Mercury accumulation in hydrothermal vent mollusks from the southern Tonga Arc, southwestern Pacific Ocean.

We provide the mercury (Hg) and monomethylmercury (MMHg) levels of the plume water, sulfide ore, sediment, and mollusks located at the hydrothermal vent fields of the southern Tonga Arc, where active volcanism and intense seismic activity occur frequently. Our objectives were: (1) to address the potential release of Hg from hydrothermal fluids and (2) to examine the distribution of Hg and MMHg levels in hydrothermal mollusks (mussels and snails) harboring chemotrophic bacteria. While high concentrations of Hg in the sediment and Hg, As, and Sb in the sulfide ore indicates that their source is

Reduction in asthma morbidity following a community-based asthma self-management programme in Tonga.

BACKGROUND: The Tonga Asthma Self-management Project assessed whether the introduction of an asthma self-management plan would reduce asthma morbidity. METHODS: The project involved a 'before and after' trial, with each participant serving as his/her own control. Asthma patients used the self-management plan to manage their asthma medication or obtain medical help based on their peak expiratory flow (PEF) rate and/or asthma symptoms. RESULTS: The 110 participants initially had relatively high asthma morbidity: 68% had had an emergency medical visit for asthma in the previous 12 months. Ninet

Oral rehydration salts and diarrhoeal diseases: effects of changing inpatient management in Tonga.

Methods of inpatient management of children with diarrhoeal diseases (DD) in 1978 and in 1980 were compared to assess the use of oral rehydration salts (ORS) and a national antidiarrhoeal programme (NAP). Inpatient notes of 369 children admitted with DD were reviewed. The management of DD differed significantly but the clinical outcome was similar, with the exception of the case-fatality rate. All deaths occurred within the first 24 h of admission, indicating that this difference was attributable to improved management of DD in the community rather than changes in the inpatient treatment regim

Multi-pathogen serosurveillance in Tonga examining demographic correlates and geographic clustering of 17 pathogens.

Integrated serosurveillance enables concurrent assessment of population immunity to vaccine-preventable diseases (VPDs) and infectious pathogens. We aimed to estimate seroprevalence of antibodies (Abs) to 17 pathogens to provide insights to inform public health strategies to control and eliminate infectious diseases in Tonga. A targeted cross-sectional survey was conducted in May-July 2024 among participants aged ≥ 5 years from Tongatapu, Ha'apai, and Ongo Niuas. Dried blood spots were collected and assayed on a multiplex bead assay (MBA) containing 26 antigens from 17 pathogens. Seropositivit

Intentional injury reported by young people in the Federated States of Micronesia, Kingdom of Tonga and Vanuatu.

BACKGROUND: Intentional injury presents a threat to the physical and psychological well being of young people, especially in developing countries, which carry the greatest part of the global injury burden. While the importance of this problem is recognized, there are limited population data in low and middle income countries that can guide public health action. The present study investigates the prevalence and distribution of intentional injury among young people in three Pacific Island societies, and examines behavioural and psychosocial factors related to risk of intentional injury. METHODS

Satellite quantification of enhanced methane oxidation applied to the stratospheric plume following Hunga Tonga-Hunga Ha'apai eruption.

Methane is a powerful greenhouse gas whose atmospheric sink remains uncertain, and emerging strategies to enhance its removal will require quantification and monitoring to verify any hypothetical future methane removal. Here we present satellite quantification of enhanced atmospheric methane oxidation, based on TROPOMI observations of a short-lived intermediate in methane oxidation, HCHO. We find a large HCHO enhancement of up to 12 ppb±10% at 30 km altitude, in the plume from the Hunga Tonga-Hunga Ha'apai eruption, persisting for ten days or more, and also explaining its low BrO levels. Effects of inappropriate cause-of-death certification on mortality from cardiovascular disease and diabetes mellitus in Tonga.

BACKGROUND: Cardiovascular disease (CVD) and diabetes mellitus are major health issues in Tonga and other Pacific countries, although mortality levels and trends are unclear. We assess the impacts of cause-of-death certification on coding of CVD and diabetes as underlying causes of death (UCoD). METHODS: Tongan records containing cause-of-death data (2001-2018), including medical certificates of cause-of-death (MCCD), had UCoD assigned according to International Classification of Diseases 10th revision (ICD-10) coding rules. Deaths without recorded cause were included to ascertain total morta

Informing the introduction of contraceptive implants in the Pacific: a mixed methods study of contraceptive beliefs and behaviours in Tonga.

OBJECTIVE: To explore the knowledge and attitudes underlying contraceptive use and non-use among Tongan adults and investigate the potential acceptability of contraceptive implants. METHODS: A mixed methods study was conducted that used two focus groups and a short self-completed survey (n=109). Participants were recruited from a Tongan non-governmental sexual and reproductive health clinic. RESULTS: Contraceptive method discontinuation was high among the study group, with reasons for discontinuation often relating to misconceptions circulating in the community about the side effects of cont

Identification of Source of Brucella suis Infection in Human by Using Whole-Genome Sequencing, United States and Tonga.

Brucella suis infection was diagnosed in a man from Tonga, Polynesia, who had butchered swine in Oregon, USA. Although the US commercial swine herd is designated brucellosis-free, exposure history suggested infection from commercial pigs. We used whole-genome sequencing to determine that the man was infected in Tonga, averting a field investigation.

