Gracilis autograft for recurrent posttraumatic instability of the superior tibiofibular joint.

BACKGROUND: Several reconstruction procedures have been proposed to manage recurrent dislocation of the superior tibiofibular joint. HYPOTHESIS: Reconstruction of the superior tibiofibular joint using a gracilis tendon autograft is effective in recurrent dislocation of the superior tibiofibular joint. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Eight patients with recurrent dislocation of the superior tibiofibular joint without anatomical predisposing factors and who practiced sports were included in the study. Evaluation included the modified Cincinnati rating system and the

Prevalence and identity of Sarcocystis spp. in roe deer (Capreolus capreolus) in Spain: a morphological study.

Muscular samples from the oesophagus, diaphragm and heart of 101 roe deer (Capreolus capreolus) hunted in Galicia (Northwestern Spain) were examined, by the compression method, for the presence of Sarcocystis spp. infection. The structure of the cyst wall was examined by light (LM) and transmission electron microscopy (TEM). The overall prevalence of infection was very high (99%), with a density of 404 cysts/sample (SD 812). The prevalence was very similar in the different examined muscle types (99% heart and diaphragm, and 98.9% oesophagus). A significantly higher intensity of infection was f

A novel neuromuscular form of glycogen storage disease type IV with arthrogryposis, spinal stiffness and rare polyglucosan bodies in muscle.

Glycogen storage disease type IV (GSD IV) is an autosomal recessive disorder causing polyglucosan storage in various tissues. Neuromuscular forms present with fetal akinesia deformation sequence, lethal myopathy, or mild hypotonia and weakness. A 3-year-old boy presented with arthrogryposis, motor developmental delay, weakness, and rigid spine. Whole body MRI revealed fibroadipose muscle replacement but sparing of the sartorius, gracilis, adductor longus and vastus intermedialis muscles. Polyglucosan bodies were identified in muscle, and GBE1 gene analysis revealed two pathogenic variants. We

Postoperative monitoring in free muscle transfers for reconstruction in brachial plexus injuries.

Free gracilis transfers are done for reanimation of the upper limb in traumatic total brachial plexus palsy. Because of buried nature of the free muscle and monitoring skin flap in the axillary or infraclavicular region, it is always a tricky situation for continuous and repeated monitoring to assess vascular status. Critical ischemia times vary between the muscle and monitoring skin flap because of which signs of ischemic changes in the monitoring skin flap are always delayed with respect to the muscle. We describe a novel method that uses the principle of evoked potentials from the muscle to

Continent perineal colostomy by electrostimulated graciloplasty in abdominoperineal resection. A preliminary report.

Despite the tendency toward sphincter-saving surgical procedures, a small proportion of rectal cancers must still be treated by abdominoperineal resection (APR). The physical, psychological and social consequences of a permanent abdominal stoma are a challenge to perform a continent perineal colostomy. Most of the attempts originate from experiences with gracilis muscle transposition in the treatment of fecal incontinence, in particular Pickrell's operation. Functional results are however conditioned by the fact that the transposed muscle takes up a different function and its natural evolution

Free microneurovascular muscle transfer in limbs to provide motor power.

The experience gained in a series of 7 free microneurovascular muscle transfers to the limbs over a seven-year period has led us to conclude that: free gracilis microneurovascular flaps can be safely transferred and can aid restoration of movement following severe injuries of the limbs, including post-replantation cases; inclusion of a cutaneous island with the muscle is safe, improves cover, and simplifies closure following muscle transfer; tendon adhesions and joint stiffness are the greatest obstacles to transferred muscle function, and early passive extension is recommended; additional pro

Hamstrings Muscle Morphology After Anterior Cruciate Ligament Reconstruction: A Systematic Review and Meta-Analysis.

BACKGROUND: Hamstrings muscle morphology is determinant of muscle function (i.e. strength). Among individuals with ACL reconstruction (ACLR), less cross-sectional area (CSA) and volume in the ACLR-limb are associated with muscle weakness, and may contribute to lower rates of return to preinjury activity level and an increased risk for long-term sequelae. OBJECTIVES: To effectively treat muscular impairments, an accurate understanding of differences in hamstrings morphology following ACLR is needed. A systematic review and meta-analysis were undertaken to describe the morphology of the hamstri

A Case of Urothelial Carcinoma With Squamous Differentiation of the Bladder, Infiltrating the Small Intestine and Rectus Abdominis, Treated With the Use of a Short Gracilis Myocutaneous Flap.

INTRODUCTION: Squamous differentiation (SD) occurs in up to 20% of muscle invasive bladder cancers. CASE PRESENTATION: An 85-year-old man with an intrapelvic mass invading the bladder, small intestine, and rectus abdominis presented to our department. Cystoscopy showed a necrotic mass at the dome of the bladder. Histopathological examination of specimens from transurethral resection indicated squamous cell carcinoma. Radical cystectomy with resection of the small intestine and rectus abdominis, and reconstruction of the abdominal wall using a left short gracilis myocutaneous flap was performe

Reconstruction of soft tissue after complicated calcaneal fractures.

