Interventional radiology in bone metastases.

Interventional radiology plays a significant role in the treatment of bone metastases by various techniques, percutaneous or endovascular. Vertebroplasty is the most well-studied technique for stabilisation of spine metastases as it induces satisfactory stabilisation of the vertebra and offers clear improvement of the quality of life. Due to the success of this technique cementoplasty of other bones, mainly pelvic girdle, has been largely developed. The development of reinforced cementoplasty allows treatment of pre-fractural osteolysis of some long bones. The heat due to the polymerisation of

Interventional radiology in the elderly.

Interventional radiological percutaneous procedures are becoming all the more important in the curative or palliative management of elderly frail patients with multiple underlying comorbidities. They may serve either as alternative primary minimally invasive therapies or adjuncts to traditional surgical treatments. The present report provides a concise review of the most important interventional radiological procedures with a special focus on the treatment of the primary debilitating pathologies of the elderly population. The authors elaborate on the scientific evidence and latest developments

[Combined therapies including interventional radiology].

Interventional tumor therapies are a therapeutic option that is increasingly applied in the treatment of malignant diseases. Thermoablation of primary and secondary liver tumors is the most widely used of them, and owing to the promising results it has given, it is becoming an increasingly well-accepted treatment procedure. These interventional radiological tumor therapies must be evaluated in comparison with standard therapies, and particularly in comparison with surgical resection, radiotherapy, and systemic chemotherapy. This comparative approach to the evaluation of different therapies is

Deterministic effects in interventional radiology.

Since the development of interventional radiology, the number and complexity of procedures has increased significantly and continues to grow. Interventional radiology procedures offer substantial health care benefits. However, associated with the increasing complexity as well as a lack of quality control programmes and specific training in radiation protection, there is an increase in the occurrence of deterministic effects in both patients and staff. There is a growing literature of case reports describing deterministic effects. A review of some case reports and the response of various intern

[Interventional radiology in thoracic emergencies].

There are numerous clinical situations in which interventional angiography fully reveals its two-fold diagnostic and therapeutic value. The present review focuses attention on the use of such procedures in certain thoracic emergencies. Indications, diagnostic results and therapeutic advantages are examined together with possible complications. Pulmonary embolism is a serious circulatory condition that is often difficult to diagnose because of the lack of specificity of its accompanying symptoms. In these cases the role of the angiographic radiologist is often three-fold: diagnosis, therapy (po

[Interventional radiology and abdominal emergencies].

Interventional radiology used in acute abdominal diseases has demonstrated its efficacy in most emergency cases especially in patients at high risk for surgery. Percutaneous drainage is a reference standard for abscesses located within the abdomen and pelvis with high success rates of up to 80%. The diagnosis and treatment of gastrointestinal hemorrhage have changed over the past two decades, with decreased diagnostic examinations and advances in pharmacologic therapy and improved endoscopic techniques. On the other hand, embolization techniques and new procedures such as trans jugular intra h

Interventional Radiology Management of Vascular Anomalies.

Vascular anomalies span a wide biological spectrum, from self-involuting infantile hemangiomas to life-threatening arteriovenous malformations. This review focuses on the conditions most commonly managed by interventional radiology, organized within the 2025 International Society for the Study of Vascular Anomalies classification framework. It covers the principles and agents of sclerotherapy for venous and lymphatic malformations, including comparative evidence for sclerosing agents, and embolization strategies for arteriovenous malformations and hereditary hemorrhagic telangiectasia -related

Visceral nerve interventions in interventional radiology.

Chronic abdominal visceral pain management often requires multidisciplinary collaboration. Image-guided visceral nerve interventions may be critical in the management of visceral pain refractory to medical treatments. Abdominal and pelvic pain is mediated by specific nerves involving specific ganglia. The well-defined location of these ganglia provides important targets for percutaneous image-guided interventions to alleviate chronic abdominal or pelvic pain. In this review, we provide an in-depth discussion of the anatomy, indications, evidence, and technical and clinical considerations and c

Interventional Radiology: Indications and Best Practices.

Interventional radiology employs image-guided techniques to perform minimally invasive procedures for diagnosis and treatment. Interventional radiology is often used to place central venous catheters and subcutaneous ports, with some evidence of benefit over surgical placement. Arterial embolization procedures are used to manage many types of hemorrhage and are highly effective for severe postpartum hemorrhage. Vascular interventions, such as endovascular treatment of varicosities, acute limb ischemia, and pulmonary embolism, are superior to surgical interventions. For chronic limb ischemia an

[Interventional radiology to treat vertebral diseases].

The vertebroplasty is based on intracorporeal acrylic resin injection. Its 2 main objectives are analgesia and bone consolidation. Its main indications are painful and inflammatory fractures on a site of osteopathic fragility and bone metastases. Its clinical efficacy is strongly correlated with a treatment performed within 3 months post fracture. Transpedicular acrylic resin injection is performed under scopic control in order to continuously follow the intra-corporeal filling of the cement and thus limit the risk of complications. Main risks of vertebroplasty are rare and represented by the

[Interventional radiology: transoral approach to C2].

