Boundary work in marketized parent-school relations: an insider autoethnography of teacher emotional labor in a Chinese international school.

BACKGROUND: The marketization of international schools embeds teachers' work within family choice, service commitments, expectations for educational advancement, and satisfaction-based evaluation. OBJECTIVE: This study examines how parent-as-client logic enters teachers' everyday parent-school communication in a Chinese international school, how it shapes teachers' emotional labor and perceived professional autonomy, and how teachers engage in boundary work to negotiate professional identity and occupational wellbeing. METHODS: The study adopts an analytic insider/practitioner autoethnograph

When using entrustable professional activities thwarts basic psychological needs: A case study of residents' experience.

BACKGROUND: Entrustable professional activities (EPAs) were introduced to link competencies with clinical work and support residents' professional and competence development. Yet their implementation has raised concerns about burdensome documentation and limited educational value. Drawing on Self-Determination Theory (SDT), we examined how residents experienced EPAs in relation to their needs for autonomy, competence, and relatedness-needs essential for optimal motivation and professional development. METHODS: We conducted an instrumental case study in a Canadian internal medicine residency p

Safety steps: A framework for promoting safe trainee autonomy in clinical emergencies.

Balancing trainee autonomy with patient safety during clinical emergencies is an enduring challenge faced by hospitalists. In this article, we propose a framework to be used in urgent medical situations, such as hypotension or acute hypoxic respiratory failure. These strategies can be used to enhance early career trainee learning, confidence, and independence in such challenging situations by fostering a supportive learning environment while emphasizing both patient safety and trainee growth.

Moral principles and medical practice: the role of patient autonomy in the extensive use of radiological services.

There has been a significant increase in the use of radiological services in the past 30 years. There are many reasons for this, but one has received little attention: the increased role of patient autonomy in healthcare. Patients demand x rays, CT scans, MRI, and positron emission tomography scans. The key question in this article is how a moral principle, such as respect for patient autonomy, can influence the extension of radiological services. A literature review reveals how patient autonomy is acknowledged in radiology, and how it is used both to explain and to justify the increase in rad

At the intersection of care and command: A thematic analysis of military nurses' experiences in a compact national defense structure.

BACKGROUND: Military nursing operates at the intersection of healthcare and defense, requiring nurses to balance professional ethics with military discipline, operational readiness, and organizational hierarchy. PURPOSE: To explore how military nurses in a small defense system construct and sustain professional identities and how organizational structures and military culture shape their autonomy, competence, well-being, and family life. METHODS: A qualitative design using reflexive thematic analysis was employed. Nineteen individual semistructured interviews were conducted with military nur

Direct family participation in care in adult intensive care: A Walker and Avant concept analysis.

BACKGROUND: Family participation is increasingly recognised in adult intensive care units (ICUs). However, family participation, involvement and engagement are often used as umbrella terms encompassing presence, emotional support, communication, shared decision-making and direct care. Consequently, the meaning and professional boundaries of direct family participation in care (DFPC) remain unclear. AIM: To clarify the defining attributes, antecedents, consequences, empirical referents and operational definition of DFPC in adult intensive care. METHODS: Walker and Avant's method guided the an

A scoping review of evidence of community pharmacist independent prescribing for common clinical conditions: beyond protocol prescribing.

OBJECTIVES: Pharmacist independent prescribers (PIPs) enhance patient care but there is variability in their integration in community pharmacy (CP). The objective was to collate and characterize literature on the integration of 'standard of care' model PIPs in CP for acute common clinical conditions (CCCs). METHODS: This review followed the Arksey and O'Malley framework. Eligibility criteria, search databases, and terms were defined. Information sources were searched from January 2006 to October 2023. Following screening, full text review, and data extraction a narrative synthesis approach wa

[Overhauling nursing education: clinical autonomy, leadership, and educational transformation].

