OBJECTIVES: To investigate the immunological characteristics and predictive factors of chronic graft-versus-host disease (cGVHD) in children after haploidentical allogeneic hematopoietic stem cell transplantation (Haplo-HSCT) to provide a basis for early intervention.
METHODS: A retrospective analysis was conducted on clinical and laboratory data of 64 children undergoing Haplo-HSCT. Peripheral blood lymphocyte subsets, immunoglobulins, and key components of the complement system were measured. Univariable and multivariable logistic regression analyses were used to identify predictors.

In two patients a peripheral neuropathy was the presenting symptom of a noncutaneous peripheral T-cell lymphoma. We report the case of a patient suffering from mycosis fungoides and plasma cell myeloma. It is postulated that this is not a fortuitous association, but that the B cell neoplasm evolved under the sustained inducer stimulus of the malignant T lymphocytes. This hypothesis is suggested by the demonstration of the helper phenotype of the cutaneous lymphocytes associated with a predominance of helper lymphocytes in blood.

A generalized state of immunosuppression during surgery has been implicated in the development of septic complications postoperatively. We studied 18 patients operated upon for benign diseases, to examine the influence of surgical trauma on circulating lymphocyte subpopulations in man. Additionally, we evaluated the effect of thymostimulin on these changes, in another group of 10 patients. The lymphocytes of two unrelated black individuals exhibited no immunofluorescent staining by a monoclonal antibody, OKT4, that reacts with T helper/inducer cells, but the lymphocytes reacted normally with four other monoclonal antibodies that identify T helper cells. Four first-degree relatives of these individuals were available for study. They had a normal proportion of OKT4+ lymphocytes but these cells had approximately half the normal number of OKT4 sites. This abnormality appears to be inherited as an autosomal recessive state.

Amebic liver abscess (ALA) is associated with a state of transient suppression of cell-mediated immunity (CMI). T4 helper-inducer cell number is reported to be reduced. However, little is known of the reason for such reduction. Twenty five patients with alopecia areata and 20 healthy controls were studied by flowcytometry employing direct two colour immunofluorescence in erythrocyte lysed whole blood. Alopecia areata remains a disease of unknown cause and unsatisfactory treatment. Histologically it is characterized by a lymphocytic infiltrate that surrounds the lower half of the hair follicle. Our findings show that 64% to 92% of this infiltrate is composed of T lymphocytes bearing the Leu 3a (helper-inducer) phenotype.
