[Tacrolimus in diseases other than atopic dermatitis].

Topical tacrolimus is an immunosuppressant that acts through the inhibition of calcineurin and thus of the T cells. This causes a decrease in the production of interleukins, the granulocyte colony stimulating factor, alpha interferon and tumor necrosis factor. Although the use of topical tacrolimus is only indicated for the treatment of moderate or severe atopic dermatitis, its immunosuppressant effect and fewer side effects regarding topical corticosteroids have lead to the increase of its use in other types of inflammatory skin diseases. The purpose of this article is to review the use of ta

Atopic dermatitis: diagnosis, treatment, and aeromedical implications.

Over the past 30 yr, the prevalence of atopic dermatitis (AD) has increased significantly in the general population. Given this increased burden of disease, more AD will be seen in civilian and military aviators. This article reviews the clinical aspects of AD with a focus on the aeromedical implications of the disease. The diagnosis of AD is mainly clinical. Treatment options are numerous, although topical corticosteroids are generally the mainstay of treatment. The new immunomodulators are options for treatment of AD and have advantages and disadvantages when compared with topical corticoste

Allergic manifestations of skin diseases--atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease which often becomes manifest in early infancy and is characterized by itchy eczematous lesions with characteristic localization. The cellular infiltrate of allergic eczematous skin diseases (i.e. AD, allergic contact dermatitis) is mainly composed of mononuclear cells. Whereas allergic contact dermatitis is always triggered by allergen-specific T cells, a number of allergic and nonallergic trigger factors appear to be relevant in AD. This article discusses data coming from immunological studies focusing on T-cell responses in AD. Th

Eczema molluscatum in tacrolimus treated atopic dermatitis.

Eczema molluscatum describes the occurrence of molluscum contagiosum virus infection in a patient with underlying atopic dermatitis. Novel, safe and effective treatment options in atopic dermatitis are the topical immunomodulators tacrolimus and pimecrolimus. One major advantage over corticosteroids is that they do not induce skin atrophy. Some physicians fear that topical immunomodulators may predispose patients to skin infections. We observed a patient with atopic dermatitis who developed eczema molluscatum during treatment with tacrolimus 0.1% ointment. After withdrawal of tacrolimus, the l

Oxygen consumption during sleep in atopic dermatitis.

Measurements of oxygen consumption (VO2) were made during sleep in 10 patients with atopic dermatitis. Two groups of healthy children acted as controls. All subjects were studied in bed in an environmental temperature of 24-26 degrees C, and sleep was confirmed during continuous electroencephalographic monitoring. Mean (SD) values of VO2 in sleeping patients who were not scratching ranged from 4.0 (0.4) to 7.4 (0.7), which was not statistically significantly different from control values which ranged from 3.24 (0.3) to 5.56 (0.4). During scratching (while asleep), which occurred in nine out of

Early diagnosis of infantile seborrhoeic dermatitis and atopic dermatitis--clinical features.

In a prospective study of thirty-seven infants presenting with dermatitis in infancy it was found that pruritus, a positive family history of atopy, and the age of onset of skin lesions were of little value in distinguishing atopic dermatitis from infantile seborrhoeic dermatitis. The most useful distinguishing features were the increased incidence of lesions on the forearms and shins in atopic dermatitis, and in the axillae in infantile seborrhoeic dermatitis. The onset of skin lesions solely in the napkin area favoured a diagnosis of infantile seborrhoeic dermatitis, but when more than one s

Effect of tralokinumab on moderate-to-severe atopic dermatitis in patients with atopic comorbidities.

BACKGROUND: Atopic dermatitis (AD) is known to be associated with other atopic comorbidities that all involve type 2 immune dysregulation. Tralokinumab is a monoclonal antibody that specifically targets interleukin-13. As comorbid atopic disease could indicate a more severe AD phenotype, it is important to assess the effect of tralokinumab treatment in patients with these comorbidities. OBJECTIVE: To assess the efficacy and safety of tralokinumab treatment for AD in adult and adolescent patients with moderate-to-severe AD with and without a patient-reported history of atopic comorbidities at

Clinical, immunological and MRI features of myelitis with atopic dermatitis (atopic myelitis).

