Prospective assessment of dorsal cheilectomy for hallux rigidus using a patient-reported outcome score.

Compared with other surgical procedures for hallux rigidus, dorsal cheilectomy involves relatively less bone removal, maintains joint motion, and leaves the potential for further salvage surgery. The Manchester-Oxford Foot and Ankle Questionnaire (MOXFQ) has a maximum score of 64 (worst foot health) and has been endorsed by the British Foot and Ankle Society to measure surgical outcome. We prospectively assessed patient-reported outcomes after dorsal cheilectomy for hallux rigidus using the MOXFQ. Patients were deemed suitable for dorsal cheilectomy if they had painful restriction of terminal

[Patient orientation and reference to the ICF as challenges in outcome assessment in rehabilitation].

Measuring the results of rehabilitation interventions presents a number of issues regarding content and method, two of which have been selected for discussion in view of the findings of current research--the significance of patient orientation and the relation to the International Classification of Functioning, Disability and Health, ICF. Compilation of patients' treatment goals, expectations and preferences, patient participation in the development of assessment instruments, compilation of patients' evaluations of treatment results and consideration of the subjectivity of the physician as wel

Potential of a Novel Patient-Reported Outcome Assessment Method Utilizing Illustrations in Rheumatoid Arthritis.

OBJECTIVES: We investigated whether the "Okomarigoto sheet," a novel patient-reported outcome measure that utilizes illustrations, correlates with disease activity and the Health Assessment Questionnaire Disability Index (HAQ-DI) in rheumatoid arthritis. METHODS: The Okomarigoto sheet comprises five items each for morning stiffness, pain, and fatigue. Individual items are scored as follows: severe symptoms = 2, mild symptoms = 1, no symptoms = 0. The total score ranges from 0 to 30; higher scores indicate greater symptom burden. Correlations between Okomarigoto sheet scores, disease activity

Systematic literature review and assessment of patient-reported outcome instruments in sickle cell disease.

BACKGROUND: Sickle cell disease (SCD) is a chronic condition associated with high mortality and morbidity. It is characterized by acute clinical symptoms such as painful vaso-occlusive crises, which can impair health-related quality of life (HRQL). This study was conducted to identify validated patient-reported outcome (PRO) instruments for use in future trials of potential treatments for SCD. METHODS: A systematic literature review (SLR) was performed using MEDLINE and EMBASE to identify United States (US)-based studies published in English between 1997 and 2017 that reported on validated PR

Patient-reported outcome assessment in vasculitis may provide important data and a unique perspective.

OBJECTIVE: To assess patients' self-estimates of the burden of disease in vasculitis and to compare data across patient populations from the US, Germany, and the UK. Outcome assessment in vasculitis primarily focuses on physician evaluations of disease activity and damage. Little is known about the patients' perspectives regarding the burden of disease. METHODS: Patients ranked (scale 0-5) a list of vasculitis-related items to estimate a combination of pain, interference with daily function, discomfort and/or annoyance, anxiety/psychological impact, and medical importance. Patients were also

Angioedema due to acquired C1 inhibitor deficiency: patient experience, conceptual disease model, and assessment of patient-reported outcome measures.

BACKGROUND: Angioedema due to acquired C1 inhibitor deficiency (AAE-C1INH) is a rare, serious condition that manifests with recurring and often painful swelling attacks. No approved treatments or validated patient-reported outcome (PRO) measures exist for AAE-C1INH. This study aimed to develop a conceptual model of AAE-C1INH and explored the relevance of adapting PRO measures validated for hereditary angioedema, another bradykinin-mediated disease. METHODS: This cross-sectional, qualitative study involved semi-structured interviews with 8 adults living with AAE-C1INH. Open-ended questions eli

Computerized-adaptive testing versus short forms for pediatric inflammatory bowel disease patient-reported outcome assessment.

INTRODUCTION: Computerized-adaptive testing (CAT) may increase reliability or reduce respondent burden for assessing patient-reported outcomes compared with static short forms (SFs). We compared CAT versus SF administration of the Patient-Reported Outcomes Measurement Information System® (PROMIS®) Pediatric measures in pediatric inflammatory bowel disease (IBD). METHODS: Participants completed 4-item CAT, 5- or 6-item CAT, and 4-item SF versions of the PROMIS Pediatric measures. We compared average T-scores, intra-class correlations (ICCs), floor and ceiling effects, and standard error of mea

Preferred Language, Patient-Provider Interactions and Standardized Outcome Assessment in Patients With Pediatric Rheumatologic Conditions.

OBJECTIVE: For families navigating juvenile idiopathic arthritis (JIA) or systemic lupus erythematosus (SLE), effective communication with health care teams is vital. We examined how language preference other than English (LPOE) associates with patient-provider interactions and standardized outcome assessment. METHODS: In this retrospective cohort of patients with JIA or SLE at a tertiary care center (2016-2024), patients with LPOE were matched one-to-two to English language preference (ELP) counterparts by age, diagnosis, polyarticular course, and disease duration. We used multivariable nega

Digital health technologies to strengthen patient-centred outcome assessment in clinical trials in inflammatory arthritis.

Common to all inflammatory arthritides, namely rheumatoid arthritis, psoriatic arthritis, axial spondyloarthritis, and juvenile idiopathic arthritis, is a potential for reduced mobility that manifests through joint pain, swelling, stiffness, and ultimately joint damage. Across these conditions, consensus has been reached on the need to capture outcomes related to mobility, such as functional capacity and physical activity, as core domains in randomised controlled trials. Existing endpoints within these core domains rely wholly on self-reported questionnaires that capture patients' perceptions

Working towards patient oriented outcome assessment in normal pressure hydrocephalus, what is the most important?

OBJECTIVE: To date, there is no standard outcome assessment scale for shunt treatment in normal pressure hydrocephalus (NPH). In designing such scale, the relative weight of each of the common presentations of the condition from the patient's or his/her carer's point of view should be taken into consideration. METHODS: A questionnaire was sent to 24 patients treated for NPH and their family/carer, assessing the patient and carer categorization of the preoperative main complaint, the weight they give to each of the common presentations of NPH and their satisfaction with treatment. RESULTS: Tw

Gait assessment for neurologically impaired patients. Standards for outcome assessment.

This study compared the temporal-distance (TD) gait values of two groups of neurologically impaired subjects with published TD gait values of healthy subjects and analyzed the influence of nine clinical characteristics on TD values in the neurologically impaired subjects. Velocity, cadence, step length, stride length, and ratio of stride length to lower extremity length were recorded for 37 subjects with hemiparesis and 24 subjects with multiple sclerosis. Temporal-distance values were well below normal values, even in functionally independent subjects. Overall, the subjects with hemiparesis h

Validation of the Seizure-Related Impact Assessment Scale: A Novel Patient-Reported Outcome Measure for Epilepsy.

BACKGROUND AND OBJECTIVES: There is a clear need in epilepsy clinical trials and practice for a measure that captures the trade-off between seizure and treatment-related adverse effects, which is reliable over time and across different treatment regimens. We aimed to create and validate the Seizure-Related Impact Assessment Scale (SERIAS) to fill this need. METHODS: This was a prospective longitudinal study of adults with epilepsy recruited from an Australian comprehensive epilepsy center. Participants completed SERIAS at baseline and 3 and 6 months later. SERIAS has 6 self-report items. Five
