Prophylaxis against ventricular arrhythmias in suspected acute myocardial infarction: a comparison of tocainide and disopyramide.

Five hundred and seventy one patients admitted to a coronary care unit with suspected acute myocardial infarction were considered for entry into a double-blind study. Two hundred and eighty-three patients were excluded, mainly because of recent treatment with beta-adrenoceptor blocking agents, life threatening arrhythmias requiring specific treatment and left ventricular failure presenting with hypotension or pulmonary oedema. Two hundred and eighty-eight entered the trial of whom 202 were subsequently confirmed to have had myocardial infarction. The effects of tocainide and disopyramide on ve

Comparative effects of procainamide, tocainide and lorcainide on Na(+)-K(+)-ATPase in guinea pig heart preparations.

1. The effects of three class 1 antiarrhythmic drugs procainamide (class 1A) tocainide (class 1B) and lorcainide (class 1C) on microsomal Na(+)-K(+)-ATPase activity were compared with those of ouabain in guinea pig heart preparations. 2. All three antiarrhythmic drugs exhibited concentration-dependent inhibitory actions on the enzyme activity in a fashion similar to that of ouabain. 3. The rank order of their potencies showed the following tendency: lorcainide much greater than tocainide greater than procainamide. However, while the actions of lorcainide were comparable to those of cardiotonic

Pharmacokinetics and haemodynamic effects of tocainide in patients with acute myocardial infarction complicated by left ventricular failure.

The pharmacokinetics and haemodynamic effects of tocainide, an orally active structural analogue of lignocaine, were studied in patients with acute myocardial infarction complicated by left ventricular failure. Fourteen patients (mean age 65 years) with acute myocardial infarction complicated by mild left ventricular failure were studied, following a single dose of tocainide (250 mg) by intravenous infusion, over 30 min. Heart rate, systemic arterial pressure, pulmonary artery pressure and cardiac output were monitored. Plasma tocainide levels were estimated by gas chromatography. The mean pla

New potent mexiletine and tocainide analogues evaluated in vivo and in vitro as antimyotonic agents on the myotonic ADR mouse.

The antimyotonic activity of chiral derivatives of mexiletine and tocainide, selected as potent use-dependent blockers of skeletal muscle sodium channels, was evaluated in vivo acutely in myotonic ADR mice. The compounds had either aromatic (Me4 and Me6) or branched isopropyl groups (Me5 and To1) on the asymmetric centre, or had this latter one methylene apart from the amino group (Me2). Therapeutic doses of mexiletine (5-10 mg/kg) and tocainide (7-20 mg/kg) significantly reduced the long time of righting reflex (TRR), typical of ADR mice. Me4, Me5 and Me6 were 2-fold more potent than mexileti

Effect of tocainide on the concentrations of norepinephrine dopamine, serotonin and histamine in rat brain and its binding affinity to neuroceptors.

Tocainide (W-36095, Xylotocan) is a derivative of lidocaine which is applied clinically as a cardiac antiarrhythmic. In addition, it diminishes the severity of symptoms in neurological disorders such as myotonia and paramyotonia and in patients with torticollis spasmodicus it brings about a distinct reduction in the tension of the neck muscles. It was investigated whether in reducing the tonus in torticollis spasmodicus tocainide could be acting on the central nervous system. The binding affinity of the substance to the dopamine, serotonin, opiate, GABA and benzodiazepine receptors was measure

Efficacy and safety of intravenous tocainide compared with intravenous lidocaine for acute ventricular arrhythmias immediately after cardiac surgery.

In this double-blind parallel study, 99 patients with acute ventricular tachyarrhythmias after open-heart surgery were given either tocainide (50 patients) or lidocaine (49 patients) intravenously as 2 bolus injections 15 minutes apart, plus a fixed-rate infusion that started at the first bolus. If needed, a third bolus was administered and simultaneously the infusion rate was doubled. The boluses and initial infusion rate for tocainide treatment were, respectively, 250, 250 and 125 mg and 1.04 mg/min, and for lidocaine treatment, 100, 50 and 50 mg and 2.08 mg/min. Liquid chromatographic determination of mexiletine and tocainide in human plasma with fluorescence detection after reaction with a modified o-phthalaldehyde reagent.

A liquid chromatographic procedure for the determination of mexiletine or tocainide in human plasma is described. Plasma, after the addition of a homologue of mexiletine or of tocainide, is extracted with dichloromethane. The extract is evaporated and reconstituted in a non-aqueous o-phthalaldehyde--mercaptoethanol reagent. An aliquot of the solution is chromatographed on a reversed-phase Ultrasphere-octyl column. The peaks are detected by fluorescence (lambda ex = 350 nm and lambda em = 445 nm). The fluorescent derivatives of the drugs and internal standards are stable at room temperature and

[The effect of anti-arrhythmic drugs on myocardial function (author's transl)].

In a randomized study the influence of anti-arrhythmic drugs on left ventricular myocardial function was tested. Propafenon, mexiletine, tocainide, disopyramide or placebo were given over seven days on top of basic treatment with digitalis, diuretics and nitrates. Propafenon, mexiletine, tocainide and disopyramide had negative inotropic effects of 16, 17, 19 to maximally 30%, respectively. Echocardiography was especially suitable for serial assessment of left ventricular failure. Propafenon, mexiletine and tocainide are the anti-arrhythmic drugs of choice in the treatment of cardiac arrhythmia

The systemic administration of local anaesthetics produces a selective depression of C-afferent fibre evoked activity in the spinal cord.

