Latent image fading in screen-film mammography: lack of clinical relevance for batch-processed films.

PURPOSE: To assess the clinical relevance of latent image fading due to time delays in batch-processed screening mammograms. MATERIALS AND METHODS: Results of all 66,667 mammographic examinations performed in a mobile van since April 1985 were retrospectively analyzed. Film was routinely batch processed at the end of the workday. The rate of repeat examination due to poor image quality, rate of abnormal interpretation, and rate of cancer detection were studied. RESULTS: Essentially no differences were found in the rates at which repeat examination was necessary, abnormal interpretations were

Efficacy of digital radiography for the detection of pneumothorax: comparison with conventional chest radiography.

As part of our continuing evaluation of the clinical applicability of digital radiography, we compared the abilities of radiologists to detect pneumothoraces on conventional chest radiographs with their performances when using three formats of digitally obtained images. Twenty-three frontal-view chest radiographs with pneumothoraces and 22 other chest radiographs, either normal or showing miscellaneous abnormalities, were interpreted by five experienced radiologists in each of four formats: conventional film-screen chest radiographs, small-format (17.8 x 21.6 cm) computed radiographs, large-fo

Using light sensitometry to evaluate mammography film performance.

The performance of commercially available light sensitometers was compared with two other methods of x-ray sensitometry to determine whether commercially available sensitometers are viable for evaluating clinical performance of mammography film. X-ray sensitometry was performed using mammography screens that were modified to accommodate a graded optical step tablet (screen sensitometry). Finally, a means for performing intensity-scale x-ray sensitometry was configured (inverse-square sensitometry). Clinical mammography x-ray exposure conditions were used and film processing quality was closely

[X-ray dose reduction in examination of infant's hip with image-intensifier-fluoroscopy-system (author's transl)].

An image intensifier--70mm camera--fluoroscopy system combined with a conventional X-ray tube is used for routine examination of the infant's hip. The high quality of 85% of these 70mm exposures makes immediate diagnostic interpretation possible. An even higher percentage can be achieved by most exact positioning of the patients. Apart from the radiation protection of the gonades, the X-ray skin dose of the primary beam is reduced by 90-95% compared with full size radiography using film screen combinations. This exposure technique can certainly be used not only for the examination of the infan

Design of a PC controlled test device for the study of patient motion in X-ray radiology: first applications and results.

Simulation and study of patient motion should be undertaken for different imaging techniques since all new radiological techniques or equipment must pass through evaluation processes to verify their functional capability and assess their performance compared with other equipment. A device to study the effects of motion on X-ray image quality was designed and built. The main goal of the PC controlled device developed by our group is to allow the adaptation of different phantoms and test objects. Preliminary experiments for the evaluation of different thorax screen-film systems were carried out

[Reduction of the radiation exposure in photofluorography by using rare earth-based fluorescent screens].

The use of highly sensitive fluorescent screens based on gadolinium in photofluorography results in considerable reduction of radiation exposure compared with conventional screens. The exposure to radiation can be reduced to a value which is hardly higher than that of a large format image. The image quality is enhanced by the incorporation of new screens because of shorter exposure times. Gadolinium foils with high image amplification are already available on a commercial basis. Exchanging them in photofluorograph equipment presents no problem.

Clinical comparison of conventional and rare earth screen-film systems for cephalometric radiographs.

This study compared cephalometric and P-A skull films taken with conventional (CaWO4) screens and rare earth screens. Patient exposure was reduced by 17 to 55 percent on two different x-ray machines by the use of rare earth screens. Results from 130 clinical evaluations showed that the diagnostic quality of radiographs taken with either system was roughly comparable. This presents a persuasive argument for the use of rare earth screens since the diagnostic quality of the films can be maintained while significantly reducing the patient's exposure to radiation.

Comparison between differently priced devices for digital capture of X-ray films using computed tomography as a gold standard: a multireader-multicase receiver operating characteristic curve study.

OBJECTIVE: Film digitizers are a specialized technology that is available for scanning X-ray radiographs; however, their cost makes them unaffordable for developing countries. Thus, less expensive alternatives are used. The purpose of this study was to compare three devices for digital capture of X-ray films: a film digitizer (US $15,000), a flatbed scanner (US $1800), and a 10-megapixel digital camera (US $450), in terms of diagnostic accuracy, defined as the area under the receiver operating characteristic curves and computed tomography as the gold standard. MATERIALS AND METHODS: The sampl

A clinical comparison between conventional and digital mammography utilizing computed radiography.