In order to clarify the characteristic features of myelitis with atopic dermatitis (AD), we compared the clinical, immunological and MRI findings between 14 myelitic patients with AD and 12 myelitic patients without AD. The myelitic patients with AD showed the following distinct features, compared with those without AD. (1) A preferential involvement of the cervical cord, as shown by neurologic as well as MRI examinations (14/14 vs. 5/12; P=0.0012), (2) paresthesia/dysesthesia as the predominant symptoms and a rare occurrence of definite muscle weakness (0/14 vs. 5/12; P=0.0120) and dysuria (1

Barbiturates enhance itch-associated scratching in atopic dermatitis mice: A possible clue to understanding nocturnal pruritus in atopic dermatitis.

In chronic pruritic diseases such as atopic dermatitis, pruritus is exacerbated during nocturnal sleep; however, the underlying mechanism remains unclear. We previously demonstrated that acute administration of the sedative-hypnotics ethanol markedly enhanced itch-associated spontaneous scratching in a diet-induced mouse model of atopic dermatitis. In the present study, to expand our previous finding and provide a general mechanism for the central modulation of chronic itch, we examined whether other hypnotic drugs, such as barbiturates and benzodiazepines, also enhance scratching, and further

Atopic Dermatitis Flare Following Initiation of Nemolizumab.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disorder marked by intense pruritus and episodic flares. While nemolizumab, an interleukin-31 (IL-31) inhibitor, has demonstrated efficacy in reducing itch severity, its broader effects on skin inflammation are not fully understood. CASE PRESENTATION: We describe a 49-year-old male with longstanding, treatment-resistant AD who began off-label therapy with nemolizumab. Within weeks of initiation, he developed a new pruritic vesiculobullous eruption localized to the palms, clinically and histologically consistent with dyshidrotic

Dermatological dichotomy: Atopic dermatitis or mycosis fungoides?

Источник пометил публикацию как отозванную

The presence of atopic dermatitis (AD) and mycosis fungoides (MF) presents diagnostic challenges due to their shared clinical features. AD, a chronic skin disorder characterized by pruritic and inflamed lesions, shares these features with MF, which is the most common cutaneous T-cell lymphoma. A 19-year-old male, who had a history of childhood AD, developed eczema-like lesions on his forearms, neck, and head. Initially, allergic contact dermatitis was suspected due to his occupational metal exposure. However, despite treatment with topical therapies, his condition worsened, requiring 3 cycles

Atopic dermatitis - Perspectives and unmet medical needs.

Atopic dermatitis (AD) is a chronic inflammatory skin disease for which many new insights have been gained in recent years through a better understanding of pathophysiology, concomitant diseases and therapeutics in particular. In this review, new and practice-relevant results from current research are presented. Many studies have been performed on the diagnosis of AD and on different subtypes, yet no diagnostic biomarker or clinical predictor of treatment response has been established. For topical treatment, some agents such as Janus kinase (JAK) inhibitors are in advanced stages of clinical t

CLA+ memory T cells in atopic dermatitis.

Circulating skin-homing cutaneous lymphocyte-associated antigen (CLA)+ T cells constitute a small subset of human memory T cells involved in several aspects of atopic dermatitis: Staphylococcus aureus related mechanisms, the abnormal Th2 immune response, biomarkers, clinical aspects of the patients, pruritus, and the mechanism of action of targeted therapies. Superantigens, IL-13, IL-31, pruritus, CCL17 and early effects on dupilumab-treated patients have in common that they are associated with the CLA+ T cell mechanisms in atopic dermatitis patients. The function of CLA+ T cells corresponds w

Aberrant serine protease activities in atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease; the three major factors responsible for AD, i.e., epidermal barrier dysfunction, allergic inflammation, and itching, interact with each other to form a pathological condition. Excessive protease activities are characteristic abnormalities that affect the epidermal barrier in patients with AD. In normal skin, epidermal serine protease activities are controlled by kallikrein-related peptidases (KLKs) and their inhibitors, including lympho-epithelial Kazal-type-related inhibitor (LEKTI). In AD lesions, KLKs are excessively expressed,

Differences between pediatric and adult atopic dermatitis.

Atopic dermatitis (AD) is a common disease with a broad spectrum of clinical manifestations. AD can manifest differently in adults than children. Core AD features are similar between children and adults overall, including lesions affecting flexural areas, presence of atopy, and xerosis. Adults have more signs of chronic disease, higher prevalence and different patterns of hand eczema, and a stronger relationship of disease activity with emotional factors, whereas children with AD have more exudative lesions, perifollicular accentuation, pityriasis alba, Dennie-Morgan folds, and seborrheic derm
