Preliminary study on the effects of bar placement on the thorax after the nuss procedure for pectus excavatum using bone scintigraphy.

AIM: Bone scintigraphy was performed to elucidate the effects of the Nuss procedure for pectus excavatum on the bony thorax. METHODS: Eight boys and 6 girls (5 - 24 years of age) underwent bone scintigraphy, using (99m)Tc-HMDP. Eleven patients were studied 5 to 21 days after the Nuss procedure; 6 were studied 20 to 24 months after the operation before bar removal. Three of 14 were studied twice after the Nuss procedure and before bar removal. RESULTS: In the early postoperative phase, RI accumulation was found at the sternum and ribs in only 1 of 6 patients under 9 years of age, whereas in a

[Ossovenography as a diagnostic criterion of post-traumatic femoral head necrosis in internal fixation devices].

Despite advances in the field of orthopedic surgery, necrosis of the femoral head still remains a serious problem. Under normal conditions magnetic resonance tomography (MRT) is the gold standard for early diagnosis. Because of magnetic interference, this technique is not applicable in the diagnosis of posttraumatic necrosis with internal fixation, however. This seems to be an ideal indication for ossovenography: contrast medium is injected into the femoral head under X-ray control and the venous flow is documented. Furthermore, it is possible to get histological samples from suspicious areas.

Late reconstruction of severe open-book deformities of the pelvis - tips and tricks.

BACKGROUND: The primary goal of treatment for open book pelvic injuries after high-energy trauma is to control haemorrhage and to close the pelvic ring anatomically. Less commonly, patients may present late with malunion or non-union. METHODS: We reviewed five operatively treated patients with delayed severe open book deformities who had a diastasis of more than 6 cm. We describe the pre-operative workup, surgical strategy and challenges experienced. They specifically include: extensive scar and contracture formation, malunion, urogenital prolapse and difficult reduction of vertical shear ele

Open Reduction Techniques for Supracondylar Humerus Fractures in Children.

Supracondylar humerus fractures are the most common elbow fractures in children. Displaced supracondylar humerus fractures that are associated with neurologic and/or vascular injuries should be treated with timely reduction via closed techniques. If closed reduction fails, reduction via open techniques is indicated. There is controversy about which surgical approach yields the best cosmetic and functional outcomes while minimizing postoperative complications. Open reduction, if indicated, has been reported to yield good outcomes in patients in whom closed reduction fails.

Distal radius fractures in athletes: approaches and treatment considerations.

Fractures of the distal radius are common injuries in both athletes and nonathletes. Management is dictated by the nature of the fracture and the patient's level of competition, age, and sport-specific demands. Treatment strategies range from nonoperative treatment for stable injuries to primarily operative treatment for unstable fractures, particularly in active athletes. Once the decision has been made to treat a distal radius fracture operatively, a wide variety of fixation options are available. However, no technique has proven superior to all others, and no single method of fixation will

Hardware complications related to the surgical fixation of slipped capital femoral epiphyses.

Slipped upper femoral epiphysis is a disabling condition with an annual incidence of 2-13 per 100,000. In situ surgical fixation is the preferred initial treatment for both stable and unstable slipped capital femoral epiphysis (SCFE) cases. The main aim is to avoid further slippage and complications such as osteonecrosis and chondrolysis. The choice of medical implants in managing this condition has changed along the years from large nail-like devices to cannulated screws. The biomechanical properties of different fixation techniques have been studied. All implants have been associated with co

Fractures of the proximal phalanx and metacarpals in the hand: preferred methods of stabilization.

Treatment of fractures of the proximal phalanx and metacarpals is based on the presentation of the fracture, degree of displacement, and difficulty in maintaining fracture reduction. A wide array of treatment options exists for the variation in fracture patterns observed. Inherently stable fractures do not require surgical treatment; all other fractures should be considered for additional stabilization. In general, of the many combinations of internal fixation possible, Kirschner wires and screw-and-plate fixation predominate. Early closed reduction typically is successful for unicondylar frac

Clinical and radiological findings after different treatment of odontoid fractures type Anderson II and III.

