BACKGROUND: Reflexivity is essential in end-of-life research due to its ethical sensitivity, emotional intensity, and relational complexity, yet it is often left to individual initiative and remains weakly institutionalized in palliative care research settings.
OBJECTIVE: To describe the design, implementation, and initial outputs of a Reflexivity Lab developed as a structured, collective reflexive method embedded within a palliative care research service.
METHODS: A Reflexivity Lab was implemented in 2023 within the palliative and supportive care service of the Lausanne University Hospital,

OBJECTIVES: To (1) systematically identify tools developed to assess and address the support needs of informal carers in palliative or end-of-life care and (2) examine the extent to which socially stratifying sample characteristics have been reported following tool use.
DESIGN: A systematic scoping review was conducted.
METHODS: Medline, CINAHL, PsycINFO, ASSIA, ProQuest Central and Web of Science databases were searched from inception to May 2024 and again in May 2025. Primary studies using any methodology reporting the development and/or use of tools to assess informal carer support needs

BACKGROUND: People with intellectual disabilities face inequities in access to end-of-life care and inequalities in its quality and delivery. This review aimed to synthesise qualitative evidence to understand their own perspectives about what contributes to optimal end-of-life care.
METHODOLOGY: Data from 93 participants in five qualitative studies were thematically synthesised to identify optimal care and inform recommendations.
RESULTS: Four overarching and interrelated analytical themes were generated. (1) Optimal care recognises heterogeneity and is person-centred. It aligns with individ

Pediatric Palliative Care provision is currently inequitable for tamariki (children) with serious illness, and their whānau in Aotearoa New Zealand. A lack of access to education for health professionals has long been identified as a barrier to implementing quality delivery of pediatric palliative care. The Education in Palliative and End of Life Care for Pediatrics (EPEC-Ped) curriculum was developed in 2012 and has been internationally recognized as a high-quality curriculum with demonstrated results in changing practice. This paper explores how this curriculum has been adapted to meet the n

BACKGROUND: This scoping review identifies what is known about patient/partner sexuality and intimacy needs during end-of-life care and how healthcare professionals' (HCPs) approach these needs.
METHODS: Following PRISMA-ScR, articles available in English published between 2010-2024 were screened.
RESULTS: A total of 18 articles were included in the review. Four themes were identified: 1) negative impact of serious illness on sexuality and intimacy, 2) barriers and facilitators to addressing sexuality, 3) tension between expansive and genital-focused approaches to sexuality and intimacy, and

Patients with life-limiting illnesses often experience profound emotional and spiritual challenges that conventional palliative care treatments may not fully alleviate. Psychedelics, with their potential to facilitate deep psychological insight, offer a promising approach to enhancing end-of-life care. Despite growing interest and emerging psychiatric applications, their use in palliative settings remains poorly understood, with existing studies limited by small sample sizes and low methodological quality. This article outlines a research agenda that prioritises adequately powered randomised c

LGBTIQ+ individuals (lesbian, gay, bisexual, transgender, intersex, queer, +) frequently encounter issues specific to them in their healthcare journey, including various forms of discrimination. However, healthcare professionals often lack knowledge or awareness of these issues. The palliative phase of an illness is naturally a period of increased vulnerability. A better understanding of these issues is necessary in order to provide individualized support to LGBTIQ+ patients in palliative care and their loved ones. This article discusses ways to improve this support.

BACKGROUND: Delirium is a common condition at the end of life and causes significant distress in patients and their loved ones. A precipitant factor can be found in less than half of the patients and the management interventions are limited.
CASE DESCRIPTION: A patient in his late sixties with low English proficiency with a metastatic neuroendocrine tumor was transferred to a palliative care unit on non-invasive bilevel ventilation. He appeared to become delirious and agitated, trying to remove the face mask, wriggling in bed, and tapping the bedrails. Haloperidol and lorazepam were required

