Active middle ear implants (AMEIs) have been studied to overcome the limitations of conventional hearing aids such as howling, occlusion, and social discrimination. AMEIs usually drive the oval window (OW) by means of transmitting vibrational force through the ossicles and the vibrational force corresponding to sound is generated from a mechanical actuator. Recently, round window (RW) stimulation using an AMEI such as a floating mass transducer (FMT) to deliver sound to the cochlea has been introduced and hearing improvement in clinical use has been reported. Although previous studies demonstr

Compared to auditory brainstem responses (ABRs), cochlear microphonics (CMs) may be more appropriate to serve as a supplement to the test of otoacoustic emissions (OAEs). Researchers have shown that low-frequency CMs from the apical cochlea are measurable at the tympanic membrane using high-pass masking noise. Our objective is to study the effect of such noise at different intensities on low-frequency CMs recorded at the ear canal, which is not completely known. Six components were involved in this CM measurement including an ear canal electrode (1), a relatively long and low-frequency tonebur

Effects of conductive hearing loss on level and spectrum are well known. However, little is known about possible additional effects on temporal aspects of sound transmission. This study investigated effects of earplugs and middle ear effusions on amplitude and timing of cochlear microphonic (CM) responses in gerbils. Bilateral CM responses to pure tones (1-16 kHz) were monitored before and after (i). unilateral earplug insertion or (ii). injection of silicone oil, of various viscosities, into one middle ear. Earplugs produced flat hearing losses (mean 13 dB) and delayed CMs more at lower (mean

Auditory brainstem responses (ABRs) and otoacoustic emissions (OAEs) are objective measures of auditory function, but are not hearing tests. Normal OAEs reflect normal cochlear outer hair cell function, and an ABR indicates a synchronous neural response. It is quite possible for a patient to have normal OAEs but absent or grossly abnormal ABR and a behavioral audiogram that is inconsistent with either test. These patients, who may constitute as much as 10% of the diagnosed deaf population, have auditory neuropathy/dys-synchrony (AN/AD). To diagnose AN/AD accurately, ABRs are obtained in respon

Prolonged exposure to low-level noise has often been used scientifically as well as clinically to induce neuroplastic changes within the central auditory pathway in order to reduce central gain, suppress tinnitus and hyperacusis, and modulate different features of central auditory processing. A fundamental assumption underling these studies is that the noise exposure levels are so low that they have no effect on the neural output of the cochlea. Therefore, functional changes occurring in the central auditory pathway must be the results of central rather than peripheral changes. In an attempt t

A new method for detecting the acoustic reflex that utilizes standard otoacoustic emissions recording techniques is introduced and discussed. Two successive identical tone bursts of 100 ms duration and 10 ms interstimulus interval are presented in the occluded ear canal at a repetition rate of one per second. If the acoustic reflex is elicited, the contraction of the stapedius muscle is delayed with respect to the onset of the first stimulus. Hence, the acoustic compliance in the ear canal decreases primarily during the second stimulus. The difference of the microphone signals produced by the

In adults and children, 203 normal ears, 124 cars with hearing-impairment and 37 ears with middle ear problems were studied concerning click-evoked oto-acoustic emissions. According to the results, this method by high specifity and sensitivity renders possible the recognition of low sensory hearing losses. Complete middle ear effusions too make themselves conspicuous by preventing the reception of these responses. The recording of click-evoked oto-acoustic emissions is a most promising auditory screening-method. A general use could essentially improve the early detection of hearing losses in i

Oxidative stress is considered a major cause of the structural and functional changes associated with auditory pathologies induced by exposure to acute acoustic trauma AAT). In the present study, we examined the otoprotective effects of 2,4-disulfophenyl-N-tert-butylnitrone (HPN-07), a nitrone-based free radical trap, on the physiological and cellular changes in the auditory system of chinchilla following a six-hour exposure to 4 kHz octave band noise at 105 dB SPL. HPN-07 has been shown to suppress oxidative stress in biological models of a variety of disorders. Our results show that administ

We have estimated the transfer curve relating instantaneous sound pressure in the ear canal to instantaneous receptor current through the outer hair cells (OHCs) in the basal turn of the guinea pig cochlea using the cochlear microphonic (CM) elicited by continuous 200 Hz tones. The transfer curve is well approximated by a Boltzmann activation curve which has been automatically analysed using a custom-built electronic circuit which continuously derives the three parameters defining the curve with a time resolution of seconds. This technique offers a convenient method of monitoring changes in OH

Previous studies have injected artificial endolymph into scala media in anaesthetized guinea pigs as an acute model of endolymphatic hydrops. Here, we have injected artificial endolymph into scala media in guinea pigs at rates of 40-80 nl/min, whilst monitoring Compound Action Potential (CAP) thresholds, the Summating Potential (SP)/CAP ratio, Cochlear Microphonic (CM) distortion, low-frequency modulated Distortion Product Otoacoustic Emissions (DPOAEs), and the Endocochlear Potential (EP). We found that abrupt recovery of CAP thresholds, SP/CAP ratio, and CM and DPOAE asymmetric distortion co

