[A case of xanthogranulomatous pyelonephritis presenting with the flank subcutaneous mass].

A 51-year-old female exhibited fever, left flank pain and left flank mass in March, 1993. Drip infusion pyelography (DIP) revealed a non-functioning left kidney with shadows of calculi, and abdominal computerized tomography (CT) showed renal calculi and multilocular cystic lesions in the left kidney extending through the perinephric space into the mass on the left flank. Percutaneous nephrostomy and percutaneous drainage were performed, followed by left nephrectomy. Histopathological findings revealed xanthogranulomatous pyelonephritis. There have been a few case reports of xanthogranulomatous

Comparison of computed tomography and ultrasound in abdominal staging of renal cancer.

The relative accuracy of computed tomography (CT) and ultrasound in abdominal staging of renal cancer was determined in 22 patients. CT is capable of detecting tumor invasion of perinephric fat and adjacent muscles, which cannot usually be shown by ultrasound. While both CT and ultrasound demonstrate venous and retroperitoneal tumor extension, CT is more reliable since bowel gas not infrequently obscures the retroperitoneum on ultrasonic scanning. However, ultrasound will often provide valuable information; and whenever a solid renal mass is detected by echography using prone scans, abdominal

Uncommon Development of Intra-abdominal Abscesses Following Acute Pancreatitis: A Case Report.

Acute pancreatitis is not an uncommon gastrointestinal condition with the potential for both local and systemic complications. While pancreatic necrosis and pseudocyst formation are well-recognized complications, the formation of distant intra-abdominal abscesses is a rare occurrence. We report the case of a 52-year-old man who presented to the emergency department with abdominal pain, noted to be secondary to pancreatitis. Abdominal imaging additionally revealed a left perinephric abscess with additional intra-abdominal abscesses thought to be secondary to acute pancreatitis. This case highli

Isolated renal mucormycosis: an unusual cause of acute renal infarction in a boy with aplastic anaemia.

Mucormycosis is an opportunistic fungal infection occurring most often in immunocompromised patients. The most common clinical presentation of infection with the fungus is rhinocerebral, followed by primary pulmonary, disseminated, gastrointestinal and cutaneous manifestations. Renal involvement in disseminated mucormycosis is well described, however, there are only few case reports describing the isolated renal mucormycosis in literature. We present an unusual case of renal mucormycosis in a patient of aplastic anaemia who presented with right flank pain and, on imaging, was found to have ren

Muconephrosis.

We report a case of a mucus-filled kidney (muconephrosis) encountered during laparoscopic nephrectomy for presumed xanthogranulomatous pyelonephritis. Conversion to open nephrectomy and en-bloc right hemicolectomy were necessary because of severe perinephric fibrosis and suspected renal-enteral fistula. Pathologic examination revealed a renal pelvic villous adenoma and diffuse intestinal metaplasia of the urothelium; no enteral communication or gastrointestinal pathologic features were found. Urothelial villous adenoma is extremely rare, and one should consider neoplastic etiologies, including

Psoas abscess: changing patterns of diagnosis and etiology.

From 1976 to 1984, 43 patients with psoas abscess were seen at the Mayo Clinic. Intestinal disease, including Crohn's disease, diverticulitis, and carcinoma, was the most frequent cause (14 patients). Eleven patients had osteomyelitis, five had postoperative complications, four had a foreign-body reaction, and three had a primary staphylococcal abscess. Two patients each had extension of a primary pancreatic and perinephric abscess. One patient had tuberculosis of the spine, and in the remaining patient, an exact cause was not determined. Definitive treatment of psoas abscess includes adequate

Renal tumor infarction with absolute ethanol.

Sixteen renal tumors were infarcted with intraarterial absolute ethanol. Renal tumor infarction was performed for one of three indications: for palliation of symptoms in patients with metastatic disease, as a preoperative measure to reduce blood loss during radical nephrectomy, or as a primary therapy in selected circumstances. In most cases, absolute ethanol was injected into the main renal artery through a balloon occlusion catheter. In all cases, ethanol produced effective infarction of the renal tumor. Only one complication developed, a perinephric abscess that developed several weeks afte

Predictors of clinically significant hemorrhage following ultrasound-guided percutaneous renal biopsy in pediatric patients.

BACKGROUND: Ultrasound-guided percutaneous renal biopsy is a cornerstone in the evaluation of pediatric kidney disease, with post-biopsy hemorrhage being the most common complication. Multiple predictors of post-biopsy bleeding have been identified in the adult population; however, data on risk factors in children is limited. OBJECTIVE: The purpose of our study is to determine any risk factors of post-biopsy hemorrhage in children. MATERIALS AND METHODS: This IRB-approved retrospective study included pediatric patients who had native or transplant renal biopsy between 2010-2024. The primary

The predictive ability of Mayo adhesive probability score for evaluating intraoperative bleeding in standard percutaneous nephrolithotomy in adult patients.

Mayo adhesive probability (MAP) score is one of the commonest tool to predict the adherence status of perirenal fat. The association between MAP score and intraoperative hemorrhage in patients undergoing micropercutaneous nephrolithotomy was comfirmed in a recent well designed study. We aimed to investigate if MAP score may predict the possibility for significant intraoperative bleeding in supine percutaneous nephrolithotomy (PCNL) performed with a 26 Fr rigid nephroscope. In this observational retrospective study, demographic, clinical, laboratory, radiological, perioperative (operation durat

A novel model using computed tomography parameters to predict shock wave lithotripsy success in ureteral stones at different locations.