Pilot assessment of soil-transmitted helminthiasis in the context of transmission assessment surveys for lymphatic filariasis in Benin and Tonga.

BACKGROUND: Mass drug administration (MDA) for lymphatic filariasis (LF) programs has delivered more than 2 billion treatments of albendazole, in combination with either ivermectin or diethylcarbamazine, to communities co-endemic for soil-transmitted helminthiasis (STH), reducing the prevalence of both diseases. A transmission assessment survey (TAS) is recommended to determine if MDA for LF can be stopped within an evaluation unit (EU) after at least five rounds of annual treatment. The TAS also provides an opportunity to simultaneously assess the impact of these MDAs on STH and to determine

RE-AIM evaluation of a community-based vaccine education and communication program to improve human papillomavirus vaccine uptake in Tonga.

There is a global need for effective strategies to improve acceptance and uptake of the human papillomavirus (HPV) vaccine, particularly in the Pacific where cervical screening and treatment options are limited. While the Pacific Island nation of Tonga has high routine childhood vaccine coverage, HPV vaccine uptake has remained low since its introduction in 2022. To improve vaccine acceptance and uptake, recent evidence supports the use of community engagement approaches like the Vaccine Champions program, which trains health and community leaders to advocate for vaccines. We assessed the reac

Understanding mariners' tsunami information needs and decision-making contexts: A post-event case study of the 2022 Tonga eruption and tsunami.

The 15 January 2022, Hunga Tonga Hunga Ha'apai volcanic eruption generated a tsunami that spread across the Pacific Ocean and prompted a tsunami advisory in Aotearoa New Zealand (NZ). Concurrently, a severe weather warning was issued for ex-Tropical Cyclone Cody, passing east of NZ and producing heightened swells along the North Island coast. Numerous boats were significantly damaged or sunk in Tūtūkākā Marina, Northland, NZ. Mariners raised concerns over the perceived lack of tsunami warnings. We interviewed mariners in Tūtūkākā to understand their experiences on the night of 15 January 2022

The structural drivers of violence against children in Fiji, Papua New Guinea, Solomon Islands, Tonga and Vanuatu: An exploratory review.

BACKGROUND: Pacific Island countries have the highest rates of violent discipline, child labour and bullying in the Asia Pacific region, yet there is limited research on the drivers of violence against children in these contexts. OBJECTIVE: To address this gap, we explored the drivers of violence against children in the four most populous Pacific Island countries, Fiji, Papua New Guinea, Solomon Islands and Vanuatu, as well as Tonga where Save the Children, who commissioned this review, planned to commence child protection programming. METHODS: We undertook an exploratory literature review i

Persistent lymphatic filariasis transmission seven years after validation of elimination as a public health problem: a cross-sectional study in Tonga.

BACKGROUND: The World Health Organization (WHO) has validated 21 countries as having eliminated lymphatic filariasis (LF) as a public health problem. Post-validation surveillance (PVS) is required in countries where LF has been eliminated. Tonga eliminated LF in 2017, but no PVS strategy has been established. We aimed to identify any persistent LF transmission in Tonga in 2024 and provide evidence to support a PVS strategy. METHODS: A four-pronged, targeted, cross-sectional study was conducted in the Tongan districts of Tongatapu, Ha'apai, and Ongo Niuas in May-July 2024 in communities, prima

Rapid Behavioral Assessment of Knowledge, Attitudes, and Practices on Dengue Fever to Inform Risk Communication and Community Engagement Efforts in the Kingdom of Tonga.

In February 2025, Tonga's Minister of Health declared a national dengue fever epidemic, emphasizing the urgent need to raise public awareness and implement preventative measures. In response to this declaration, a study was conducted from March to May 2025 to investigate the sociodemographic determinants affecting dengue fever awareness and preventative behaviors across Tonga's primary islands: Tongatapu, Ha'apai, and Vava'u. Researchers conducted a survey of 140 randomly selected households, achieving a 93% response rate. The results revealed that although 90% of respondents were aware of den

Operational research to inform post-validation surveillance of lymphatic filariasis in Tonga study protocol: History of lymphatic filariasis elimination, rational, objectives, and design.

BACKGROUND: Lymphatic filariasis (LF), a mosquito-borne helminth infection, is an important cause of chronic disability globally. The World Health Organization has validated eight Pacific Island countries as having eliminated lymphatic filariasis (LF) as a public health problem, but there are limited data to support an evidence-based approach to post-validation surveillance (PVS). Tonga was validated as having eliminated LF in 2017 but no surveillance has been conducted since 2015. This paper describes a protocol for an operational research project investigating different PVS methods in Tonga

Behavioural and social drivers of HPV vaccine uptake in Tonga: a mixed-methods study using the WHO BeSD framework and Talanoa in a Pacific Island setting.

BACKGROUND: Small Pacific Island nations sustain a disproportionate cervical cancer burden. Despite an excellent routine immunization program, HPV vaccine coverage in Tonga remains consistently low after a decade of programming. We used the WHO Behavioural and Social Drivers (BeSD) framework to identify drivers of HPV vaccine uptake among caregivers and to examine the underlying contextual mechanisms shaping vaccination decisions in this Pacific Island setting. METHODS: A mixed-methods cross-sectional study was conducted in August 2025 across all five main island groups of Tonga. We enrolled
