Conventional poly(methyl methacrylate) (PMMA) intraocular lenses (IOLs) and heparin-surface-modified (HSM) IOLs were implanted after extracapsular cataract extraction in 40 human eyes with exfoliation syndrome. The study was double-masked and randomized. Two years after surgery 17 cases with HSM IOLs and 15 with PMMA IOLs were examined. We observed that pigment and cell deposits were more frequent on the PMMA than on the HSM IOLs. The incidence of posterior synechia formation was similar in both groups. However, the incidence of neodymium:YAG laser posterior capsulotomy and posterior capsule o

Major problems in pediatric cataract patients include noncompliance with contact lenses resulting in amblyopia. Advances in intraocular cataract surgery have provided a better environment to perform intraocular lens (IOL) implantation in children. We prospectively analyzed the results of 34 consecutive pediatric patients who underwent cataract removal and insertion of an IOL. Operative technique performed was posterior scleral beveled or frown incision and IOL endocapsular fixation. Subgroups included 10 eyes with traumatic cataract, and 24 eyes with developmental cataracts. Six patients had b

Late capsular bag distension syndrome occurs most of the time with the accumulation of opaque milky fluid behind the IOL, without refractive change, like in one of our cases. The other case that we will describe had an accumulation of a clear fluid behind the IOL with an induced hyperopia. To the best of our knowledge, this is the first published report of capsular bag distension syndrome with a shift towards hyperopia. In both case the visual acuity and the initial refraction was restored after Nd:YAG posterior capsulotomy.

We summarize the findings of five randomized prospective, controlled studies that evaluated the following factors associated with the development of cystoid macular edema (CME) following cataract surgery: ultraviolet (UV) light from the operating microscope; postoperative UV light exposure; primary capsulotomy; retrobulbar hyaluronidase; and prostaglandin synthesis in the eye. The following were the major findings: (1) The presence of a UV-light filter on the operating microscope during cataract surgery made no statistically significant difference in the early postoperative (two to six months)

Adhesive capsulitis of the ankle is also known as frozen ankle and results in marked fibrosis and contracture of the ankle capsule. Arthroscopic capsular release is indicated for symptomatic frozen ankle that is resistant to conservative treatment. It is contraindicated for ankle stiffness due to degenerative joint disease, intra-articular malunion, or adhesion of the extensors of the ankle. The procedure consists of endoscopic posterior ankle capsulectomy and arthroscopic anterior ankle capsulotomy. It has the advantages of being minimally invasive surgery and allowing early postoperative vig

Intraocular lens implantation in young children can yield satisfactory results when the surgical techniques are modified and adapted to the child's eye. Between June 1983 and July 1988, 84 posterior chamber lenses were implanted in 80 children aged 2 months to 8 years. An elective posterior capsulotomy followed by an anterior vitrectomy was performed in all cases prior to the lens insertion. This step allowed a permanent clear visual axis which in turn has facilitated visual rehabilitation of the pseudophakic eye. The best results occurred in the developmental cataract group followed by the tr
