Tarsal coalition in paediatric patients.

Tarsal coalition is due to failure of segmentation between two or more foot bones during embryological development at a site where the joint cleft fails to develop. Depending on the nature of the tissue connecting the bones, the abnormality is a syndesmosis, synchondrosis, or synostosis. Although the coalition exists at birth, synostosis usually develops only late during growth. Talo-calcaneal and calcaneo-navicular coalitions account for over 90% of all cases of tarsal coalition. The joint at the site of the coalition is stiff. Pain during physical activity is the main symptom, although recur

Tarsal coalition and painful flatfoot.

The prevalence of tarsal coalition is probably 1% or less. The two sites most commonly affected are the calcaneonavicular joint and the middle facet of the talocalcaneal joint. Diagnosis should be suspected in the preteen or teenage patient with insidious or sudden onset of pain in the midfoot to hindfoot associated with a lack of motion in the subtalar joint. Initial treatment with immobilization or an orthosis may relieve symptoms, but most patients will have persistent symptoms that warrant surgical correction. Long-term results indicate that excision of the coalition is moderately successf

Computed tomography in tarsal coalition.

The limitations of routine radiography of the feet in demonstrating tarsal coalitions are well known. Even with the use of multiple projections of the foot, tarsal coalitions may escape detection. Computed tomographic examinations of the feet were performed in persons suspected of having tarsal coalitions. The CT images were obtained in both the longitudinal and axial axes of the foot. Results of these examinations suggest the longitudinal projection to be most helpful in demonstrating talonavicular coalitions and the axial projection in demonstrating talocalcaneal coalitions. Endoscopic Resection of Tarsal Coalitions.

Endoscopic resection of tarsal coalitions is technically feasible for both talocalcaneal and calcaneonavicular coalitions. Careful consideration of each individual patient is necessary before proceeding with endoscopic resection. Endoscopic resection of these coalitions may offer benefits in terms of faster recovery and less wound problems, but this has not been proven. Several case reports and case series appear in the literature and are reviewed here along with the different techniques reported. Better-quality evidence is required to assess the clinically relevant benefits and the recurrence

Tarsal coalitions: an instructional review.

Tarsal coalitions, the majority of which are congenital, are abnormal unions between two or more tarsal bones. Acquired tarsal coalitions may result from trauma or from rheumatic conditions. Tarsal coalition may cause peroneal spastic flatfoot or rigid valgus foot without peroneal spasm. Surgery is not always essential in the treatment of tarsal coalitions; some patients will respond to conservative treatment.

[The Grice technique for tarsal coalition].

OBJECTIVE: Show that the surgical release of the coalition area and the Grice subtalar arthrodesis are useful for the treatment of tarsal coalition, and describe the surgical techniques used. MATERIAL AND METHODS: This is a prospective, longitudinal, non-randomized clinical trial conducted from January 2001 to December 2007. Patients of both genders were selected, ages 10-15 years of age, with a diagnosis of tarsal coalition without any other type of orthopedic or neurologic comorbid condition. The Harris classification of tarsal coalition was used. AP, lateral and oblique X-rays of both feet

The rigid flatfoot. Tarsal coalitions.

Tarsal coalitions are the major cause of painful rigid flatfoot deformity in children and adolescents. Talocalcaneal and calcaneonavicular coalitions are the most common sites. They are often bilateral and may be asymptomatic. They represent a failure of fetal mesenchymal differentiation, and onset of symptoms in children often corresponds to the time of ossification of the fibrous or cartilaginous coalition. The most common presenting symptom is pain, and diagnosis is aided by plain radiographs and computerized tomography. Some patients respond to conservative measures, but surgical treatment

The relative incidence of tarsal coalition.

The overall incidence of tarsal coalition in the general population is unknown, but it is far less than 1%. Calcaneonavicular coalition was found to be the most common type (53%), followed by talocalcaneal coalition (37%). The other types appear infrequently. Most calcaneonavicular and talocalcaneal coalitions present as a painful foot of some type. Peroneal spastic flatfoot is a rare finding and occurred in only two of 22 talocalcaneal coalitions in the present study. Talonavicular coalitions were always associated with other congenital foot anomalies and were typically asymptomatic. Age at p

Diagnosing tarsal coalition in medieval Exeter.