PURPOSE OF THE STUDY: In the period from 06/00 to 08/02, 31 patients with odontoid fractures type Anderson II and III were treated and stastically recorded. 25 patients were followed up; the progess of 24, documented in detail radiographically, were evaluated independently by a traumatologist and by a radiologist. The usual time of immobilization when treating odontoid fractures type Anderson type II and III with the halo-fixator is 12 weeks. For this 12 weeks that it is worn, objective assessment of bone healing is performed radiographically and the results critically considered in terms of t

Conversion of failed proximal femoral internal fixation to total hip arthroplasty in osteopetrotic bone.

Osteopetrosis is a rare disease. When fractures are encountered, fixation is extremely difficult. A 38-year-old man with osteopetrosis who sustained a displaced Pauwels type IV fracture with 3 failed internal fixations required total hip arthroplasty. Two main problems were perceived during preoperative planning: the removal of failed and broken metalwork and reaming of the tight intramedullary canal to allow seating of the femoral stem. We present technical solutions to aid the surgeon in the management of this difficult type of case using custom-made tungsten carbide instrumentation.

Spinal instrumentation in the management of degenerative disorders of the lumbar spine.

The use of spinal instrumentation as an adjunct to fusion for the treatment of degenerative disorders of the lumbar spine is controversial. Instrumented lumbar fusions, in specific instances, may improve patient outcomes. For patients undergoing single level primary lumbar arthrodesis, the available data do not conclusively support the efficacy of spinal instrumentation. However, in the setting of previous failed lumbar surgery, iatrogenic or degenerative lumbar spondylolisthesis, spinal instrumentation may be useful as an adjunct to fusion. Possible advantages associated with the use of instr

Open reduction and interfragment wire fixation of comminuted nasal fractures.

Manual reduction is the first choice for all types of acute nasal fractures; open reduction is the choice for fractures that do not respond to manual reduction or complicated fractures at the base of the nasal pyramid. From 1968 to 1986, 564 cases of nasal fractures were treated at the Department of Plastic and Reconstructive Surgery, Jikei University School of Medicine, Tokyo. Of these, 241 were acute nasal fractures, 52 of which were associated with facial bone fractures. Deviated types of fractures that cannot be correctly repositioned by manual reduction and some depressed fractures often

Complications of external skeletal fixation.

External skeletal fixators (ESF) have a reputation for causing problems. Effective solutions exist for some of these problems. Delayed union can be avoided by accurate reduction, early bone grafting, and early removal of the ESF. Nerve and vessel injury can be avoided by special attention to the anatomy of the lateral arm and forearm, the upper medial thigh, and the junction of the third and fourth quarters of the leg. Pin tract infections continue to haunt ESF systems but are minimized by special attention to necrosis of tissue at the pin-skin and pin-bone interfaces. Undue soft tissue or bon

Le Fort III advancement with gradual distraction using internal devices.

The purpose of this report is to demonstrate a practical clinical method for advancement of the midface using a combination of Le Fort III osteotomy and gradual distraction. Use of internal, submergible distraction devices and modification of the Ilizarov distraction protocol are presented. Case reports show the effect of departing from the Ilizarov protocol by eliminating the latency period, initiating distraction intraoperatively, and rapidly distracting the segments postoperatively. A method of controlling fragment distraction rate by regulating pressures within the osteotomy site is descri

Cervical corpectomy and strut grafting.

Cervical corpectomy and strut grafting is a deceptively simple procedure that has been performed for many years for a variety of cervical spine disorders (infection, neoplastic disease, and trauma) but most commonly for cervical spondylosis. The procedure requires attention to detail to ensure adequate decompression of the neural structures and avoiding injury to the soft tissues of the neck and the vertebral artery in the transverse foramina. The following description of the technique is one we have successfully used for cervical corpectomy and strut grafting. We also discuss patient selectio

Lumbar arthrodesis for degenerative conditions.

There is significant disagreement among spine surgeons regarding the optimal technique of arthrodesis for treatment of degenerative disorders of the lumbar spine. Degenerative conditions of the lumbar spine include degenerative disk "disease," post-decompression degeneration, degenerative spondylolisthesis, junctional degeneration, spondylolis, and low-grade lytic spondylolisthesis. Although it is impossible to develop strict evidence-based criteria for the selection of one surgical approach over another, some generalizations are possible based on empiric process, anecdotal experience, and pub