Free radical injury has been implicated in cochlear damage resulting from exposure to high-intensity noise and due to carbon monoxide (CO) hypoxia. Although exposure to noise plus CO is common in occupational settings and noise-induced hearing loss (NIHL) is enhanced in the presence of CO, potential mechanisms resulting in auditory impairment have not been studied. This study evaluates protective effects of the free radical scavenger phenyl-N-tert-butylnitrone (PBN) against potentiation of NIHL by CO. Three PBN administration protocols have been evaluated in subjects exposed to noise plus CO o

BACKGROUND: Noise exposure in neonatal units has long been suspected of being a cause of hearing loss associated with such units. The noise intensity to which the neonate is exposed varies with the type of ventilatory support used. Also, the post-nasal space is an enclosed cavity that is close to the inner ear and an area of turbulent and hence potentially noisy airflow.
AIM: To determine noise intensities within the ear and post-nasal space in neonates on different modes of ventilatory support using probe microphones, measures previously not undertaken.
METHODS: A portable instrument with a

Mechanical stimulation of the round window (RW) with an active middle ear prosthesis (AMEP) has shown functional benefit in clinical reports in patients with mixed hearing loss (MHL). Further objective physiological data on the efficacy of RW stimulation is needed, however, to demonstrate that RW stimulation with an AMEP can generate input to the inner ear comparable to acoustic input. Cochlear microphonic (CM) and mechanical (stapes velocity) responses to sinusoidal stimuli were measured by electrode and laser Doppler vibrometry in eight chinchillas in response to normal acoustic stimulation

Distortion product otoacoustic emissions (DPOAEs), a sensitive detector of outer hair cell (OHC) function, cochlear microphonics (CM), and hair cell loss have been monitored in 12 awake guinea pigs before and after 2 h exposure to specific, played-back industrial noise (105 dB SPL maximal intensity). All animals had stable DPOAE levels before noise exposure. In the first hours after noise exposure DPOAE levels were reduced significantly. In about 70% a partial recovery of the DPOAEs was found within 4 months after noise exposure. In 16% of the investigated ears no recovery of DPOAEs was observ

Previous measures of distortion-product otoacoustic emissions (DPOAEs) in young adults suggested that these responses were capable of detecting the functional effects of auditory aging. The primary goal of the present study was to provide more detailed evidence for the influence of aging processes on the ability of healthy, older ears to generate DPOAEs. Toward this end, DPOAEs were examined in a series of human subjects, with clinically normal hearing, ranging in age from 31 to 60 years. Acoustic-distortion products were measured in two basic forms consisting of "audiograms" and response/grow

Auditory neuropathy is a type of hearing loss that constitutes a change in the conduct of the auditory stimulus by the involvement of inner hair cells or auditory nerve synapses. It is characterized by the absence or alteration of waves in the examination of brainstem auditory evoked potentials, with otoacoustic and/or cochlear microphonic issues. At present, four loci associated with non‑syndromic auditory neuropathy have been mapped: Autosomal recessive deafness‑9 [DFNB9; the otoferlin (OTOF) gene] and autosomal recessive deafness‑59 [DFNB59; the pejvakin (PJVK) gene], associated with autoso

Hidden auditory neuropathy is characterized by reduced performances in challenging auditory tasks with the preservation of hearing thresholds, resulting from diffuse loss of cochlear inner hair cell (IHC) synapses following primary degeneration of unmyelinated terminals of auditory fibers. We report the audiological and electrophysiological findings collected from 10 members (4 males, 6 females) of a large Italian family affected by dominant optic atrophy, associated with the OPA8 locus, who complained of difficulties in understanding speech in the presence of noise. The patients were pooled i

Commercially obtained aged male CBA/J mice presented a complex pattern of hearing loss and morphological changes. A significant threshold shift in auditory brainstem responses (ABR) occurred at 3 months of age at 4 kHz without apparent loss of hair cells, rising slowly at later ages accompanied by loss of apical hair cells. A delayed high-frequency deficit started at 24 kHz around the age of 12 months. At 20-26 months, threshold shifts at 12 and 24 kHz and the accompanying hair cell loss at the base of the cochlea were highly variable with some animals appearing almost normal and others showin

Guinea pigs, which were either anesthetized (A) or conscious (U), were exposed to four 2 h sessions of broad-band noise of 96 dB SPL. Cochlear microphonics and N1 thresholds were measured prior to killing from 1 to 13 days later. The cochleas were examined by SEM and by section. The U series suffered less N1 threshold loss and recovered within 10-13 days, while the large initial loss in the A series did not completely reverse within the period of study. Initially, the IHC stereocilia in the basal half of the cochlea showed marked bending, the affected area being somewhat more extensive in the

The theory and practice to record the brain stem response and middle latency have been described. Normative data and sources of error have been presented. The early evoked potentials are a powerful tool, to determine the degree of hearing loss in infants and to detect possible brain stem lesions. This exact diagnosis is an assumption for a successful rehabilitation.
