Polycythemia Prevalence and Risk Factors in Pilots.

INTRODUCTION: Pilots are frequently exposed to thrombotic risk as a result of immobility from air travel. As hypoxemia is associated with secondary polycythemia, and polycythemia increases the risk of thrombosis, intermittent exposure to high-altitude hypoxic environments could escalate the risk of thrombosis in pilots. Our objectives were to find the prevalence of polycythemia in airplane pilots (primary outcome) and to assess associated risk factors of polycythemia (secondary outcome).METHODS: This study is a cross-sectional descriptive study. Data was collected from paper-based and computer

Chiari-I malformation in two fighter pilots.

This report describes two cases of Chiari Malformation Type I (Chiari-I) in fighter pilots of the Turkish Air Force. Chiari-I is a congenital malformation characterized by herniation of cerebellar tonsils through the foramen magnum. Patients have symptoms and signs related to dysfunction of the brainstem, spinal cord, and cerebellum. They generally are symptomatic in the earlier years of life. However, asymptomatic cases can eventually become symptomatic in later years. Symptoms can be provoked by increasing intracranial pressure (Valsalva or straining). We report on two pilots with Chiari-I m

Behavioral airsickness management program for student pilots.

The management of recurrent airsickness in student pilots has traditionally involved positive reinforcement of motivation and limited use of medication. The Behavioral Airsickness Management (BAM) program seeks to establish an effective behavioral and cognitive intervention format for managing airsickness symptoms. There were 37 student pilots experiencing recurrent airsickness who were exposed to an assessment and rehabilitation program designed to develop diaphragmatic breathing skills while rapidly reducing physiological tension via cue-evoked relaxation strategies. Cognitive modification t

Low Back Pain in Commercial Airline Pilots.

BACKGROUND: In their working life, airline pilots are exposed to particular risk factors that promote nonspecific low back pain (LBP). Because of the varying incidence internationally, we evaluated the point prevalences of acute, subacute, and chronic nonspecific LBP, as well as the current prevalences in German airline pilots. Furthermore, we compared the prevalence to the general German population and to European counterparts.METHODS: An anonymous online survey of 698 participating German airline pilots was evaluated. The impairment between groups was analyzed. Prevalences from our data were

Recurrent Middle Ear Barotrauma in Student Pilots.

INTRODUCTION: The present study evaluated the efficacy of the nine-step inflation-deflation tympanometric test for predicting recurrence of middle ear barotrauma.METHODS: Student pilots who were diagnosed with middle ear barotrauma from October 2010 to April 2011 were enrolled. The grade of barotrauma was assigned using Teed's classification. All subjects underwent tympanometry and nine-step inflation-deflation testing at first visit and after improvement. Recurrence was monitored for 2 wk after they resumed flight. The relationships among the recurrence of middle ear barotrauma, the grade and

Incidence of colorectal neoplasms among male pilots.

AIM: To assess the prevalence of colorectal neoplasms (adenomas, advanced adenomas and colorectal cancers) among Israeli military and commercial airline pilots. METHODS: Initial screening colonoscopy was performed on average-risk (no symptoms and no family history) airline pilots at the Integrated Cancer Prevention Center (ICPC) in the Tel-Aviv Medical Center. Visualized polyps were excised and sent for pathological examination. Advanced adenoma was defined as a lesion >10 mm in diameter, with high-grade dysplasia or villous histology. The results were compared with those of an age- and gende

Elevated bilirubin and visual perception in pilots.

BACKGROUND: A fatal aircraft accident of a Bo-105 helicopter on May 20, 1996 led to the suspicion that the pilot's raised serum bilirubin level could have contributed to the accident. It was suspected that an increase of bilirubin beyond 20.52 micromol x L(-1) could disturb visual perception. METHODS: In 1998 and 1999, 101 pilots with functional hyperbilirubinemia, diagnosed as Gilbert's syndrome, were subjected to perceptual-psychological testing (signal detection test) during their periodic medical examinations at the German Air Force Institute of Aviation Medicine. The results of the psych

Mortality among US commercial pilots and navigators.

The airline industry may be an occupational setting with specific health risks. Two environmental agents to which flight crews are known to be exposed are cosmic radiation and magnetic fields generated by the aircraft's electrical system. Other factors to be considered are circadian disruption and conditions specific to air travel, such as noise, vibration, mild hypoxia, reduced atmospheric pressure, low humidity, and air quality. This study investigated mortality among US commercial pilots and navigators, using proportional mortality ratios for cancer and noncancer end points. Proportional ca

Spondylolithesis in pilots: a follow-up study.

There were 21 pilots followed for 12-131 months in order to determine the natural history of spondylolithesis (SLL). Of these 21, 16 had follow-up X-ray examinations, and only 1 was found with significant progression of the posterior vertebral displacement. Of the 12 pilots with SLL and low back pain (LBP), 4 had recurrent single episodes of acute LBP, but all remained active and continued to fly over the follow-up period. None of the 9 pilots who had SLL discovered on routine X-ray examination developed LBP over the follow-up period. We conclude that pilots with SLL can continue to fly with m

Heat stress exposure of aerial spray pilots.

Heat stress (WBGT index) in the cockpits of agricultural spray pilots, and its physiological and subjective effects, were measured for 9 pilots flying in hot weather for a large Israeli aerial spray company. There was concern that heat exposure may be one of the factors involved in a recent increase in the fatal and non-fatal crash rate among Israeli spray pilots. WBGT index calculations were based on sequential cockpit wet bulb, dry bulb, and globe temperatures read when aircraft landed. The WBGT was always above 25 degrees C and exceeded 26.7 degrees C in 70% of observations. In pilots, a da

Impairment of flying efficiency in anancastic pilots.

