Vacuum-assisted close versus conventional treatment for postlaparotomy wound dehiscence.

PURPOSE: The conventional treatment for postlaparotomy wound dehiscence usually involves surgical revision. Recently, vacuum-assisted closure has been successfully used in postlaparotomy wound dehiscence. The aim of the present study was to compare the clinical outcome of 207 patients undergoing vacuum-assisted closure therapy or conventional treatment for postlaparotomy wound dehiscence. METHODS: TWO HUNDRED AND SEVEN CONSECUTIVE PATIENTS UNDERWENT TREATMENT FOR POSTLAPAROTOMY WOUND DEHISCENCE: vacuum-assisted closure therapy (January 2007 through August 2012, n = 25) or conventional treatme

Vacuum-venipuncture skills: time required and importance of tube order.

BACKGROUND: The purpose of this study was to assess specific vacuum-venipuncture skills and the influence of the time involved in skin puncture and blood collection. METHODS: Thirty subjects undergoing venipuncture in which video analysis was possible were included. These procedures were carried out by four nurses and recorded with a digital camera. Venipuncture skills classified by our observations were delineated on the basis of frame-by-frame video images, and a graph of x and y coordinates was created. RESULTS: With the first blood-collection tube, strong blood flow required the practiti

[Failure of vacuum extractions: risk factors, maternal and fetal issues].

OBJECTIVES: Determine cases which are at risk of vacuum extraction failure as well as maternal and foetal issues depending on the delivery outcome. MATERIAL AND METHODS: It was a retrospective study comparing 147 vacuum failures, from January 2002 to December 2010, with a control group randomly composed of 526 successful vacuum extractions. The outcomes were high risk situations of vacuum failure, maternal and neonatal morbidity depending on the delivery method (caesarean section or other instrumental extraction). RESULTS: The global vacuum failure rate was 3.3 %. During labour, we identifie

[Neonatal subgaleal haemorrhage causing fatal course after vacuum-assisted extraction].

We present a case that emphasizes the importance of early recognition of neonatal subgaleal haemorrhage through knowledge of risk factors and adequate communication within the health team treating the woman in labour. Especially vacuum-assisted extraction of the child significantly multiplies the incidents of subgaleal haemorrhage. The high mortality is directly associated with disseminated intravascular coagulation and bleeding. Early monitoring and treatment with replacement of blood volume is crucial. Education concerning this condition is therefore of great importance.

Fetal injury associated with routine vacuum use during cesarean delivery.

The use of a vacuum device as a routine procedure at the time of repeat cesarean delivery was associated with major fetal intracranial hemorrhage. In the absence of clear evidence of benefit, the routine use of vacuum extraction at the time of cesarean delivery is not justified, given its potential for serious fetal injury.

Fasciotomy closure using simultaneous vacuum-assisted closure and hyperbaric oxygen.

Fasciotomies performed for compartment syndrome and ischemic vascular disease often requires closure in 2 to 4 weeks by skin graft. This leaves the patient with an unsightly scar and a limb with reduced strength. The use of vacuum-assisted closure (VAC) and hyperbaric oxygen therapy (HBOT) quickly reduce the edema and permit earlier closure with adjacent skin. A study of three trauma patients with compartment syndrome, fasciotomies, and the use of the VAC and HBOT to close the fasciotomy wounds with adjacent skin is presented. The pathophysiology of compartment syndrome and ischemia-reperfusio

Underestimation of DCIS at MRI-guided vacuum-assisted breast biopsy.

OBJECTIVE: The study objective was to assess the rate of underestimation of ductal carcinoma in situ (DCIS) at MRI-guided 9-gauge vacuum-assisted breast biopsy. MATERIALS AND METHODS: An institutional review board-approved retrospective review was performed of 373 consecutive lesions that had undergone MRI vacuum-assisted breast biopsy. In 34 lesions with subsequent surgery, vacuum-assisted breast biopsy yielded DCIS without frank microinvasion or invasion. DCIS underestimates were lesions for which vacuum-assisted breast biopsy yielded DCIS without frank microinvasion or invasion at biopsy a

Mercury vapor levels in exhaust air from dental vacuum systems.