OBJECTIVE: To combine non-contrast computerized tomography (NCCT)-based parameters with stone and patient characteristics that are already known to affect shock wave lithotripsy (SWL) success and assess this novel model's effectiveness in predicting SWL success for single ureteral stones in different locations. MATERIALS AND METHODS: Data of patients treated by SWL for a single ureteral stone between January 2017 and January 2019 were retrospectively reviewed. Demographic parameters of patients and stone characteristics were combined with NCCT-based parameters. NCCT-based parameters included

Abdominal involvement in Erdheim-Chester disease (ECD): MRI and CT imaging findings and their association with BRAFV600E mutation.

OBJECTIVES: To use magnetic resonance imaging (MRI) and computed tomography (CT) to define abdominal involvement in Erdheim-Chester disease (ECD), and to investigate the association between these findings and the BRAFV600E mutation. METHODS: This prospective study was performed on 61 ECD patients (46 men). The MRI and CT imaging studies were reviewed independently by two experienced radiologists. The association between BRAFV600E mutation and imaging findings was analysed using Fisher's exact test, and odds ratios with 95% confidence intervals. RESULTS: Perinephric infiltration was the most

Retroperitoneal Actinomycosis: A Rare Sequela of an Infected Obstructing Ureteral Stone.

Background: Actinomycosis is a condition in which Actinomyces, a normal component of the oral and gastrointenstial flora, becomes pathogenic in the setting of damaged tissue, leading to widespread tissue destruction across fascial planes. Prior literature describing this condition is rare, particularly cases involving the retroperitoneum. In this study, we report a case of retroperitoneal actinomycosis caused by an infected, obstructing ureteral stone. Case Presentation: A 48-year-old woman with a history of substance abuse, malnutrition, and gastric bypass presented to the emergency room with

Rupture of a Calyceal Diverticulum Secondary to Ureteroscopy: A Rare Complication.

We present a case of a 45-year-old female who experienced rupture of a right calyceal diverticulum caused by ureteroscopy. Fifteen hours after the operation, she had severe right flank pain and a high fever (38.9°C). Computed tomography revealed perinephric extravasation of urine and bleeding inside the diverticulum. We diagnosed rupture of a calyceal diverticulum; therefore, we continued antibiotic administration and pain relief medication. She became afebrile on postoperative day 4 and was discharged from the hospital on postoperative day 7. Owing to renal cortex thinning in the diverticula,

Triparametric ultrasound in differentiating multicystic renal masses: a rare presentation of unilateral focal renal lymphangioma.

We describe a rare case of renal lymphangioma presenting as a focal unilateral multicystic renal mass and document the first reported use of triparametric ultrasound (B-mode, Doppler, and contrast-enhanced ultrasound) in its diagnosis and discrimination from other focal multicystic lesions. Renal lymphangiomas are rare, benign, typically developmental lesions composed of cystic dilatation of the lymphatic ducts, usually occurring bilaterally as perinephric collections or parapelvic cysts mimicking hydronephrosis. Radiologists have an important role in suggesting the diagnosis, as clinical pres

Ureteral obstruction by prostate cancer leads to spontaneous ureteric rupture: a case report.

Spontaneous ureteric rupture (SUR) is an unusual entity associated with perinephric or retroperitoneal extravasation of urine. Patients with SUR are often presented with severe and progressive abdominal or flank pain. It is commonly related to the obstruction of genitourinary system, among which urinary calculi represents the most frequent cause. Prostate cancer with ureteral orifice invasion can lead to ureteral obstruction, which can also be a threat to SUR. Herein, we present a case of a 68-year-old male with SUR after prostate cancer invaded the left ureteral orifice. To our best knowledge

[Unilateral renal limphangiectasia: report of one case].

Renal lymphangiectasia is a rare disorder characterized by the presence of cystic masses in renal parenchyma, renal sinus or perinephric space. This condition, congenital or acquired, is probably caused by an alteration of renal lymphatic drainage to retroperitoneal lymph ducts. We report a 48-year-old woman, who consulted in the emergency room due to left flank pain and microscopic hematuria. Blood pressure and renal function were normal. A CT scan showed a multilocular cystic mass in the left renal sinus without alterations in the renal parenchyma. Renal lymphangiomatosis was diagnosed. The

Resolution of Page kidney-related hypertension with medical therapy: a case report.

A 24-year-old man developed systolic hypertension as a result of renal contusion, perinephric fluid collection, and renal compression (Page kidney) after blunt renal trauma. The patient was treated with an angiotensin-converting enzyme inhibitor for 30 days, after which his blood pressure normalized and the medications were discontinued. Follow-up clinical and laboratory examinations at 3, 6, and 12 months showed normal results. Transient hypertension may develop in patients after blunt renal injury as a result of Page phenomenon. Appropriate medical therapy is warranted and may be successful

Nephrobronchial fistula - a case report.

We report the case of a 58-year-old woman with a hydronephrotic left kidney who presented with a 4-month history of anorexia, weight loss and intermittent left loin pain associated with cloudy urine. Her urine grew lactose fermenting coliforms, and was treated with antibiotics. A computerized axial tomography scan (CT scan) was equivocal and she underwent retrograde ureteric stenting, which drained a pyonephrosis. She went on to develop a chest infection due to a lung abscess. A CT scan revealed a left perinephric collection extending across the diaphragm into the lower lobe of the left lung.