Previous work utilizing a specially designed mammographic test object has indicated that the image quality of computed radiography (CR) approaches that of a conventional film-screen technique of equivalent radiographic speed. In an attempt to correlate these results with the clinical situation a subjective rating study of the primary physical parameters of noise, sharpness, and contrast has been conducted. Using a randomized viewing sequence two radiologists experienced in mammography were asked to score these parameters utilizing a 5 point scale. A total of 138 images was used, 62% containing

[Diagnosis of metastases in thoracic images. Progress by further development of the conventional film-foil technique?].

In addition to digital radiography and AMBER, the development of asymmetric screen-film systems is another attempt to optimize chest radiography. Due to reduced contrast in the parenchyma, the former asymmetric screen-film systems did not show sufficient image quality. Three new asymmetric systems with completely different composition are available now. In-Sight VHC (Kodak), High Light GUV (3M) and Opthos D (Agfa) were compared to standard chest films using densitometric curves, a chest phantom for high and low contrast detectability, a nodule detection phantom and patient studies. Monte Carlo study on the imaging performance of powder Lu2SiO5:Ce phosphor screens under x-ray excitation: comparison with Gd2O2S:Tb screens.

Lu2SiO5: Ce (LSO) scintillator is a relatively new luminescent material which has been successfully applied in positron emission tomography systems. Since it has been recently commercially available in powder form, it could be of value to investigate its performance for use in x-ray projection imaging as both physical and scintillating properties indicate a promising material for such applications. In the present study, a custom and validated Monte Carlo simulation code was used in order to examine the performance of LSO, under diagnostic radiology (mammography and general radiography) conditi

Radiation dose reduction using a CdZnTe-based computed tomography system: comparison to flat-panel detectors.

PURPOSE: Although x-ray projection mammography has been very effective in early detection of breast cancer, its utility is reduced in the detection of small lesions that are occult or in dense breasts. One drawback is that the inherent superposition of parenchymal structures makes visualization of small lesions difficult. Breast computed tomography using flat-panel detectors has been developed to address this limitation by producing three-dimensional data while at the same time providing more comfort to the patients by eliminating breast compression. Flat panels are charge integrating detector

Reduction of patient doses from barium meal and barium enema examinations through changes in equipment factors.

A study of patient doses for barium meal and barium enema examinations has been made for X-ray departments throughout Grampian Region. Dose-area products are substantially less than UK reference levels. Contributions from fluoroscopy and radiography varied significantly between different units, with fluoroscopy making up 35-92% of the mean doses for barium meals and 24-57% for barium enemas. Equipment related factors had a greater influence on patient doses than radiologists' techniques. The gain of the image intensifier and the exposure factors selected by the automatic exposure control (AEC)

Retrospective analysis of a detector fault for a full field digital mammography system.

This paper describes objective and subjective image quality measurements acquired as part of a routine quality assurance (QA) programme for an amorphous selenium (a-Se) full field digital mammography (FFDM) system between August-04 and February-05. During this period, the FFDM detector developed a fault and was replaced. A retrospective analysis of objective image quality parameters (modulation transfer function (MTF), normalized noise power spectrum (NNPS) and detective quantum efficiency (DQE)) is presented to try and gain a deeper understanding of the detector problem that occurred. These m

Detection of substantial numbers of latent tuberculosis and positive hepatitis B serology results in rheumatology patients preparing to receive intensified immunosuppressive therapy in a low-prevalence country: why screening still matters.

INTRODUCTION /OBJECTIVES: International guidelines recommend screening for latent tuberculosis infection (LTBI) and chronic viral hepatitis infections before initiating intensified immunosuppressive therapy. We assessed the prevalence of positive screening tests for LTBI, hepatitis B virus (HBV), and hepatitis C virus (HCV) in patients screened at a large rheumatology outpatient center in Germany. METHOD: This retrospective cohort study used electronic health records from adult rheumatology patients. The presence of LTBI was evaluated by chest X-rays, patient medical history/self-report, and