OBJECTIVE: The objective of this study is to estimate the frequency and types of tarsal coalition represented in medieval remains from Exeter, England. MATERIALS: 183 individuals from the Cathedral Green site in Exeter, England. METHODS: Gross macroscopic analysis combined with radiographic examination and comparison with clinical and archaeological literature. RESULTS: Eight coalitions of various forms were ultimately identified; these include five calcaneonavicular, one talocalcaneal, one calcaneocuboid, and one lateral cuneiform-third metatarsal coalition. CONCLUSIONS: These frequencies

Tarsal coalition: Systematic review of arthroscopic management.

INTRODUCTION: The standard of care in the treatment of symptomatic tarsal coalitions is open surgery. However, certain limitations exist with open surgery, which include limited visualization leading to an incomplete resection and possible recurrence of the tarsal coalition. Arthroscopic tarsal coalition resection (TCR) is an alternative that is gaining traction, primarily as the safety profile of posterior ankle and subtalar arthroscopy is more well understood. This study provides a systematic review of the outcomes of arthroscopic TCR. METHODS: PubMed and Embase were searched independently

Management of Complex Tarsal Coalition in Children.

Complex tarsal coalition includes extensive talocalcaneal coalition, double or triple coalition, coalition with severe hindfoot deformities, or coalition with a ball-and-socket ankle deformity. Careful preoperative physical examination including diagnostic injection is important in treatment planning. Both radiographic examination and computed tomographic scan that involve not only the foot but also the ankle are necessary to analyze the location and size of the coalitions, determining the presence of arthritis in the involved or adjacent joints, and if there are any deformities including a ba

[Tarsal-carpal coalition syndrome: a familial case].

Tarsal-carpal coalition syndrome (TCC, OMIM #186570) is an autosomal dominant disorder characterised by fusion of the carpals, tarsals, and phalanges, with the short first metacarpals causing brachydactyly and humeroradial fusion. Mutations in the NOG gene have been reported in many families. We describe a family with carpal tarsal fusion seen at a Skeletal Dysplasia Clinic and look at the differential diagnoses.

Rigid painful flatfoot secondary to tarsal coalition.

Rigid flatfoot secondary to tarsal coalition requires proper clinical and roentgenographic evaluation. In patients with limited subtalar motion and pain in the tarsal area, a coalition should be suspected. Proper roentgenographic evaluation with standard anteroposterior, lateral, and oblique views is essential. Calcaneonavicular coalition, as visible on an oblique film, may be a solid bony fusion or, more often, a cartilaginous coalition, which is characterized by flattening of the calcaneus and navicular at their junction. Axial (Harris) views demonstrate coalition in the middle and posterior

Excision of a Middle Facet Tarsal Coalition.

UNLABELLED: Middle facet tarsal coalition is one of the commonly seen tarsal coalitions in clinical practice that can cause pain and associated flatfoot deformity. Excision of the coalition is one of the treatment options for symptomatic cases. Although symptoms may subside in children following a period of immobilization, resection should be considered as a treatment alternative for children and adolescents because of the potential for restoration of subtalar joint movement. The indications for excision of the coalition are not consistently reported in the literature, and the procedure is not

Total Bilateral Tarsal Coalition: A Case Report.

A 35-year-old male presented with pain and swelling of his right foot and ankle. His pain developed gradually and without overt pedal deformity. The radiologic examination revealed complete fusion of all tarsal bones of his right foot. Comparative radiographs of his left foot also showed complete tarsal coalition. Ankle osteoarthritis was diagnosed, and supportive treatment enabled the patient to return to an asymptomatic status and continue the duties of his employment. This case is presented to describe an adult patient with bilateral, massive tarsal coalition who was essentially asymptomati

[Progress in diagnosis and treatment of tarsal coalition].

OBJECTIVE: To review the progress in the diagnosis and treatment of tarsal coalition. METHODS: Recent literature concerning the diagnosis and treatment of tarsal coalition was reviewed. RESULTS: Tarsal coalition is a bridge between the tarsal bones of the foot. The most common types are talocalcaneal and calcaneonavicular coalitions. Calcaneonavicular coalitions can be diagnosed with an oblique radiograph of the hindfoot. Most talocalcaneal coalitions require computer tomography for diagnostic confirmation. Magnetic resonance imaging may be useful for diagnoses of cartilaginous and fibrous c
