Surgical management of spontaneous intrabiliary rupture of hydatid liver cysts.

PURPOSE: The most common complication of hydatid liver cysts is spontaneous rupture into the biliary tract. This study was conducted to evaluate the surgical management of spontaneous intrabiliary rupture of a hydatid liver cyst in 41 patients. METHODS: The preoperative diagnosis was confirmed by ultrasound in all 41 patients, 37 of whom were jaundiced. RESULTS: According to Gharbi's classification, 39% of the cysts were type III and they ranged from 3 to 18 cm in diameter, with a mean diameter of 9 cm. The mean total bilirubin and alkaline phosphatase values were 6.3 mg/dl and 450 IU, respe

A urethral stent for the treatment of detrusor-striated sphincter dyssynergia.

OBJECTIVE: To assess the technique, efficacy and complications of the Ultraflex urethral stent (Boston Scientific Corp., Boston, MA) for the treatment of detrusor-striated sphincter dyssynergia (DSD). PATIENTS AND METHODS: Forty consecutive patients with DSD who had a Ultraflex stent placed in the membranous urethra were evaluated prospectively. DSD was caused by spinal cord injury in 30, multiple sclerosis in six and other neurological diseases in four. All patients were either tetraplegic or paraplegic and unable to use intermittent self-catheterization. Previous bladder management consiste

Cystic duct leaks in laparoscopic cholecystectomy.

BACKGROUND: Cystic duct leak (CDL) appears to complicate laparoscopic cholecystectomy (LC) more often than open cholecystectomy (OC). No mention of CDL was found in a literature review that covered 48,822 OCs and their complications. PATIENTS AND METHODS: Fifty-four patients who developed biliary tract injuries following LC were reviewed for: the time from LC to presentation, presenting symptoms, method of diagnosis, treatment, outcome, and follow-up. RESULTS: Seventeen of 54 biliary tract complications (31%) were CDLs. The CDLs presented at a median of 4 days after LC with pain (76%) and na

Spectrum and management of major complications of laparoscopic cholecystectomy.

Laparoscopic cholecystectomy has become the most prevalent method of treating uncomplicated, symptomatic cholelithiasis in the United States and elsewhere. As experience with this procedure grows, certain pitfalls are becoming apparent. Since October 1990, we have treated 22 patients for major injuries incurred during laparoscopic cholecystectomy, including 15 women and 7 men (range: 23 to 85 years). One patient had previous upper abdominal surgery; no other patient had any relative contraindication to laparoscopic surgery. The most frequent site of injury (19 patients) was the extrahepatic bi

[Routine preoperative cholangiopancreatography?].

Since the early 50s, the routine use of operative cholangiography in biliary surgery has decreased the rate of negative choledochotomy and retained stones. In the last twenty years no further progress has occurred and negative choledochotomy rate remains at 30% and for retained stones at 1.3%. Modern endoscopic technology has improved the diagnosis and treatment of biliary diseases so we think that the routine use of endoscopic retrograde cholangiopancreatography (ERCP) as a preoperative study for patients to be surgically treated of biliary disease can reduce the incidence of negative choledo

Assessment of bile flow by radioscintigraphy in patients with biliary-type pain after cholecystectomy.

Scintigraphy of the biliary system using 99mTc di-isopropyl iminodiacetic acid (DIDA) was performed in 65 subjects who had previously undergone cholecystectomy. Of the 65 subjects, 20 were free of pain and 45 had biliary-type pain both with (group I) and without (group II) features of sphincter of Oddi dysfunction. This dysfunction comprised dilatation of the bile duct, a transient rise in serum levels of liver enzymes after episodes of pain, or both abnormalities. After computer acquisition of images at intervals of 60 seconds for at least 90 minutes, time/activity curves were generated for f

[Surgical treatment of acute hemorrhagic pancreatitis. Report of 75 cases. (author's transl)].

Out of 75 definite cases of acute hemorrhagic pancreatitis, 39 were associated with a biliary lesion of which 16 were definitively the cause of the pancreatitis (11 embedded gall stones = 1/5th of the gall stones embedded in the ampulla of Vater and producing acute hemorrhagic pancreatitis). The biliary pancreatites were twice as severe as the primary pancreatites. This justifies the emergency exploration of the bile duct in any case of severe pancreatitis, suggesting acute hemorrhagic pancreatitis. The course of the disease is unforeseeable, certain large hematomas may become reabsorbed witho

Systematic review and meta-analysis of the outcome of puborectalis division in the treatment of anismus.

AIM: Anismus is a common cause of obstructed defaecation syndrome (ODS). The aim of the present review is to assess the efficacy and safety of puborectalis muscle (PRM) division in the treatment of anismus. METHOD: PubMed, Scopus, Web of Science and the Cochrane Library were searched for studies that assessed the outcome of PRM division in the treatment of anismus. The main outcome measures were subjective improvement in ODS, decrease in the Wexner constipation score and ODS score, and complications, namely faecal incontinence (FI). RESULTS: Ten studies (204 patients, 63.7% male) were includ

Protocol for the CREST Choles (Chinese REgistry Study on Treatment of Cholecysto-Choledocholithiasis) study: an ambispective, multicenter, observational, open-cohort study.

INTRODUCTION: The best approach for choledocholithiasis remains a matter of debate. Choledocholithiasis is usually treated with endoscopic sphincterotomy (EST), laparoscopic common bile duct exploration (LCBDE) or laparoscopic transcystic common bile duct exploration (LTCBDE). Data pertaining to the clinical outcomes of these approaches in the management of patients with cholecysto-choledocholithiasis in China are limited. An analysis of the economic burden associated with these treatments is lacking. The Chinese REgistry Study on the Treatment of Cholecysto-Choledocholithiasis (CREST Choles)

Minor papilla endotherapy in patients with ventral duct obstruction: identification and management.

