[Subcutaneous phaeohyphomycosis of the right thumb].

Black fungi are a group of fungi that are characterized by the development of a pale brown to black color in the cell walls of their vegetative cells, conidia, or both. A mycotic infection caused by a member of black fungi can be subdivided into three clinical entities: phaeohyphomycosis, chromoblastomycosis, and mycetoma. Phaeohyphomycosis is distinguished from mycetoma by the absence of grain (organized, interwoven mycelial aggregates) formation, and from chromoblastomycosis by the absence of sclerotic bodies (thick-walled muriform cells). Phaeohyphomycosis is a rare disease and has been spo

Phaeohyphomycosis of the Eyelid: A Case Report.

Phaeohyphomycosis is a fungal infection caused by dematiaceous fungi that presents as a superficial, cutaneous, subcutaneous, or systemic infection. Subcutaneous phaeohyphomycosis is the most common manifestation and presents as a subcutaneous nodule or cystic lesions and abscesses. It usually results from traumatic implantation of the saprophytic fungus from soil and vegetative matter; therefore, the commonest sites of infection are the extremities. Rarely, involvement of the eyelid is seen with extensive facial infection or as a part of multiple nodular lesions in the face, scalp, or other r

Ruxolitinib-Associated Phaeohyphomycosis: A Case Report.

We present the findings of a rare fungal infection phaeohyphomycosis caused by Pleurostoma richardsiae (P. richardsiae) in an immunocompromised 82-year-old male with myelodysplastic syndrome on treatment with the Janus kinase inhibitor ruxolitinib. Hematologists, infectious disease physicians, and surgeons should consider a fungal etiology in cases of cellulitis refractory to standard treatments in those patients with underlying immune dysfunction and/or in those receiving therapy with ruxolitinib or similar agents.

Three rare cases of cutaneous phaeohyphomycosis.

Phaeohyphomycosis is a chronic infectious condition caused by dematiaceous fungi which usually involve the skin and subcutaneous tissue. Subcutaneous phaeohyphomycosis is characterised by papulonodules, verrucous, hyperkeratotic or ulcerated plaques, cysts, abscesses, pyogranuloma, non-healing ulcers or sinuses. In India, commonly associated genera are Exophiala, Phialophora, Cladosporium, Curvularia, Fonsecaea and Alternaria. This condition involves the presence of brown-walled hyphal structures in the dermis and epidermis. Here, we are reporting a rare case series of three patients of phaeoh

[Subcutaneous phaeohyphomycosis caused by Phaeoacremonium parasiticum].

BACKGROUND: From the available literature, it is demonstrated that dematiaceous fungal infections mostly affect immunosuppressed patients. These infections can occur in different forms, from subcutaneous infection to disseminated forms that may compromise the life of the patient. In many cases the infection is related to the inoculation of the microorganism by diverse traumatic mechanisms, which determines the course of the infection to be slower in some cases. CASE REPORT: We describe two cases of phaeohyphomycosis caused by Phaeoacremonium parasiticum: A cancer patient with subcutaneous les

Case of phaeohyphomycosis producing sporotrichoid lesions.

A 90-year-old Japanese woman, taking prednisolone (5-10 mg/day) for polyarthritis, presented to our hospital with multiple subcutaneous lesions on her left arm in 2009. Her history included excision of a phaeomycotic cyst on the left middle finger in 2007. There were three subcutaneous nodules approximately 15 mm in diameter around her left wrist and a large soft cystic lesion measuring 80 mm × 60 mm on her left elbow. A granuloma with neutrophilic infiltration was detected in the deep dermis of a biopsy specimen. Chains composed of round brown cells and short pseudomycelia were found in the g

Subcutaneous phaeohyphomycosis caused by Cyphellophora pluriseptata.

We present the first case of subcutaneous phaeohyphomycosis caused by Cyphellophora pluriseptata, a fungus described in 1986 by DeVries. The patient was apparently healthy and presented an extensive infiltration of the left ear simulating borderline leprosy. Microscopically a heavy infiltration of lymphocytes admixed with multinucleated giant cells in the dermis was observed in the absence of abscesses. The histopathological diagnosis was achieved through the finding of dematiaceous hyphae. Based on a review of the specific literature the authors discuss the unusual aspects of this case and em

Sporotrichoid phaeohyphomycosis due to Alternaria infectoria.

We describe a cardiac transplant patient who had human cutaneous alternariosis with a sporotrichoid distribution of skin lesions. In this patient identification of the causative organism Alternaria infectoria was achieved by sequencing the rDNA internal transcribed spacer domain. Treatment with itraconazole led to clinical resolution within 4 months.