A total of 35 flap reconstructions were done to cover exposed calcaneal bones in 31 patients. All patients had calcaneal fractures, 19 of which were primarily open. Soft tissue reconstruction for the closed fractures was indicated by a postoperative wound complication. A microvascular flap was used for reconstruction in 21 operations (gracilis, n = 11; anterolateral thigh, n = 5; rectus abdominis, n = 3; and latissimus dorsi, n = 2). A suralis neurocutaneous flap was used in eight, local muscle flaps in three, and local skin flaps in three cases. The mean follow-up time was 14 months (range 3

Facial palsy and achondroplasia: a rare association.

The association of facial palsy and achondroplasia is rare. The authors present two cases of such association. To their knowledge this has never been described before in the medical literature. Both patients underwent a two-stage facial reanimation procedure using nerve grafting and a free gracilis muscle transfer with very satisfactory functional and cosmetic results.

Free functional gracilis transfer for reconstruction of isolated anterior deltoid atrophy following surgical proximal humerus fixation.

BACKGROUND: Palsy of the clavicular head of the deltoid is a complication after surgical fixation of proximal humerus fractures. Flexion of the shoulder joint may be impaired as a result. Additionally, patients may complain of joint instability, visible atrophy, and pain. Where nerve reconstruction is not possible, muscle transfers remain as secondary reconstructive procedures. METHODS: Three patients with anterior deltoid palsy after proximal humerus fixation received a free functional gracilis transfer to the shoulder. Postoperatively, patients underwent biofeedback-based rehabilitation. Be

Free muscle transfer in posttraumatic plexopathies: part III. The hand.

BACKGROUND: In lower root avulsion plexopathies, free muscle transfers for hand reanimation provide the only hope for the paralyzed hand, as the outcomes of hand functional restoration after primary brachial plexus reconstruction are uniformly poor. The purpose of this study was to analyze the outcomes of free gracilis muscle transfers for hand reanimation in severe brachial plexus injuries in relation to the respective motor donors. METHODS: Since 1981, 71 free gracilis muscles have been transplanted for hand reanimation. Thirty-eight were for finger flexion and 33 were for finger extension.

Functioning free gracilis myocutaneous flap transfer provides a reliable single-stage facial reconstruction and reanimation following tumor ablation.

BACKGROUND: Ablative orofacial defects incorporating mimetic facial musculature/nerve cause hemifacial expressive dysfunction and considerable morbidity but are rarely reanimated immediately using free functioning gracilis myocutaneous flaps. METHODS: Disrupted buccal branches provided a recipient facial nerve for 24 gracilis reinnervations. An additional 15 free flaps were used for extensive composite defects. Smile outcome was graded according to Terzis' criteria after 2 years of recurrence-free follow-up. The effects of postoperative radiotherapy, integrity of the oral commissure, and doub

Free functioning gracilis transplantation for reconstruction of elbow and hand functions in late obstetric brachial plexus palsy.

BACKGROUND: In late obstetric brachial plexus palsy (OBPP), restoration of elbow and hand functions is a difficult challenge. The use of free functioning muscle transplantation in late OBPP was very scarcely reported. In this study, we present our experience on the use of free functioning gracilis transfer for restoration of elbow and hand functions in late cases of OBPP. PATIENTS AND METHODS: Eighteen patients with late OBPP underwent free gracilis transfer for reconstruction of elbow and/or hand functions. The procedure was indicated when there was no evidence of reinnervation on EMG and in

Neurovascular free-muscle transfer to treat facial paralysis associated with hemifacial microsomia.

Despite the wide spectrum of hemifacial microsomia manifestations, treatment mainly focuses on mandible and ear abnormalities, rather than on facial paralysis. In fact, the surgical treatment of facial paralysis associated with hemifacial microsomia is quite underdeveloped, because the degree of paralysis is frequently incomplete or partial. Timing and type of surgery are also difficult to determine. Neurovascular free-muscle transfer is now a standard procedure for the dynamic smile reconstruction of longstanding facial paralysis. This type of strategy has considerable potential in the treatm

[A family with nonsynonymous mutations of filamin C actin-binding domain].

We report a sibling case of progressive muscle weakness beginning in their 40s. The younger brother was initially suspected to have spinal muscular atrophy based on electromyography findings, but no SMN1 gene deletion was identified. Whole-exome sequencing in the siblings detected a heterozygous nonsynonymous mutation, c.577G>A (p.Ala193Thr), located in the actin-binding domain (ABD) of the filamin C (FLNC) gene. Muscle weakness of a sibling started in the proximal lower limbs and gradually progressed to the distal lower limbs, trunk, and upper limbs. Muscle MRI showed marked involvement of th

Gracilis free flap in head and neck reconstruction beyond facial palsy reanimation.

INTRODUCTION: The gracilis muscle free flap has gained popularity in head and neck reconstruction due to minimal donor-site morbidity, reliable vascular pedicle, strong muscular component, and possibility to perform nerve coaptation. However, almost all the existing evidence in the literature is related to its use for facial palsy reanimation. The aim of this study was therefore to review and provide a comprehensive summary of all the possible indications and outcomes of this versatile free flap in head neck reconstructive surgery. MATERIALS AND METHODS: A systematic review of the literature
