Contemporary Management of Acute Decompensated Heart Failure and Cardiogenic Shock.

Cardiogenic shock (CS) is a life-threatening condition characterized by end-organ hypoperfusion and hypoxia primarily due to cardiac dysfunction and low cardiac output. Unfortunately, the mortality and morbidity associated with CS have remained high despite notable advances in heart failure management. Treatment should be carefully guided by hemodynamics assessment. Although inotropes, vasopressors, mechanical circulatory support, and catheter intervention for critical valve lesion are not always recommended, they are helpful in selected patients. Early diagnosis, accurate hemodynamic assessme

Cardiac Arrest in the Operating Room: Part 2-Special Situations in the Perioperative Period.

As noted in part 1 of this series, periprocedural cardiac arrest (PPCA) can differ greatly in etiology and treatment from what is described by the American Heart Association advanced cardiac life support algorithms, which were largely developed for use in out-of-hospital cardiac arrest and in-hospital cardiac arrest outside of the perioperative space. Specifically, there are several life-threatening causes of PPCA of which the management should be within the skill set of all anesthesiologists. However, previous research has demonstrated that continued review and training in the management of t

Extracorporeal life support - state of the art.

Extracorporeal life support (ECLS) has become an accepted therapeutic measure in the treatment of infants, children and adults with reversible respiratory or cardiac failure. The principle behind ECLS involves obtaining access to drain blood from the venous circulation into the extracorporeal circuit where it is oxygenated and cleansed of carbon dioxide before being returned to the circulation. The UK Collaborative ECMO Trial showed that an ECLS policy was clinically effective in terms of improved survival without a rise in severe disability at age 1 year. Long-term follow-up has confirmed the

Case Report: Dynamic J-point elevation as a novel precursor to torsade de pointes: electrocardiographic markers for proactive management.

This case report presents a 41-year-old male who experienced out-of-hospital cardiac arrest (OHCA) and was successfully resuscitated through prehospital emergency medical services and in-hospital advanced cardiovascular life support. Continuous electrocardiographic (ECG) monitoring revealed a unique pattern of J-point and ST-segment elevation in lead II, which preceded the development of torsade de pointes (TdP). The complete ECG evolution, from initial J-point elevation to the onset of polymorphic ventricular tachycardia, was meticulously documented. This rare case provides valuable insights

The Alpha-bent and Tunneling: A Novel Technique for Fixing the Transvenous Pacer Lead during Temporary Transvenous Pacing in the Emergency Department.

The last step in the management of symptomatic bradycardia according to the advanced cardiac life support algorithm is temporary transvenous pacemaker insertion (TPI). TPI done by an emergency physician in the emergency department (ED) is on the rise particularly in South India owing to the increased incidence of yellow oleander poisoning. As in ED, we use passive fixation leads, fixation of a transvenous pacer lead is very important. In the following case series, we describe two novel techniques namely, "the alpha-bent" and "tunneling" for fixing the transvenous pacer lead. This technique of

Analysis of cerebral Interleukin-6 and tumor necrosis factor alpha patterns following different ventilation strategies during cardiac arrest in pigs.

Hypoxia-induced neuroinflammation after cardiac arrest has been shown to be mitigated by different ventilation methods. In this prospective randomized animal trial, 35 landrace pigs were randomly divided into four groups: intermittent positive pressure ventilation (IPPV), synchronized ventilation 20 mbar (SV 20 mbar), chest compression synchronized ventilation 40 mbar (CCSV 40 mbar) and a control group (Sham). After inducing ventricular fibrillation, basic life support (BLS) and advanced life support (ALS) were performed, followed by post-resuscitation monitoring. After 6 hours, the animals we

Massive Pulmonary Embolism Presenting as Recurrent Cardiac Arrest: Successful Resuscitation and Thrombolysis in a 74-Year-Old Male.

Massive pulmonary embolism (PE) is a life-threatening emergency often presenting with hemodynamic collapse or cardiac arrest. Despite advances in resuscitation and reperfusion therapies, outcomes remain poor. We report the case of a 74-year-old man with a history of hypertension, hyperlipidemia, and diabetes mellitus who presented with acute severe dyspnea leading to pulseless electrical activity (PEA) cardiac arrest. Return of spontaneous circulation was achieved with advanced cardiac life support, but he subsequently experienced recurrent PEA arrest requiring intubation, vasopressors, and an

Update on COVID-19 Myocarditis.

Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) gained worldwide attention at the end of 2019 when it was identified to cause severe respiratory distress syndrome. While it primarily affects the respiratory system, we now have evidence that it affects multiple organ systems in the human body. Cardiac manifestations may include myocarditis, life threatening arrhythmias, acute coronary syndrome, systolic heart failure, and cardiogenic shock. Myocarditis is increasingly recognized as a complication of Coronavirus-19 (COVID-19) and may result from direct viral injury or from exaggerat

Focused Echocardiography in Life Support: The Subcostal Window : What the Surgeon Should Know for Critical Care Applications.

CONTEXT: Focused echocardiography evaluation in life support (FEEL) for emergency and critical caremedicine is an innovative approach to introducing limited-in-scope echocardiography in a timely fashion into periresuscitation care. FEEL is an advanced life support-conformed concept and a simple procedure that can be readily used in shock roomor pre-hospital scenarios as an extension of focused abdominal sonography for trauma (FAST). The subcostal window plays a pivotal role in this context, because it can easilybe applied inthesupine position, and is usually better than other windows in patien

Pulseless electrical activity following traumatic cardiac arrest: Sign of life or death?

BACKGROUND: Generally considered a sign of life, PEA is the most common arrhythmia encountered following pre-hospital traumatic cardiac arrest. Some recommend cardiac ultrasound (CUS) to determine cardiac wall motion (CWM) prior to terminating resuscitation efforts. This purpose of this study was to evaluate the outcomes of patients with traumatic cardiac arrest presenting with PEA, with and without CWM. METHODS: Trauma patients who underwent pre-hospital CPR were identified from the registries of two level-1 trauma centers. Pre-hospital management by emergency medical transport services was

VenoArterial Extracorporeal Membrane Oxygenation in Cardiac Arrest Suspected due to Massive Pulmonary Embolism: A Case Report.

INTRODUCTION: High-risk pulmonary embolism (PE) is an uncommon but potentially reversible cause of out-of-hospital cardiac arrest, frequently presenting as pulseless electrical activity (PEA). Early identification and multidisciplinary intervention are critical, yet confirmatory imaging is often delayed. Current guidelines recommend consideration of venoarterial extracorporeal membrane oxygenation (VA-ECMO) support in specialized centers when high-risk PE is suspected and the patient presents with cardiogenic shock or cardiac arrest. CASE REPORT: We describe a 50-year-old woman with a remote

Imaging insights in veno-venous and veno-arterial extracorporeal membrane oxygenation (ECMO): CT protocols, underlying pathophysiology, and main complications.

Extracorporeal membrane oxygenation (ECMO) is a vital life support technique employed in patients experiencing pulmonary or cardiopulmonary failure. This procedure entails the use of a pump to replace heart function and an oxygenator to ensure adequate blood oxygenation. ECMO systems are categorized into two main configurations: veno-venous (VV) and veno-arterial (VA) circuits. VV-ECMO is employed for isolated respiratory failure, while VA-ECMO provides temporary mechanical circulatory support for patients with cardiogenic shock or cardiac arrest. A less common alternative, veno-arterial-venou

Is ward experience in resuscitation effort related to the prognosis of unexpected cardiac arrest?

BACKGROUND: The aim of this study was to investigate the outcome of patients of unexpected cardiac arrest initially resuscitated by first responders with dissimilar experiences under the support of cardiac arrest team (CAT). METHODS: All unexpected cardiac arrest patients receiving in-hospital resuscitation with the activation of CAT in a tertiary-care teaching hospital over a 12-month period were recorded according to the Utstein criteria. We prospectively recorded various factors at resuscitation and retrospectively evaluated the outcome. Outcome measures included return of spontaneous circ

Risk factors and predisposing conditions for amniotic fluid embolism: a comprehensive review.

Amniotic fluid embolism (AFE) is a rare, yet life-threatening obstetric emergency characterised by sudden collapse of the mother due to circulatory and respiratory failure, often accompanied by coagulopathy. It accounts for a significant proportion of peripartum cardiac arrests and maternal deaths, with an incidence of 2-8 per 100,000 deliveries. The specific pathophysiology behind AFE remains unclear. However, one hypothesis states that amniotic fluid or fetal debris enters the maternal circulation, triggering a severe inflammatory and immunologic response. Diagnosis of AFE is primarily clini

Successful management of neonatal myocardial infarction with ECMO and intracoronary r-tPA lysis.

Acute myocardial infarction (MI) is a life-threatening condition rarely encountered in neonates. The patients usually present with sudden cardiogenic shock. Clinical management in neonates is extremely challenging. If treatment is delayed, the prognosis is dismal. We report on a 4-day-old full-term male newborn presenting with acute MI and cardiogenic shock secondary to proximal thromboembolic occlusion of the left descending coronary artery. Hemodynamic stabilization could only be achieved after extracorporeal membrane oxygenation (ECMO) support. Coronary artery patency restoration was perfor