BACKGROUND AND AIMS: Pancreatic duct (PD) cannulation via the major papilla may be compromised by downstream obstruction of the ventral PD from a stone or stricture. In patients with a patent accessory PD, cannulation of the minor papilla permits stenting or stone removal upstream of the ventral PD obstruction. Data on this technique are limited. METHODS: University of Colorado Hospital and Maine Medical Center endoscopy databases were queried for ERCPs with minor papilla cannulation. Technical success was defined as deep cannulation of the minor papilla. Minor papilla endotherapy included sp

Diagnosis of ganglionar tuberculosis by endoscopic ultrasonography.

INTRODUCTION: The endoscopic ultrasonography (EUS) is an endoscopic technique of proven clinical validity today, having a significant impact on the diagnosis and evaluation of several diseases with a low complication rate. The EUS-fine-needle aspiration (FNA) allows the evaluation of subepithelial lesions, extra-luminal lymph nodes or the gastrointestinal tract that are difficult to access by other methods with safe and high diagnostic accuracy. AIMS AND METHODS: In the case reported, the EUS-FNA was useful for the differential diagnosis with residual biliary microlithiasis and the diagnosis

TG13 flowchart for the management of acute cholangitis and cholecystitis.

We propose a management strategy for acute cholangitis and cholecystitis according to the severity assessment. For Grade I (mild) acute cholangitis, initial medical treatment including the use of antimicrobial agents may be sufficient for most cases. For non-responders to initial medical treatment, biliary drainage should be considered. For Grade II (moderate) acute cholangitis, early biliary drainage should be performed along with the administration of antibiotics. For Grade III (severe) acute cholangitis, appropriate organ support is required. After hemodynamic stabilization has been achieve

Mirizzi syndrome with endoscopic ultrasound image.

We describe a 66-year-old Caucasian man with type 1 Mirizzi syndrome diagnosed on endoscopic ultrasound. He presented with acute onset of jaundice, malaise, dark urine over 3-4 days, and was found to have obstructive jaundice on lab testing. CT scan of the abdomen showed intrahepatic biliary ductal dilation, a 1.5 cm common bile duct (CBD) above the pancreas, and possible stones in the CBD, but no masses. Endoscopic retrograde cholangiopancreatography (ERCP) by a community gastroenterologist failed to cannulate the CBD. At the University Center, type 1 Mirizzi syndrome was noted on endoscopic

[Identification of internal fistolous orifice: evolution of methylene blue technique with a mini-probe].

INTRODUCTION: Often perianal fistulas can be difficult typing and traditional anatomical classifications do not help the surgeon in accurate diagnosis of the fistula as an outpatient procedure and therefore does not allow a timely surgical currettage. The aim of our study was to introduce in the management of anal fistulas performed on an outpatient injection with methylene blue of the fistula through a small polyethylene catheter in order to detect in real time the internal opening of the fistula and drive so the next currettage surgery. Indications for endoscopic or surgical treatment of chronic pancreatitis.

In patients suffering from chronic pancreatitis, pain as the predominant symptom remains a therapeutic challenge which often cannot be tackled conservatively. Since pancreatic duct obstruction - frequently within the pancreatic head - is an important etiological factor, treatment in these cases aims at decompressing the duct either endoscopically or surgically. Endoscopic drainage includes sphincterotomy, dilation of strictures, removal of stones, and insertion of a stent; it has a success rate of 30-100%. Surgical treatment may be accomplished by drainage or resection procedures. Drainage pro

Evidence based switch to perianal block for ano-rectal surgeries.

BACKGROUND: Evidence suggests that switch from spinal/general anaesthesia (SA/GA) to perianal block (PAB) may prove advantageous for proctologic surgeries. This study evaluates the practicability of this evidence based switch. METHODS: Feasibility and efficacy of PAB for proctologic surgeries was prospectively evaluated on 100 consecutive patients over 11 months. Thirty ml of local anesthetic (0.25% bupivacaine+1% lignocaine with adrenaline) was infiltrated into the anal sphincter and perianal skin, under sedation, for achieving PAB. Time taken for onset of anesthesia; success/failure of bloc

Surgical options in the management of cystic duct avulsion during laparoscopic cholecystectomy.

BACKGROUND: Avulsion of cystic duct during laparoscopic cholecystectomy (LC) is not a common intraoperative complication, but may be encountered by any laparoscopic surgeon. Surgeons are rarely familiar with management of this condition. METHODS: Patients with gall stone related problems who were scheduled for LC at the minimal invasive surgery unit of a tertiary referral hospital during a 5 years period (April 2002-April 2007) were prospectively enrolled. RESULTS: 12 cases were identified (incidence: 1.15%). All 12 patients had gallbladder inflammation. Five patients had acute and seven pat

An endoscopic finding in patients with primary sclerosing cholangitis: retraction of the main duodenal papilla into the duodenum wall.

BACKGROUND: The diagnosis of primary sclerosing cholangitis (PSC) is reached by typical cholangiographic findings and liver biopsy. The characteristic cholangiographic abnormalities consist of multifocal strictures and dilatations in the intrahepatic and/or extrahepatic bile ducts. Patients may develop cirrhosis and portal hypertension. Endoscopy may reveal esophageal/gastric varices and portal hypertensive gastropathy. OBJECTIVE: To define a novel endoscopic finding in patients with PSC. DESIGN: Case series. SETTING: Single tertiary referral center in Turkey. PATIENTS: Ten patients with P