PURPOSE: To assess the value of a direct transoral approach to C2 for interventional procedures. MATERIALS AND METHODS: A transoral approach to C2 was used in 4 patients (9-64 year old) with lytic lesion of the C2 vertebra and/or base of the odontoid over a 10 year period. Three patients underwent biopsy and 1 patient underwent vertebroplasty using biplane fluoroscopy. All procedures were performed under general anesthesia using a 13G needle directly introduced in the C2 vertebra through the posterior pharyngeal wall. RESULTS: None of the patients reported ill-effects from the procedure. Nee

Interventional radiology of the adrenal glands.

Interventional radiology of the adrenal glands comprises angiographic and percutaneous procedures. Vascular maneuvers have never received widespread application. Venous retrograde adrenal gland ablation fell into disuse because it induced only temporary reduction of adrenal function, caused intense long lasting pain, and was considered unsafe. Chemoembolization of cortico-adrenal carcinoma has been shown to slow tumor growth and may be employed if the patient is inoperable. Percutaneous FNB, routinely performed in large hospital centers, has a high diagnostic yield and a low complication rate.

[Interventional radiology of the digestive system].

The authors report on the use of interventional radiology modalities in the treatment of some gastrointestinal diseases (intussusception, stenosis, hemorrhage, etc). The results are compared with literature data relative to both interventional radiology and conventional therapeutic procedures. In our series, hydrostatic reduction of ileocolic intussusception was successful in 60% of cases. Transluminal dilatation with a balloon catheter was attempted in 108 patients with alimentary tract stenosis and was successful in 93. Hemorrhage and ischemia can be treated by means of transcatheter therapy

Intravascular US: Applications in Interventional Radiology.

Interventional radiology applications of intravascular US (IVUS) continue to expand, complementing intraprocedural angiography and providing a unique vantage from which to guide endovascular interventions. Vascular pathologic conditions become sonographically visualized rather than inferred from the planar appearance of the opacified vascular lumen. Perivascular targets become sonographically visualized rather than approximated on the basis of fluoroscopic landmarks. The authors introduce broad categories of IVUS catheters, namely radial and side-firing varieties, as well as prevailing options

Interventional Radiology Options after Visceral Surgery.

BACKGROUND: Postoperative management of patients undergoing visceral surgery can present challenging clinical situations with significant morbidity and mortality. Interventional radiological techniques offer quick, safe, and effective minimally invasive treatment options in the postoperative management of visceral surgery. SUMMARY: Most commonly done procedures include - but are not limited to - fluid or abscess drainage, biliary diversion, bleeding embolization, and re-canalization of a thrombosed vessel. While bleeding from side branches after hepatobiliary and pancreatic surgeries can be m

Neurological pain relief interventional radiology procedures.

Neurological interventions have taken on a significant role in interventional radiology (IR) practice. Indications fall under three main categories: (1) intraprocedural pain management, (2) cancer pain palliation, and (3) chronic non-cancer pain control. Short-term regional anaesthesia can be achieved with local anaesthetics, while longer-term pain control can be attained with radiofrequency neuromodulation (pulsed or otherwise) or thermal/chemical neurolysis. This review article summarises the therapeutic options, applications, and techniques of commonly used peripheral nerve and plexus inter

Urinary Drainage Procedures in Interventional Radiology.

Urinary drainage procedures are used to treat a wide range of clinical situations including pyonephrosis, preservation of renal function in patients with ureteral obstruction, as a means to access the collecting system for stone retrieval or lithotripsy and to divert urine from a distal leak or fistula. Several different drainage devices are available and include those that provide obligatory external drainage (nephrostomy), both internal and external drainage (nephroureteral stent) and internal drainage (double-J stent). Each device requires some maintenance and effort on the patient's part-f

Musculoskeletal interventional radiology: ultrasound and CT.

We aim to describe imaging-guided (ultrasound and CT) interventional techniques in the musculoskeletal system that can be performed by general radiologists, whether in hospitals, primary care clinics, private offices, or other settings. The first requirement for doing these procedures is adequate knowledge of the anatomy of the musculoskeletal system. The second requirement is to inform the patient thoroughly about the technique, the risks involved, and the alternatives available in order to obtain written informed consent. The third requirement is to ensure that the procedure is performed in

Neuroendovascular therapies in pediatric interventional radiology.

Transarterial embolization of head and neck tumors can be done for a variety of indications, including tumoral epistaxis and high-output cardiac failure, but is mainly done before surgical resection. This technique offers higher surgical cure rates and faster recovery, while decreasing surgical morbidity by curtailing blood loss and reducing the duration of surgery. Patients with epistaxis are considered for endovascular management when they have failed conservative management, including topical vasoconstrictors, cautery, and nasal packing (anterior and posterior). Sudden arterial bleeding in