Patients with a life-threatening disease (LTD) sometimes suffer from end-of-life distress (EOLD) which refers to the physical, psychological, emotional, and spiritual suffering related to chronic illness and the possibility of death. Palliative care interventions seek to improve the quality of life of patients with EOLD and their significant others. Currently, a range of psychological and pharmacological palliative care interventions may be used to mitigate the various symptoms related to EOLD. However, the evidence for their efficacy is inconclusive with only short- to moderate effects. Anoth

BACKGROUND AND OBJECTIVE: The indication "existential suffering (ES)" for palliative sedation therapy is included in most frameworks for palliative sedation and has been controversially discussed for decades. The appellative character of ES demands rapid relief and sedation often appears to be the best or only solution. ES is still poorly understood and so often neglected by health care professionals due to a lack of consensus regarding assessment, definition and treatment in the international medical literature. Based on a selective review of the literature on ES we propose a different view o

BACKGROUND: The utilisation of palliative sedation is often favoured by patients approaching end of life due to the presence of multiple difficult-to-manage symptoms during the terminal stage. This study aimed to identify the determinants of the use of palliative sedation.
METHODS: To identify pertinent observational studies, a comprehensive search was performed in PubMed, Embase, Cochrane Library, and PsycINFO databases from their inception until March 2022. The methodological quality of the chosen prospective and retrospective cohort studies was assessed using the Newcastle Ottawa Scale, wh

As prison-based hospice programs are slowly implemented across the United States, scarce attention has been devoted to the views of correctional staff who provide essential end-of-life care to dying incarcerated individuals. These professionals must maneuver their diverse responsibilities and emotional perspectives to deliver compassionate care to a marginalized population. A textual analysis of narratives of correctional staff participating in the hospice program at Louisiana State Penitentiary was incorporated to explore the transformative experiences resulting from staff members' collaborat

BACKGROUND: The number of older people living alone is constantly increasing. This group faces special challenges regarding remaining at home when their care needs increase, especially in later life. When no family is available, friends and neighbors, so called non-kin carers, are important sources of support.
OBJECTIVE: A literature review was conducted to evaluate existing research on the relevance of non-kin support for older people living alone, from initial limitations to the end of their lives, particularly when they wished to remain at home.
METHOD: The literature search followed the

BACKGROUND: There is an ongoing need for interventions to improve quality of end-of-life care for patients in inpatient settings.
OBJECTIVE: To compare two methods for implementing a Comfort Care Education Intervention for Palliative Care Consultation Teams (PCCT) in Veterans Affairs Medical Centers (VAMCs).
DESIGN: Cluster randomized implementation trial conducted March 2015-April 2019. PCCTs were assigned to a traditional implementation approach using a teleconference or to an in-person, train-the-champion workshop to prepare PCCTs to be clinical champions at their home sites.

BACKGROUND: Socioeconomic inequalities in access to, and utilization of medical care have been shown in many jurisdictions. However, the extent to which they exist at end-of-life (EOL) remains unclear.
METHODS: Studies in MEDLINE, EMBASE, CINAHL, ProQuest, Web of Science, Web of Knowledge, and OpenGrey databases were searched through December 2019 with hand-searching of in-text citations. No publication date or language limitations were set. Studies assessing SES (e.g. income) in adults, correlated to EOL costs in last year(s) or month(s) of life were selected. Most people have some idea of how they want to live their life; however, an estimated two-thirds of Americans have not completed their advance directives. This becomes an issue when up to 90% of patients develop delirium during their final days of life, at which point we depend on advance directives or surrogate decision-makers. Here, we present a case of terminal delirium in a patient with advanced cancer and a history of alprazolam abuse who had not discussed his end-of-life wishes with the medical team or with his estranged family. Treatment was provided to address reversible causes of deli

The purpose of this study was to determine whether Severance Hospice Home Care Program was able to meet its objectives. This was done in order to show in detail the effects of hospice home care on the quality of life of terminally ill patients and to provide rationale for setting up more hospice home care programs in korea. The results of the study were as follows: The subjects of the study were 100 terminally ill patients who had died while in the hospice program and 64 family members who were registered with Severance Hospice Home Care Program between march 1988 and Feb. 1990.