OBJECTIVE: This study was undertaken to determine mercury (Hg) vapor levels in the air exhausted from dental vacuum systems. METHODOLOGY: Hg vapor concentrations from the dental vacuum system exhaust ports of three dental clinics were measured utilizing the Jerome 431-X mercury vapor analyzer and the United States Occupational Safety and Health Administration's (OSHA) method ID-140 in units of ng Hg/m3. Air velocity measurements and temperatures were determined with a constant temperature thermal anemometer. Hg emissions per unit time were then calculated in ng Hg/min. Ambient Hg concentratio

Scrotal and penile reconstruction using the vacuum-assisted closure device.

Fournier's gangrene is a necrotizing, soft tissue infection that spreads along the deep external fascial planes of the perineum, scrotum (or vagina), penis, thighs and lower abdomen. Due to the rapid progression of the disease, patients are frequently left with a large integumentary deficit. Treatment of the skin defect has been described in a multitude of ways including split-thickness skin grafting, burying the testes in the thighs, thigh flaps and a variety of myocutaneous and fasciocutaneous flaps. Many of the previously described techniques have worked well for smaller defects and for clo

[Vacuum pack technique--a good method for temporal abdominal closure].

UNLABELLED: Abdominal compartment syndrome(ACS) is a clinical entity characterised by increased intraabdominal pressure leading to multiple organ failure, fatal if left untreated. The treatment of abdominal compartment syndrome is surgical decompression with a temporary abdominal wall substitute. To avoid the development of abdominal compartment syndrome, temporary abdominal closure (TAC) should be considered after celiotomy for trauma. A new method for TAC was introduced at Ullevaal University Hospital in 2002, the "vac pac". METHOD: The "vac pac" technique is described and the patients trea

[Vacuum sealing of extensive wound healing disorders after kidney transplantation].

With the introduction of mycophenolate mofetil (MMF) in renal transplantation, acute rejection episodes diminished and short-term graft survival improved. Better graft outcome, however, is followed by several surgical complications attributed to MMF. Patients with risk factors (adiposity, diabetes mellitus, advanced age) show an increased rate of healing by second intention. We treated two patients with the vacuum sealing technique so that after 15 days a secondary suture became possible in each case. To date the vacuum sealing technique has been used mainly in traumatology, abdominal surgery,

Vacuum application through a nonfully dilated cervix: a viable option.

The aim of this cross-sectional study was to assess the feasibility of vacuum delivery through a nonfully dilated cervix. The study group consisted of 39 women with vacuum deliveries through a nonfully dilated cervix larger than 9 cm and station of the head at S or more +2 cm. These were compared to a control group of 215 vacuum deliveries at a fully dilated cervix and 46 women who underwent cesarean section at a nonfully dilated cervix larger than 9 cm. The main indication for vacuum extraction in the study group was fetal distress and in the control groups prolonged 2nd stage, dysfunctional

MR-Guided vacuum biopsy of 206 contrast-enhancing breast lesions.

PURPOSE: To determine the accuracy and clinical use of MR-guided vacuum biopsy (VB) of enhancing breast lesions. MATERIAL AND METHODS: 254 lesions were referred to MR-guided vacuum-assisted breast biopsy. In 43 (16 %) patients the indication was dropped because the lesions could not be identified at the time VB was scheduled. This was due to hormonal influences (n = 37), to too strong compression (n = 3) or to misinterpretation of the initial diagnostic MRI (n = 3). In 5 cases (2 %) VB was not performed due to obesity (n = 2); problems of access (n = 2) or a defect of the MR-unit (n = 1). VB

Venoarterial air embolus: a complication of vacuum-assisted venous drainage.

Minimal access techniques with cardiopulmonary bypass use smaller cannula systems for management of cardiopulmonary bypass. To augment flow rates through the smaller cannula, the technique of vacuum-assisted venous drainage has been used. We describe a complication of vacuum-assisted venous drainage by inadvertent positive pressurization of the venous circuit resulting in a paradoxic air embolus across a patent atrial septal defect. Hazards of the current cardiopulmonary bypass systems and techniques for avoiding this potential complication are discussed.

[Vacuum assisted wound closure after dermatofasciotomy of the lower extremity].

Between 1 January 1992 and 15 March 1994 25 patients with compartment syndromes of the lower limb were treated by the vacuum-sealing technique (VS). Ten out of 25 patients had multiple injuries. Eight compartment syndromes were located in the thigh, 14 in the lower leg and 3 in the foot. The average time of VS was 12.7 (4-31) days with 2.1 (1-8) changes per patient. The wounds were either closed by secondary suturing (n = 20) or by skin grafts after partial closure of the wounds by suturing (n = 5). One patient developed a superficial wound necrosis, which healed spontaneously without invasive
