This research aimed to examine the diagnostic accuracy and clinical significance of endoscopic ultrasonography (EUS) in the context of small rectal neuroendocrine neoplasms (NENs). A total of 108 patients with rectal subepithelial lesions (SELs) with a diameter of < 20 mm were included in the analysis. The diagnosis and depth assessment of EUS was compared to the histology findings. The prevalence of NENs in rectal SELs was 78.7% (85/108). The sensitivity of EUS in detecting rectal NENs was 98.9% (84/85), while the specificity was 52.2% (12/23). Overall, the diagnostic accuracy of EUS in ident

A retrospective analysis was made of 372 consecutive patients who underwent colonoscopy because of rectal bleeding. The three distinct patterns of bleeding studied were chronic (224 cases), recent major (93 cases), and acute bleeders (55 cases). In 50% of the cases, colonoscopy detected a lesion other than diverticula. These lesions consisted of several forms of colitis, arteriovenous malformations (AVMs), ulcers, and most importantly, neoplasms in 34% of the cases. In 13% of the cases, an invasive neoplasm was detected and 76% of them were early stage (Dukes A or B). A third of the neoplasms

Mucoceles, commonly observed in the appendix, are mucin-filled, dilated structures arising from a range of etiologies. Cases associated with dysplastic or neoplastic epithelium can rupture and disseminate within the abdominopelvic cavity. Similar lesions in other parts of the colon are exceedingly rare, with only 16 colonic mucoceles having been reported. The first case of a colonic mucinous neoplasm with dysplasia resembling a low-grade appendiceal mucinous neoplasm involving rectal stump was described in 2016. Here, we present the second such case arising in the rectal stump, identified in a

The aim of this study is to develop a prognostic model for evaluating radiotherapy response in patients diagnosed with rectal neoplasms by integrating genomic and clinical data. Publicly accessible datasets from The Cancer Genome Atlas and Gene Expression Omnibus were analyzed to identify differentially expressed genes associated with drug resistance and mitophagy. Functional enrichment analyses were conducted to investigate relevant biological pathways. A prognostic risk model was constructed using least absolute shrinkage and selection operator regression and validated via receiver operating

The approach to local excision of benign and early-stage, well-selected neoplasms of the rectum continues to evolve. We demonstrate here that local excision of a rectal neoplasm using the da Vinci Robotic Surgical System can be successfully performed. To our knowledge, this is the first time robotic transanal surgery (RTS) has been used in this manner.

BACKGROUND: Esophageal cancer, specifically esophageal squamous cell carcinoma (ESCC), poses a significant health challenge characterized by a poor prognosis and high mortality rates. Surgical interventions, such as minimally invasive esophagectomy, are the primary treatment approach; however, the occurrence of postoperative complications, particularly pneumonia, presents substantial risks. Small airway disease (SAD) affecting the bronchioles' small airways is a frequently underestimated respiratory condition with potential implications for the postoperative outcomes of ESCC patients.

PURPOSE: After a full-thickness total wall excision of a rectal tumor, suturing the defect is generally recommended. Recently, due to various contradictory studies, there is a trend to leave the defects open. Therefore, this study aimed to determine whether leaving the defect open is an adequate management strategy compared with suturing it closed based on postoperative outcomes and recurrences.
METHODS: A retrospective review of our prospectively maintained database was conducted. Adult patients who underwent transanal surgery for rectal neoplasm in our institution from 1997 to 2019 were ana

This systematic review synthesizes current evidence on robotic transanal minimally invasive surgery (TAMIS) for rectal neoplasms, analyzing data from 26 clinical studies (2011-2024) involving 1,284 patients. The findings demonstrate robust short-term oncologic outcomes, including an 89.1% R0 resection rate for malignant lesions and an 8.3% local recurrence rate at a median follow-up of 28 months. Technical advancements, particularly the development of single-port robotic systems, have yielded significant improvements in procedural efficiency, reducing operative time by 22% while enhancing surg