The reform of nursing education marks a turning point for the profession by defining nursing practice around five areas of competence centered on clinical autonomy, reasoning, and nursing consultation. It establishes a curriculum based on real-world scenarios, integrating scientific knowledge, reflective practice, and evidence-based nursing to inform decision-making at the patient's bedside. At the heart of the new framework, nurses become clinical decision-makers and care pathway leaders, working in interprofessional collaboration, within the context of the healthcare system's strategic trans

Measurement of trainee autonomy in robotic-assisted lung resections using objective performance indicators.

Trainee autonomy in robotic-assisted thoracic surgery is critical for growth but difficult to quantify. Robotic platforms can log intraoperative kinematic and event data that can be transformed into metrics known as objective performance indicators (OPIs). This study sought to use OPIs obtained during robotic-assisted anatomic lung resections to assess trainee console autonomy at a high-volume robotic center. OPI data were obtained from 59 lobectomies and segmentectomies performed on a multi-port robotic platform between March 2022 and May 2023. Cases were divided into anatomic steps and annot

IRB reformation: is unfettered access the answer?

The purpose of this column is to consider if research using only an interview should be evaluated by an institutional review board (IRB) or if it may be considered exempt. A review of atrocities committed by researchers both in the biomedical and psychosocial domains will be considered as a backdrop to the discussion. Issues that plague IRBs will be examined including lack of transparency, overreach of the process, and stipulations that change the nature of research. Finally, recommendations for reform will be entertained.

[Genetic counselling: theoretical aspects and prenatal practice].

Genetic counselling provides families with accurate medical information and psychological support. Respect and concern for the emotional well-being should be taken into account while discussing genetics aspects and recurrence risks. The importance of autonomy and confidentiality, central to genetic counselling, is reinforced by the new Swiss law (LAGH). In many countries, most of the genetic counselling is provided by genetic counsellors who have a specialised post-graduate training. Genetic counselling plays an increasing role in different medical specialities. In particular, it is essential

'Doctor, what would you do in my position?' Health professionals and the decision-making process in pregnancy monitoring.

OBJECTIVE: Routine prenatal screening for Down syndrome challenges professional non-directiveness and patient autonomy in daily clinical practices. This paper aims to describe how professionals negotiate their role when a pregnant woman asks them to become involved in the decision-making process implied by screening. METHODS: Forty-one semi-structured interviews were conducted with gynaecologists-obstetricians (n=26) and midwives (n=15) in a large Swiss city. RESULTS: Three professional profiles were constructed along a continuum that defines the relative distance or proximity towards patien

Autonomy support in an exercise intervention with colorectal cancer patients during chemotherapy: a qualitative perspective.

OBJECTIVE: Physical activity (PA) has emerged as an important element of supportive care for cancer patients, but few patients engage with exercise. Considering that autonomy support is associated with healthy lifestyles, it would be useful to know the specific autonomy-supportive techniques that can help to encourage PA in colorectal cancer (CRC) patients. This study aims to qualitatively explore autonomy support perceptions through a self-determination-theory-based exercise program (FIT-CANCER) with CRC patients during chemotherapy treatment. METHODS AND MEASURES: A total of 27 participants

Toward Autonomous Clinics: Human-Robot Collaboration in Clinical Care.

Robotic systems are entering clinical care, but full autonomy remains constrained by patient variability, safety requirements, dynamic clinical environments, and the need for professional oversight. Human-robot collaboration (HRC) offers a more realistic path: clinicians retain judgment and responsibility, while robots and AI support sensing, planning, action, and decision-making within defined task boundaries. In this narrative review, we examine recent advances in clinical HRC from a systems perspective, covering system architecture, learning and control, safety mechanisms, autonomy assessme

Supporting Autonomy in ADL Care. How Nursing Students Learn in Theory Classes: A Qualitative Multi-Perspective Study.

AIM: This qualitative study examined how autonomy and autonomy support are addressed in the theoretical part of Dutch nursing education. BACKGROUND: Supporting clients' autonomy in activities of daily living is essential for dignity and quality of life, but remains complex and often suboptimal due to contextual factors such as care settings, nurse-client relationships, and systemic pressures. Nurses must balance professional skills with sensitivity to clients' values. Little is known about how nursing students are prepared to support autonomy. METHOD: A document analysis of curricula and tea
