Auditory brain stem evoked potentials in patients with leprosy.

Nineteen, randomly selected male patients with lepromatous leprosy were evaluated electrophysiologically. All of these patients had long-standing disease and were treated with dapsone alone. There were statistically significant differences between the values obtained in this group of leprosy patients compared to 20 age-matched controls in auditory brain stem evoked potentials (ABEP). The findings are consistent with a pathologic process located mainly between the cochlear nucleus and the lateral lemniscus in the auditory brain stem pathways. It should be emphasized that our patients had long-s

Intramedullary epidermoid cyst in the brain stem: case report.

BACKGROUND: The incidence of intracranial epidermoid cysts is between 0.2% to 1.8% of all brain tumors. They usually occur in the cerebellopontine angle or parasellar region. Intramedullary epidermoid cyst is rare and only nine cases have been previously reported. METHODS: A 69-year-old woman had progressive left hemiparesis for 2 years. Computed tomography scanning and magnetic resonance imaging (MRI) revealed that there was a cystic tumor in the pons that was compressing the brain stem. A part of the tumor cyst wall and the content of the cyst were surgically removed by right subtemporal tr

Auditory brain-stem responses (ABRs) in sleep respiratory disorders.

Neurophysiological repercussions of sleep respiratory disorders (SRDs) are still unclear. It has been shown that SRDs induce disturbance on auditory event-related potentials with a delayed latency of the cognitive P300 potentials. Since the Auditory Brainstem Responses (ABRs) explore the auditory brainstem structures and are independent on cognition, we evaluated the neurological impact of SDRs on central nervous system by ABRs in these patients. Four groups of patients were studied: snorers (S, 17 subjects), sleep apnoea syndrome (SAS, 48 subjects), apnoeic patients with respiratory insuffici

Olivary degeneration after cerebellar or brain stem haemorrhage: MRI.

Magnetic resonance (MR) images of seven patients with olivary degeneration caused by cerebellar or brain stem haemorrhages were reviewed. In four patients with cerebellar haemorrhage, old haematomas were identified as being located in the dentate nucleus; the contralateral inferior olivary nuclei were hyperintense on proton-density- and T2-weighted images. In two patients with pontine haemorrhages, the old haematomas were in the tegmentum and the ipsilateral inferior olivary nuclei, which were hyperintense. In one case of midbrain haemorrhage, the inferior olivary nuclei were hyperintense bila

Brain stem electric response audiometry in neonates with hyperbilirubinemia.

Auditory evoked responses using BERA were studied in 30 newborn babies with plasma bilirubin > or = 15 mgm/dl and repeated after treatment of neonatal jaundice with bilirubin levels of < or = 10 mgm/dl. A few jaundiced babies (16.5%) showed absent BERA response at the initial/subsequent examination. After treatment, 3/30 babies showed absent wave form responses and 2 of these were clinically kernicteric. Jaundiced babies had prolonged wave peak latencies and inter peak latencies. Treated babies showed a tendency towards recovery in their BERA responses which were however not complete. Total pl

A pontine tuberculoma mimicking an exophytic brain stem tumor.

A 21 year old man with a five month history of facial paresis, diplopia, and gait disturbance diagnosed with a brain stem tumor at another institution was evaluated. Computed tomography, magnetic resonance imaging, clinical and laboratory evaluations suggested a pontine astrocytoma, and the mass was excised. Pathological diagnosis was a tuberculoma confirmed by postoperative positive Mantoux testing. CT and MRI appearances of tuberculomas have been described, but can not differentiate them completely from other tumors such as astrocytomas. Because the incidence of tuberculomas has increased in

[Brain stem insufficiency in acute ischemia and cerebral hemorrhage].

Consciousness disorders are closely related to the general dysfunction of the brain stem, and called by some authors brain stem insufficiency. To evaluate the degree of brain stem dysfunction, an original scale has been elaborated. Each group of brain stem functions are scored. Consciousness being the most important symptom of the brain stem insufficiency is scored about 50% in a 63-score scale. Scores are used to achieve the most objectivity in the clinical monitoring of the brain stem insufficiency. Its utility was examined in 75 patients with either ischemia or cerebral hemorrhage. Patients

Brain stem and cervical cord dysraphic lesions in iniencephaly.

Iniencephaly is a rare, lethal, axial dysraphic malformation complex diagnosed on the basis of three cardinal features: deficiency of the occipital bone, cervicothoracic spinal retroflexion, and rachischisis. The majority of the patients also have various associated viscerae malformations. An iniencephalic female fetus delivered at 35 5/7 weeks of gestation revealed severe anomalies of the central nervous system and the spine: the cerebellar vermis was hypoplastic, the medulla oblongata was flattened and broadened, and the cervical canal was widely patent dorsally. The thoracolumbar spinal cor

[Auditory brain-stem response in obstructive sleep apnea syndrome].

Auditory brain-stem responses (ABRs) of 20 patients suffering from obstructive sleep apnea syndrome (OSAS) were studied. Their average I, V latencies and III-V intervals were significantly prolonged; the I-III intervals were statistically shortened; but the III latencies and I-V intervals basically were normal. ABRs of 16 patients were recorded again in 1-2 months following uvulopalatopharyngoplasty (UPPP). All parameters except thresholds were not significantly different from preoperative ones. These results may imply that there may be some effects of OSAS on cochlea and auditory brain-stem s

[Severe brain stem ischemia caused by subclavian steal syndrome].