BACKGROUND: Local excisions are important in a tailored approach to treatment of rectal neoplasms. In cases of low risk T1 local excision facilitates rectal-preserving treatment. Transanal minimally invasive surgery (TAMIS) is the most recent alternative developed for local excision. In this study we evaluate the results after implementing TAMIS as the routine procedure for local excision of rectal neoplasms.
METHODS: All patients who underwent TAMIS from January 2016 to January 2020 at St. Olav's University Hospital were included, and clinical, pathological and oncological data were prospect

Multiple primary malignancies in an individual patient are thought to be a common feature of hereditary disease. Asymptomatic renal neoplasms have been described in up to 4% of rectal cancer patients. We have assessed whether microsatellite instability is present in the rectal and renal tumors of patients presenting at our institution with primary renal and rectal cancers. Of the seven patients presenting with both diagnoses, paraffin blocks were available from five cases of colorectal cancer and six renal neoplasms. Five of six cases of renal tumors were informative and all were microsatellit

From November 1989 to April 1990, 16 patients with rectal cancer were examined preoperatively by means of double contrast barium enema and defecography. Double contrast barium enema was used to identify the cancer: based on the distance of cancer from anal rima, the patients were divided into 3 groups: 1) less than 6 cm; 2) 6-11 cm; 3) more than 11 cm. In all patients defecography was performed at the end of barium enema to evaluate rectal wall mobility. Thus, the morphological information yielded by barium enema could be integrated with the dynamic data from defecography. The evaluation was t

BACKGROUND: Diminutive rectal neuroendocrine neoplasms (NENs) are often treated by local excision, but the presence of regional nodes or distant metastasis is reported in an increasing number of reports.
METHODS: We retrospectively recruited 44 patients with rectal NENs below 2 cm. These patients were not suitable for local excision due to invasion of the muscularis propria, involvement of regional lymph nodes or distant metastasis. A multivariate logistic regression model was conducted to identify the risk factors for distant metastasis. Survival analysis was performed to evaluate their prog

BackgroundThis study aimed to evaluate contemporary national trends in the presentation, management, and outcomes of early-onset rectal neuroendocrine neoplasms (EORNETs).MethodsA retrospective cohort analysis of patients <50 years who underwent surgery for rectal neuroendocrine neoplasms was performed using the National Cancer Database (NCDB) (2004-2023). Patients undergoing surgery were categorized as local tumor excision or radical resection. Five-year overall survival (OS) was estimated using Kaplan-Meier analysis, and multivariable Cox proportional hazards analysis was used to identify fa

BACKGROUND: Recently, several endoscopic techniques have been used to improve the R0 resection rate of rectal neuroendocrine neoplasms (R-NENs). However, none of these methods can achieve 100% complete resection (CR), particularly in the vertical direction. Endoscopic full-thickness resection (EFTR) has proven to be an effective method for the treatment of submucosal tumors but is seldom utilized in the eradication of R-NENs.
AIM: To review cases of R-NENs removed using EFTR and to evaluate the safety and efficacy of this technique.
METHODS: This retrospective cohort study enrolled 160 patie

BACKGROUND: Rectal neuroendocrine neoplasms are relatively rare. Patients with rectal neuroendocrine neoplasms undergoing radical surgery have a higher rate of lymph node metastases. Robust evidence on the status of lateral pelvic lymph node metastases and the role of lateral pelvic lymph node dissection in those patients is lacking. This case-series study aimed to explore and address these issues.
METHODS: This single-center, prospective case series consecutively enrolled patients with biopsy-proven rectal neuroendocrine neoplasms in a tertiary referral hospital between June 2022 and January

BACKGROUND: Rectal neoplasm is one of the most common malignancies worldwide. Screening programs for rectal neoplasm result in early diagnosis and a decrease in disease-related mortality and morbidity. In selected patients, early rectal cancer may be treated with local excision. Owing to poor exposure during conventional transanal excision, transanal minimally invasive surgery (TAMIS) was developed, and TAMIS is feasible for the local excision of selected rectal neoplasms. However, the limited range of motion is a major disadvantage of this operation. Therefore, robotic TAMIS was developed to
