[Therapeutic modalities in the treatment of palmar and axillary hyperhidrosis].

The authors summarize their therapeutic methods of palmar and axillary hyperhidrosis in this article. They discuss the characteristics, frequency of upper limb and patho-anatomical features of chronic sweating. Authors summarized their almost 40 years experience in treatment methods, technological improvements as well as they review recent knowledge and relevant literature. The open thoracotomic desympathization applied in the beginnings was gradually replaced by a minimally invasive VATS-sympathectomy (Video Assisted Thoracoscopic Surgery). The possibilities of conservative treatments, local

Analysis of family history of palmoplantar hyperhidrosis in Japan.

Palmoplantar hyperhidrosis (PPH) is a disorder characterized by excessive sweating of the palmar surfaces of the hands and feet due to emotional sweating. There have been reports based on family histories, and the involvement of genetic factors has been suggested. Among 410 PPH patients who visited our hospital from August 2006 to October 2008, onset age and family history were investigated in order to prepare pedigree charts, and family histories were confirmed in 147 patients (36%). Between the family history and negative family history (sporadic) groups, no significant differences were seen

Surgical approaches and techniques in the management of severe hyperhidrosis.

Thoracoscopy has afforded a huge advance for upper thoracic sympathetic procedures compared with prior open procedures. Different clinical syndromes of hyperhidrosis exist and require different forms of treatment. The classic severe palmoplantar pattern of hyperhidrosis will not respond effectively in the long term to any nonoperative treatment and requires sympathectomy for cure. Thoracoscopic sympathectomy is the first-line treatment in these patients. The author's preference is sympathotomy at the second or third rib level, because this method spares the ganglion. It is possible that gangli

Guidelines for the prevention, diagnosis and treatment of compensatory hyperhidrosis.

With the objective of establishing guidelines for the prevention, diagnosis and treatment of compensatory hyperhidrosis, consensus meetings were held. Attendees included a general surgeon and thoracic surgeons affiliated with the Brazilian Society of Thoracic Surgery. The topics addressed were those that would ostensibly broaden multidisciplinary knowledge. Based on recent guidelines for the prevention, diagnosis and (clinical and surgical) treatment of compensatory hyperhidrosis, as well as on a review of the medical literature, the participants prepared a preliminary text, whose recommendati

Temperament and character profile in patients with essential hyperhidrosis.

BACKGROUND/AIMS: The main debates on hyperhidrosis are whether there are some personality features such as anxiety or stress, or whether the psychological symptoms are due to negative social feedback from early life. We aimed to investigate the temperament and character dimensions with a psychobiological inventory. METHODS: We recruited patients with hyperhidrosis (n=26) and chronic renal failure (n=44) as well as healthy subjects (n=22) from dermatology and nephrology clinics and the general population in Afyonkarahisar. All subjects were required to perform a temperament and character inven

Evolution of clipping for thoracoscopic sympathectomy in symptomatic hyperhidrosis.

BACKGROUND: Severe hyperhidrosis is treated by thoracic sympathetic chain interrupting through chain transection or clipping. Our study compared the efficacy of these 2 methods. METHODS: Retrospectively, patients who underwent thoracoscopic sympathectomy from 1999 to 2005 had demographic, operative, and postoperative data analyzed. RESULTS: Fifty-four operations were performed for refractory sweating of the palm (72%), axilla (66%), foot (53%), and head/neck (19%). Thirty-seven (69%) underwent clipping; 17 (31%) underwent chain transection. There was no difference in age, sex, or blood loss.

Epidemiological survey of primary palmar hyperhidrosis in adolescents.

BACKGROUND: Despite recent advances in recognition and treatment of primary palmar hyperhidrosis (PPH), the epidemiological survey has hardly been conducted. The aim of this study was to investigate the prevalence and epidemiological characteristics of primary PPH among adolescents in three cities of southeast China. METHODS: Stratified-cluster sampling was carried out and cross-sectional epidemiological survey by questionnaire was applied among 33 000 college and high school students. RESULTS: The prevalence rate of PPH was 4.36% affecting both sexes equally. Prevalence rate of severe PPH w

Topical atropine sulfate for the treatment of axillary hyperhidrosis.

BACKGROUND: Axillary hyperhidrosis does not have a low-cost, free of secondary effects, satisfactory treatment. Eccrine hidrocystomas have been successfully treated with topical atropine solution. Hypothesis Axillary hyperhidrosis could respond to the topical application of atropine solution. METHODS: Ten patients were selected. Eight with mild pure primary axillary hyperhidrosis and two with compensatory sweating after sympathectomy. One milliliter of a water solution of atropine sulfate at 1% was applied twice a day over the affected area and massaged for 30 s. Treatment was maintained for

Azygos lobe in endoscopic thoracic sympathectomy for hyperhidrosis.