Previous work on the effects of stress upon flying efficiency has shown two types of psychological stress to exist; these are usually referred to as cognitive and affective stress, respectively. A refinement in affective stress analysis has enabled two subtypes of stress to be demonstrated--one is characterized by activity of the sympathetic nervous system (SNS) and subjective feelings of rage, fear, anxiety, or frustration; the other by excessive rumination. These two subtypes are found in differing personality configurations. Ten pilots exhibiting the former type of stress, and 6 with the la

[Pilots with diabetes mellitus: cleared for take-off?].

In this issue, we outline the developments that have increasingly enabled people with insulin-dependent diabetes to professionally operate a vehicle. We focus on all professions in passenger transport in the Netherlands, with the pilot as a reference. A protocol has been developed in the UK to enable safe and responsible flying by selected pilots with type 1 diabetes mellitus and insulin dependent type 2 diabetes mellitus. It is used in several countries within and outside Europe, but not yet in the Netherlands. Modern diabetes care, innovative monitoring, good medical supervision and support

Recurrent Stroke Risk in Pilots with Atrial Fibrillation.

INTRODUCTION: Atrial fibrillation (AF) is one of the most common cardiac arrhythmias in the general population and is considered disqualifying aeromedically. This study is a unique examination of significant outcomes in aviators with previous history of both AF and stroke.METHODS: Pilots examined by the FAA between 2002 and 2012 who had had AF at some point during his or her medical history were reviewed, and those with an initial stroke or transient ischemic attack (TIA) during that time period were included in this study. All records were individually reviewed to determine stroke and AF hist

Melanoma incidence in Australian commercial pilots, 2011-2016.

OBJECTIVES: Occupational exposure to cosmic and ultraviolet radiation may increase airline pilots' risk of cutaneous melanoma. Meta-analyses of available data show a higher than average incidence of melanoma in airline pilots, but the most recent systematic review revealed that few contemporary data are available. Moreover, all relevant studies have been conducted in Northern Hemisphere populations. We therefore aimed to examine if Australian commercial pilots have a raised incidence of melanoma compared with the general population. METHODS: We examined all melanoma histologically diagnosed a

Medical certification of pilots with lower-extremity amputations.

More than 1.2 million Americans live with absence of a limb. Modern prosthetic limbs, talented prosthetists, and enthusiastic therapists enable many motivated individuals with absent limbs to pursue very active lifestyles. This article presents a case report of a first-class pilot who had experienced a traumatic above-knee amputation and includes a brief review of recent U.S. limb loss and amputation data, as well as aeromedical issues associated with amputation.

Apical hypertrophic cardiomyopathy and arrhythmia in military pilots.

Apical hypertrophic cardiomyopathy (ApHCM), a subtype of hypertrophic cardiomyopathy, may be found incidentally in healthy young adults. Arrhythmias are poor prognostic signs, and are the most frequent cause of sudden cardiac death. We present two cases of military aviators with ApHCM. One was a high-performance jet weapon system operator, who had asymptomatic non-sustained ventricular tachycardia (NSVT) and subsequently a symptomatic episode of paroxysmal atrial fibrillation. The second was a helicopter pilot, who had asymptomatic NSVT. Both aviators continue their aviation duties without exp

Only pilots get tired and other urban myths.

Beginning in 1985, the Federal Aviation Administration (FAA) came to the realization that regulating crew rest for pilots was an overdue and effective risk-management tool. Fatigue was being increasingly recognized as a contributing factor in many emergency medical services (EMS) accidents. Federal aviation regulations were changed to include pilot crew rest and flight hour and duty time limitations.

Adaptation of the horizontal vestibuloocular reflex in pilots.

OBJECTIVES/HYPOTHESIS: The vestibular function in pilots has been reported as being different to that in other, normal subjects. However, there is little evidence as to why and when this occurs, and its cause is still in doubt. The aim of this study was to evaluate changes in vestibular function in student pilots attending an elementary flight course, and in other active pilots, through the use of the slow harmonic acceleration (SHA) test and to verify the occurrence and timing of any vestibular function changes. STUDY DESIGN: Cross-sectional and prospective group design. METHODS: The six st

Human factors in aviation crashes involving older pilots.

BACKGROUND: Pilot errors are recognized as a contributing factor in as many as 80% of aviation crashes. Experimental studies using flight simulators indicate that due to decreased working memory capacity, older pilots are outperformed by their younger counterparts in communication tasks and flight summary scores. OBJECTIVE: This study examines age-related differences in crash circumstances and pilot errors in a sample of pilots who flew commuter aircraft or air taxis and who were involved in airplane or helicopter crashes. METHODS: A historical cohort of 3306 pilots who in 1987 flew commuter

General aviation crashes involving military personnel as pilots.

BACKGROUND: Injuries sustained in off-duty activities are a major cause of mortality and morbidity among military personnel. Reducing these off-duty fatalities is a continuing priority of the military. METHODS: General aviation crashes recorded by the National Transportation Safety Board between 1983 and 1998 were analyzed for military pilots (n = 205) and other military personnel (n = 185), and compared with all other general aviation crashes (n = 32,807) to identify differences in the crash circumstances and sustained injury severities. RESULTS: During the 16-yr study period, a total of 45
