Optimization of the Preanalytical Phase by Estimating Serum Indices Using an Automated Classifier.

BACKGROUND: Most laboratory errors occur in the preanalytical phase. Among the most common preanalytical errors are interferences due to hemolysis, lipemia, and icterus. Our objective was to evaluate a serum interference estimation methodology by the RSD classifier, to identify other biochemical parameters affected by preanalytical interferences, and to determine the economic impact of its implementation. METHODS: The serum indices of 65,529 requests measured by the RSD system and by the analytical determination on the Cobas 711 or Cobas 8000 platforms were collected. We proceeded to the sear

Quality Indicator-Driven Six Sigma Scale Assessment of Extra-Analytical Phases in Clinical Chemistry Laboratory-Insights from a Tertiary Care Hospital.

BACKGROUND: In recent years, the error rate in the analytical stage of laboratory testing has been drastically reduced due to automation, good-quality reagents, standardized assays, and increased participation of laboratories in proficiency testing programs. Nevertheless, high error rates in extra-analytical phase remains a critical vulnerability impacting diagnostic performance and patient safety. OBJECTIVE: The present observational study was designed to quantify error rates and evaluate efficiency of extra-analytical processes, using quality indicators on the Sigma metrics. METHODS: We ex

Are pre-analytical factors fully considered in forensic FFPE molecular analyses? A systematic review reveals the need for standardised procedures.

The need for molecular analyses has become increasingly common in the forensic sciences, particularly in forensic pathology, to better shape the causes of death. This approach is called the "molecular autopsy," where conventional medico-legal findings are often enhanced with specific molecular tests to provide reliable clinical and forensic diagnoses. In this context, FFPE (Formalin-Fixed Paraffin-Embedded) tissue samples collected during forensic autopsies are the only available specimens in retrospective studies for molecular DNA and/or RNA analyses. It is well known that pre-analytical para

High anion gap metabolic acidosis caused by D-lactate: mind the time of blood collection.

INTRODUCTION: D-lactic acidosis is an uncommon cause of high anion gap acidosis. MATERIALS AND METHODS: A 35-year old woman was admitted to the emergency room with somnolence, drowsiness, dizziness, incoherent speech and drunk appearance. Her past medical history included a Roux-en-Y bypass. Point-of-care venous blood analysis revealed a high anion gap acidosis. Based on the clinical presentation, routine laboratory results and negative toxicology screening, D-lactate and 5-oxoprolinuria were considered as the most likely causes of the high anion gap acidosis. Urine organic acid analysis reve

[Blood cultures in clinical practice: Principles, procedures, and quality management].

Blood cultures remain the cornerstone of diagnosing bloodstream infections, and their appropriate use directly impacts timely diagnosis, targeted antimicrobial therapy, and patient safety. However, their diagnostic yield and clinical interpretability are determined not only by laboratory processing itself, but also by the entire sequence of interconnected pre-analytical, analytical, and post-analytical steps. This review article summarizes the principles of blood culture testing, focusing on appropriate indications and timing of blood collection, recommended number of culture sets and blood vo

Making a science out of preanalytics: An analytical method to determine optimal tissue fixation in real-time.

Modern histopathology is built on the cornerstone principle of tissue fixation, however there are currently no analytical methods of detecting fixation and as a result, in clinical practice fixation is highly variable and a persistent source of error. We have previously shown that immersion in cold formalin followed by heated formalin is beneficial for preservation of histomorphology and have combined two-temperature fixation with ultra-sensitive acoustic monitoring technology that can actively detect formalin diffusing into a tissue. Here we expand on our previous work by developing a predict

[Metals: common clinical applications in inductively coupled plasma mass spectrometry].

Inductively coupled plasma mass spectrometry (ICP-MS) is an analytical technique for inorganic, quantitative, and multi-elemental compounds commonly used in clinical biology. In recent years, it has gradually replaced atomic absorption techniques. It is based on the combination of an ion source formed of an inductively coupled plasma torch by a high-frequency electromagnetic generator, and a mass spectrometer, which allows the separation of ions generated according to their mass and energy. It allows the simultaneous detection of most metal and metalloid elements (about 80, 40 generally). It i

Preanalytical step of urinary protein measurement: from urine sampling to preparation for analysis of the specimen.

The preanalytical phase is a key step in urinary protein measurement. It is a complex step, which includes urine sampling, storage and transport to the laboratory and preparation for analysis of the specimen. It can lead to numerous errors, since urine sampling is made by the patient himself. According to different cases, the procedures for urine sampling are presented in appendix 1 to 4. The aim of the presented guidelines is to improve the preanalytical step. In addition to the sampling made by a well-informed patient, the laboratory has to optimize transport and sample preservation, accordi

Sample preparation strategies for the determination of psychoactive substances in biological fluids.

This review focuses on the existing analytical procedures for the determination of new psychoactive substances (NPS) in biological fluids by chromatographic methods. Direct analysis of samples is scarcely employed and most proposed methodologies include a sample pre-treatment in order to remove matrix interferents and, in some cases, pre-concentrate extracts. Current extraction methods for NPS determination in plasma/serum, urine, and oral fluids have been widely discussed, such as liquid-liquid, solid-phase, and micro extraction approaches, highlighting the advantages and drawbacks of the pro