BACKGROUND: Before the introduction of endoscopic thoracic sympathectomy (ETS) for the management of hyperhidrosis, anomalous azygos lobes of the lung were rarely encountered. When performing ETS, it is important to have knowledge of and be prepared to deal with this anatomical anomaly. METHODS: A database of 2,272 patients who underwent ETS was reviewed to determine the incidence of azygos lobe, describe the characteristics of patients with azygos lobe, and evaluate the outcomes. RESULTS: Twenty cases (0.9%) of azygos lobe were identified. Two distinctive variations were encountered, one in

Thoracoscopic sympathectomy for axillary hyperhidrosis: the influence of T4.

BACKGROUND: Recent data suggest that severe compensatory sweating after sympathectomy for hyperhidrosis is more common than previously reported. In particular, T2-T4 sympathectomy for axillary hyperhidrosis leads to significantly more disabling sweating compared with T2-T3 sympathectomy for palmar hyperhidrosis. However, it is not known whether this is a result of the additional transection of the T4 segment or if patients with primary axillary hyperhidrosis are more prone to experience disabling compensatory sweating. METHODS: A follow-up study by questionnaire was made of 100 consecutive pa

[Surgical treatment for compensatory hyperhidrosis in Adie syndrome].

Ross syndrome is characterized by a triad of tonic pupil, areflexia and segmental hypohidrosis. Hypohidrosis may be accompanied by contralateral hyperhidrosis, probably due to a compensatory mechanism. We report a case of Ross syndrome with socially disabling left-sided hyperhidrosis. Sympathectomy of the second and third thoracic ganglia was performed with satisfactory results. With excellent results in primary hyperhidrosis and very low morbidity, thoracic sympathectomy is the definitive treatment for hyperhidrosis.

Hyperhidrosis and botulinum toxin A: patient selection and techniques.

Focal idiopathic excessive eccrine sweating presents most commonly as an affliction of three anatomically distinct area: the axillae, the palms and soles, and the upper face. The true incidence is not known, but about half of the patients referred to us with this condition have at least one first-degree relative similarly affected. Only a fraction of patients afflicted are thought to seek medical care because of the social stigma, lack of understanding on the part of medical providers as to the cause and nature of the problem, and, until now, lack of effective nonsurgical therapy. [Multiple sclerosis with sensory disturbances and hyperhidrosis of the hand].

INTRODUCTION: Hyperhidrosis may result from brain, spinal cord or peripheral nervous system injuries. We report the case of a 26-year-old patient who presented with sensorial disorders and focal hyperhidrosis of the right hand. OBSERVATION: MRI revealed multiple lesions very suggestive of active demyelinization. A lesion in the left paramedian posterior portion of the pons was enhanced after gadolinium infusion. The diagnosis of multiple sclerosis was suggested by MRI data and oligoclonal IgG bands on cerebrospinal fluid electrophoresis. CONCLUSIONS: Focal hyperhidrosis may be due to a lesio

Treatment of plantar hyperhidrosis with botulinum toxin type A.

BACKGROUND: Hyperhidrosis (excessive, uncontrollable sweating) can be embarrassing and disabling, significantly impacting social and professional performance and quality of life. Treatments aim to reduce sweating, but few are effective, often carrying the risk of significant side-effects. The aims of this study were to evaluate the efficacy and safety of botulinum toxin type A (BTX-A) for plantar hyperhidrosis and to investigate the role of the Dermojet as a potential injection technique. METHODS: Ten adult patients (five men, five women), aged 19-51 years, with severe, previously unresponsiv

Treatment of compensatory gustatory hyperhidrosis with topical glycopyrrolate.

Gustatory hyperhidrosis is facial sweating usually associated with the eating of hot spicy food or even smelling this food. Current options of treatment include oral anticholinergic drugs, the topical application of anticholinergics or aluminum chloride, and the injection of botulinum toxin. Thirteen patients have been treated to date with 1.5% or 2% topical glycopyrrolate. All patients had gustatory hyperhidrosis, which interfered with their social activities, after transthroacic endoscopic sympathectomy, and which was associated with compensatory focal hyperhidrosis. After applying topical g

Early experience with thoracoscopic sympathectomy for palmar hyperhidrosis.

Excessive craniofacial, palmar and axillary hyperhidrosis can be very distressing in young people, especially in hot climates. In this study, we are presenting our operative experience and the long-term effect of the technique of thoracospic electrocoagulation of the thoracic sympathetic chain in the treatment of this condition. We reviewed the results of 22 thoracoscopies performed on 16 patients at Asir Central Hospital in Abha, Saudi Arabia during the period from January 1999 to December 2002. The patients were 11 males and five females with a mean age of 26.9+/-5 years (range 19-35 years).