An electrophysiological analysis of the antinociceptive effects of systemic lidocaine and its longer acting primary amine congener, tocainide, has been performed in the decerebrate-spinal unanaesthetised rat. Neither of these local anaesthetic drugs when administered systemically in doses of up to 10 mg/kg (lidocaine) or 100 mg/kg (tocainide), produced any evidence of a block in the conduction of action potentials in A beta, A delta or C primary afferents. The local anaesthetics also failed to reduce mustard oil induced neurogenic extravasation, a test of cutaneous C-fibre terminal function. L

Effects of intrathecal local anesthetics on spinal excitability and on the development of autotomy.

The effect of 500 micrograms intrathecal (i.t.) lidocaine and tocainide injected prior to unilateral sciatic nerve section on the development of autotomy behavior was examined in rats. Neither drug inhibited autotomy compared with saline controls. The effect of lidocaine and tocainide on the flexor reflex in decerebrate, spinalized rats was also examined. Both drugs at 500 micrograms i.t. totally blocked the flexor reflex and hyperexcitability following acute nerve section, with a longer duration of action with tocainide. Recovery from the drugs was followed by a prolonged period of reflex hyp

Failure of mexiletine to control trigeminal neuralgia.

The analgesic effects of lidocaine and tocainide on trigeminal neuralgia have been established. However, both drugs are unpractical: lidocaine can only be used intravenously, and tocainide may exhibit serious haematological side effects. Mexiletine, a structural analogue of lidocaine that can be safely administered by the oral route, was given, as the sole drug, to four patients with active trigeminal neuralgia. After at least seven days on mexiletine they had no clear benefit. The four patients subsequently improved with carbamazepine or phenytoin. Our observations suggest that mexiletine alo

Treatment of myotonia with antiarrhythmic drugs.

The effects of disopyramide, phenytoin, mexiletine, and tocainide were compared in 30 patients with myotonic disorders. The severity of myotonia was assessed by clinical and electromyographic criteria at the end of each treatment phase lasting four weeks. Mexiletine (MXT) and tocainide (TCD) were found to be the most potent antimyotonic agents. The antimyotonic efficacy of MXT and TCD is explained by their fast-blocking effect on voltage-dependent sodium channels in the muscle membrane. The benefits of myotonia control with pharmacological agents must be weight against the risk of therapy in t

Lipidosis-like alterations in cultured macrophages exposed to local anaesthetics.

In this ultrastructural study, the simple model of cultured rat peritoneal macrophages was used to examine whether local anaesthetics can induce lipidosis-like alterations. Exposure (24h) of macrophages to 1 X 10(-5) M dibucaine, or to 5 X 10(-5) M tetracaine, quinidine, and quinine, respectively, led to the occurrence of lamellated cytoplasmic inclusions in most cells. This is interpreted as indicating lipidosis. Type and degree of alterations were similar to those induced by the reference compound chlorphentermine (5 X 10(-5) M) for which lipidosis has previously been shown by biochemical me

Potential anti-thrombotic effects of local anaesthetics due to their inhibition of platelet aggregation.

The aim was to study the possible anti-aggregating effects of local anaesthetics on platelets stimulated by physiological doses of adenosine-diphosphate and collagen. Platelet-rich plasma was therefore incubated in an aggregometer with lidocaine, bupivacaine or tocainide in various concentrations and for different incubation times. It was found that of the local anaesthetics tested, lidocaine was the most effective anti-aggregating compound. Furthermore, the longer the incubation time with the different local anaesthetics, the more efficient the anti-aggregating effect. These results may have

New drugs for treating cardiac arrhythmias.

The new and potent antiarrhythmic drugs herald an exciting era in the treatment of cardiac arrhythmias. Encainide, tocainide, and mexiletine are effective in treating acute and chronic ventricular arrhythmias. Verapamil promises to be the drug of choice for paroxysmal supraventricular tachycardia. Acetubolol provides the advantages of selective beta receptor blockade and has minimal influence on pulmonary function. Aprindine and amiodarone block conduction in accessory pathways and are very suitable for treating tachycardias associated with Wolff-Parkinson-White syndrome. Ethmozin is a phenoth

Antithrombotic effects of lidocaine and related compounds on laser-induced microvascular injury.

The aim was to study whether topically applied local anaesthetics and related compounds exert an antithrombotic effect. The assay was carried out through vital microscopy of the microcirculation in the hamster cheek pouch model as injured by laser microbeam irradiation, essentially in order to record the differences in the incidence of thrombus formation between two main experimental and control series. The application of lidocaine hydrochloride was found to inhibit thrombus formation and also to restore the microcirculation after laser-induced injury. The other investigated compounds, mono-et

Non-antiepileptic drugs for trigeminal neuralgia.

BACKGROUND: Trigeminal neuralgia was defined by the International Association for the Study of Pain as a sudden, usually unilateral, severe, brief, stabbing recurrent pain in the distribution of one or more branches of the fifth cranial nerve. Standard treatment is with anti-epileptic drugs. Non-antiepileptic drugs have been used in the management of trigeminal neuralgia since the 1970s. This is an update of a review first published in 2006 and previously updated in 2011. OBJECTIVES: To systematically review the efficacy and tolerability of non-antiepileptic drugs for trigeminal neuralgia. S