Cutaneous phaeohyphomycosis due to Alternaria tenuissima.

A 50-year-old gardener with a 3-year history of pemphigus vulgaris and steroid-induced diabetes complained of various papulonodular lesions on the left elbow. The lesions had appeared 11 months previously. Skin biopsy showed chronic granulomatous inflammation of the dermis which contained septate hyphae and large spores mainly free. Fragments of the biopsy specimen, cultured on Sabouraud glucose agar without cycloheximide, for 4 days, produced soft white colonies which later turned blackish brown, especially on the underside. On the basis of the microscopic characters of the conidiophores and

Systemic phaeohyphomycosis caused by Exophiala dermatitidis.

We report a case of systemic phaeohyphomycosis due to Exophiala dermatitidis in a 24-year-old man. At the age of 17, the patient had noticed cervical swellings. On palpation at his initial examination, more than a dozen firm lymph nodes between 1 and 5 cm in diameter were found on both sides of the neck and in both axillae. Examination of biopsy specimens of lymph nodes revealed pale brown hyphae in granulomatous lesions and Exophiala dermatitidis was isolated from mycological cultures of the local tissues. The clinical course was marked by an episode of jaundice six months later, and surgery

Cutaneous phaeohyphomycosis due to Cladosporium cladosporioides.

A 54-year-old man, affected by pemphigus vulgaris and severe steroid-induced diabetes, developed seven red-brown, firm, slightly raised 0.1-1 cm papular lesions on the anterior aspect of both knees and thighs. A cutaneous biopsy showed a granulomatous infiltrate with numerous fungal elements scattered in the dermis and also within giant cells. Cultures of cutaneous biopsy fragments on Sabouraud glucose agar in presence of chloramphenicol resulted in the growth of dark-green colonies at 25 degrees C. They were identified as typical Cladosporium cladosporioides. As far as we know, this species w

[Chromoblastomycosis and phaeohyphomycosis, overlooked fungal diseases].

Dark-pigmented microscopic fungi are worldwide-spread soil saprophytes often found on plant remnants. In chromoblastomycosis, infectious particles of these fungi enter the human body at the site of injury and may cause chronic infection, mainly in tropical and subtropical endemic areas. Chromoblastomycosis is almost exclusively diagnosed in patients with fully functioning immunity, with typically muriform cells present in infected tissue distinguishing this condition from phaeohyphomycosis. Phaeohyphomycosis, a less specific disease caused by dark-pigmented fungi, usually makes tissue necrotiz

Subcutaneous phaeohyphomycosis caused by Cladophialophora bantiana.

Primary subcutaneous phaeohyphomycosis can rarely be caused by Cladophialophora bantiana, and we present the histologic and culture findings of such a case. A 32-year-old African American woman with systemic lupus erythematosus presented with a 2-year history of multiple, recurrent, tender, and ulcerated skin nodules with purulent drainage on her upper back. Histologic sections of the excision demonstrated features of phaeohyphomycosis. Culture findings were characteristic of C bantiana. Of interest, at age 10 she had sustained traumatic implantation of wood splinters into this area during a t

Cerebral phaeohyphomycosis caused by Fonsecaea monophora.

We report a case of cerebral phaeohyphomycosis in a 53-year-old immunocompetent diabetic male, caused by Fonsecaea monophora. Computerized tomography of the brain revealed an abscess, which yielded F. monophora in pure culture. The patient's condition deteriorated on treatment with voriconazole and 5-fluorocytosine, and improved subsequently with high-dose itraconazole. The genus Fonsecaea has recently been revised and the new species F. monophora generated from molecular analysis of isolates, most of which were originally identified as Fonsecaea pedrosoi. This is the fourth case of cerebral i

We report a case of systemic phaeohyphomycosis in a 13-year old girl. The clinical presentation was with fever, weight loss, cholestatic jaundice with hepatosplenomegaly and generalized lymphadenopathy. The patient was diagnosed initially as tuberculous adenitis and was on antituberculous therapy, but with worsening of symptoms. Lymph node and liver biopsy showed granuloma with fungal hyphae suggestive of phaeohyphomycosis. The patient responded to antifungal therapy. Later she developed secondary biliary cirrhosis with cholangitis due to long stricture of the common bile duct and common hepat

Panniculitis is caused by a heterogeneous group of clinical entities that tend to manifest themselves as erythematous-violaceous subcutaneous nodules on the legs. In addition to associations with rheumatic diseases including vasculitis, its causes may include various infections. The microorganisms most commonly involved are bacteria and fungi. Among the latter, Candida Sp,, Nocardia Sp., Sporotrichosis, cryptococcosis, histoplasmosis, cromomycosis, and inter alia, have been described.
