophageal AC, but neither analysis stratified outcomes by pCR status. To our knowledge, this is the first synthesis focused on the prognostic meaning of pCR by treatment modality.
METHODS: Semi-structured search of MEDLINE, Embase, Cochrane Library,

Pathologic complete response (pCR) in bladder cancer, most often defined as pT0N0 at radical cystectomy (RC) following transurethral resection of the bladder tumor with/without neoadjuvant therapy, has emerged as a powerful prognostic marker. Predicting pCR before RC and understanding its prognostic nuances are increasingly important as perioperative chemotherapy and immunotherapy evolve. In this study, we reviewed the contemporary evidence regarding definitions and clinical contexts of pCR in bladder cancer; clinical, pathological, imaging, molecular, and liquid-biopsy predictors of pCR; and

While recent advances in chemotherapy for pancreatic cancer have improved treatment outcomes, pathological complete responses(pCR)remain rare. Here, we present four cases of pancreatic cancer in which a pCR was achieved following chemotherapy. Three cases were classified as borderline resectable and one was resectable. Two patients received gemcitabine+nab-paclitaxel, 1 received mFOLFIRINOX, and one received gemcitabine+S-1 therapy. Histopathological examination after resection revealed only fibrosis and scarring. No tumor cells were found in any of these cases, leading to diagnoses of pCR. Tw