Subclavian steal syndrome (SSS) is produced by occlusion of the proximal subclavian artery and subsequent retrograde filling of the subclavian artery via the vertebral artery. The resulting alteration of blood flow may cause a variety of symptoms secondary to decreased blood supply to the brain and upper extremity of the affected side. A case of severe brain stem ischemia caused by SSS was reported. We performed a subclavian-subclavian graft shunt for this patient and obtained good results. Such a case has been only rarely described in the literature. Complementary information obtained from di

Dysphagia following brain-stem stroke. Clinical correlates and outcome.

We studied 23 individuals (16 men, seven women; mean age 57 years) who had brainstem strokes confirmed by computed tomography of the head or magnetic resonance imaging. Videofluoroscopic modified barium-swallowing examination showed aspiration in 15 of 23 patients. Of the 15 aspirating patients, the majority had bilateral strokes with multiple lesion loci, most often in association with large-vessel disease. All 15 patients had involvement of the pons or medulla. Statistical analyses revealed a significant association between aspiration and pharyngeal residue observed under videofluoroscopy, c

Subcortical arteriosclerotic encephalopathy: brain stem findings with MR imaging.

Magnetic resonance images of 44 patients who had varying degrees of supratentorial signal abnormalities compatible with subcortical arteriosclerotic encephalopathy (SAE) were reviewed for posterior fossa findings. Brain stem lesions frequently accompanied supratentorial SAE and were seen as multiple, fairly symmetric areas of poorly defined, increased signal intensity on axial T2-weighted images. Involvement was generally confined to the central portions of the mid and upper pons. The typical appearance of brain stem involvement by SAE and its probable pathogenesis are reviewed.

Abnormal auditory brain-stem responses in hallucinating schizophrenic patients.

Abnormal auditory brain-stem responses (ABRs) were recorded in 10 out of 20 schizophrenic in-patients. The response abnormalities did not show any correlation to the degree of psychopathology, sub-group of schizophrenia, age, sex, or cerebral ventricular enlargement. Nor was there any correlation to previous neuroleptic treatment: a pathological ABR was recorded in 5 of the 8 patients who had never received such medication. A statistically significant relationship was found between ABR pathology and auditory hallucinations: 9 of the 11 patients who admitted having hallucinations exhibited brai

[Histopathologic criteria of primary and secondary brain stem lesions].

The brain stems from 52 corpses were microscopically examined. These cases died as a result of closed head injuries, which were clinically diagnosed and/or diagnosed postmortem as primary brain stem lesions. The morphological changes in these cases were compared with morphological changes in the brain stems of corpses who died from cerebral hemorrhage with additional secondary brain stem lesions. The examinations revealed acidophilic necrosis of the vessel walls in brain stem hemorrhages with fibrin impregnation of the vessel walls. Fibrin penetration to the perivascular space was the basic mo

Electrocochleography and brain-stem potentials in Ramsay Hunt syndrome.

Audiometric investigations and electrophysiologic recordings of cochlear and brain-stem auditory evoked potentials (BAEPs) were performed in 13 patients to elucidate further the type of hearing disorders in Ramsay Hunt syndrome. Transtympanic electrocochleography showed no enhancement of summating potential and did not suggest secondary endolymphatic hydrops. The recording of BAEPs was clearly abnormal in several of the 13 patients. The striking feature of the abnormalities in these patients was the prolongation of the latencies of waves III and V with the preservation of wave I, which clearly

Universal dissociated anesthesia due to bilateral brain-stem infarcts.

A 59-year-old man had loss of pin-prick and temperature sensation over his head, face, neck, trunk, and all extremities while light touch, vibration, joint position sense, and deep pain were preserved. This was the cumulative result of infarctions in the territories of the right superior cerebellar and left posterior inferior cerebellar arteries that occurred three years apart. To our knowledge, a syndrome of bilateral discrete interruption of spinothalamic tracts and of the spinal tracts of the trigeminal system in the brain stem has not been reported. Dissociated sensory loss enveloping the

Maturation of contralateral brain-stem responses in preterm infants.

Average auditory brain-stem responses (ABRs) evoked by left and right ear stimulation of varying intensities were recorded from ipsilateral and contralateral scalp placements in human neonates. Subjects included pre-term infants, healthy full-term infants and high risk, full-term infants. Pre-term infants were classified into 4 age groups ranging from 29 to 41 weeks post-conceptional age (PCA). Significant ipsilateral/contralateral differences were observed. Further, this effect was highly dependent upon age. The youngest pre-term infants were devoid of contralateral activity despite robust ip

Brain stem gliomas in childhood. Rational approach and treatment.

Thirty-six infants and children with brain stem glioma diagnosed between 1967 and 1980 were reviewed. Posterior fossa craniotomy was performed in 25, with biopsy or partial resection in 18, cyst aspiration in 2, and exploration alone in 5. The 19 surgical specimens obtained revealed neoplastic cells in 15 but only gliosis or blood clot in 4. There was a significant discrepancy in pathological spectrum between autopsy and surgical specimens. The review of the literature shows that 51.6% of autopsy confirmed tumors were malignant while 26.8% of surgically biopsied tumors were benign. Since biops
