Immunologic Contributions Following Rotator Cuff Injury and Development of Cuff Tear Arthropathy.

»: Rotator cuff tear arthropathy (RCTA) describes a pattern of glenohumeral degenerative changes following chronic rotator cuff tears that is characterized by superior humeral head migration, erosion of the greater tuberosity of the humeral head, contouring of the coracoacromial arch to create a socket for the humeral head, and eventual glenohumeral arthritis. »: Acute and chronic inflammatory changes following rotator cuff tears are thought to contribute to cartilage damage, muscle fibrosis, and fatty infiltration in the glenohumeral joint. »: In vitro animal studies targeting various infla

Is there a relation between rotator cuff injury and core stability?

BACKGROUND: Strong core stabilization not only minimizes the load on the vertebral column, but also improves strength and endurance of peripheral joints, and enables the energy transfer to distal segments. Despite the current interest surrounding core stability, none of the studies investigated the effect of core stability on the formation of rotator cuff tear or healing after repair. OBJECTIVE: To determine the relationship between core stability and upper extremity functional performance in patients who underwent rotator cuff repair surgery and to compare those with healthy subjects of simi

Partial rotator cuff injury in athletes: bursal or articular?

UNLABELLED: A painful shoulder is a very common complaint among athletes, especially in the case of those in sports involving throwing. Partial lesions of the rotator cuff may be very painful and cause significant functional limitation to athletes' sports practice. The incidence of partial lesions of the cuff is variable (13-37%). It is difficult to make the clinical and radiological diagnosis, and this condition should be borne in mind in the cases of all athletes who present symptoms of rotator cuff syndrome, including in patients who are diagnosed only with tendinopathy. OBJECTIVE: To eval

MRI findings of rotator cuff myotendinous junction injury.

OBJECTIVE: The purpose of this article is to describe the MRI features of rotator cuff myotendinous junction injuries. MATERIALS AND METHODS: We retrospectively identified MRI cases with myotendinous junction injury of the rotator cuff muscles and reviewed clinical, imaging, and surgical records. MR images were reviewed independently by two musculoskeletal radiologists to grade myotendinous junction injuries (strain, partial tear, or complete tear) and to assess for concurrent tendon tears (partial or full thickness) and bone changes (fracture or contusion). RESULTS: The final study group co

In vitro kinematics of the shoulder following rotator cuff injury.

BACKGROUND: Tears in the rotator cuff may result in altered mechanics of the glenohumeral joint. It is not unusual for some patients with cuff tears to exhibit near normal motion with the injured shoulder, while other patients with the same magnitude of injury are unable to achieve full elevation on the injured side. METHODS: The effect of simulated tears of the rotator cuff on active glenohumeral joint kinematics was investigated by testing eight cadaveric specimens using an in vitro shoulder simulator. Active abduction of the humerus was produced by applying forces to simulate loading of th

Chronic rotator cuff injury and repair model in sheep.

BACKGROUND: Most rotator cuff surgery is performed on chronic tears. As there is no animal model in which to examine the physiology of muscle and tendon injury and repair in this setting, we developed a chronic rotator cuff injury model in sheep. METHODS: The infraspinatus tendon was released in thirty-six female sheep. Biopsy specimens were obtained from the muscle and were analyzed for fat content. The force generated by the muscle with supramaximal stimulation was recorded intraoperatively. A control group (twelve sheep) underwent an immediate tendon repair. In the remaining twenty-four sh

MRI Study of Trauma-Induced Rotator Cuff Injuries and Their Correlation to Mechanism of Injury.

INTRODUCTION: Traumatic rotator cuff injury is a frequent cause of acute shoulder pain, weakness, and functional limitation. Unlike degenerative tears, trauma-related tears usually follow a definite injury event and may show specific imaging patterns depending on the mechanism of trauma. MRI allows detailed assessment of tendon integrity, tear morphology, retraction, muscle status, bone marrow edema, joint effusion, bursitis, and associated shoulder abnormalities. This study aimed to evaluate MRI findings in trauma-induced rotator cuff injuries and correlate them with the mechanism of injury.

More than pain and physical limitation: the declined cognitive performance associated with rotator cuff injuries.

BACKGROUND: Rotator cuff (RC) injuries often lead to shoulder pain, physical limitations, sleep disturbances, and emotional distress. However, the relationship between these symptoms and cognitive decline remains unclear. METHODS: We recruited 150 patients with RC injury, who completed the Visual Analog Scale (VAS) for pain, the American Shoulder and Elbow Surgeons scale (ASES), the Pittsburgh Sleep Quality Index (PSQI), the Beck Depression Inventory II (BDI-II), and the State-Trait Anxiety Inventory (STAI). Cognitive function was assessed using the Montreal Cognitive Assessment (MoCA), Digit

Effect of preoperative sleep quality on rapid postoperative recovery from non-traumatic rotator cuff injuries.

BACKGROUND: Global aging has led to a continuous increase in rotator cuff injuries, often accompanied by clinical-related sleep disorders. There is a lack of clinical research on whether preoperative sleep quality affects rapid postoperative recovery; therefore, it is necessary to conduct clinical follow-up studies to explore the relationship between preoperative sleep quality and postoperative rehabilitation, in order to provide rehabilitation guidance for clinical practice. METHODS: In this prospective cohort study, 256 patients who underwent arthroscopic rotator cuff repair (ARCR) at our c

Macrophages in rotator cuff injuries and tendon-to-bone healing: Heterogeneity, pathophysiology, and therapeutic strategies.

Rotator cuff tears (RCTs) are a leading cause of shoulder disability, with high postoperative failure rates primarily attributed to poor tendon-to-bone healing. Macrophages act as central regulators of the inflammatory and regenerative microenvironment in the injured enthesis, tendon stump, subacromial bursa, and rotator cuff musculature. This review highlights the spatiotemporal heterogeneity and phenotypic plasticity of macrophages revealed by single-cell and spatial transcriptomics. Rather than following a simple M1-to-M2 transition, macrophages adopt context-dependent inflammatory, resolvi

Understanding Fibrous Tissue in the Effective Healing of Rotator Cuff Injury.

The rotator cuff is a crucial group of muscles and tendons in the shoulder complex that plays a significant role in the stabilization of the glenohumeral joint and enabling a wide range of motion. Rotator cuff tendon tears can occur due to sudden injuries or degenerative processes that develop gradually over time, whether they are partial or full thickness. These injuries are common causes of shoulder pain and functional impairment, and their complex nature highlights the essential role of the rotator cuff in shoulder function. Scar formation is a crucial aspect of the healing process initiate
