Improving retinal vessel assessment precision by integrating deep learning with interactive editing and graphical modeling.

We present the SEoul Retinal Vessel Assessment Library (SERVAL), a novel software platform for precise quantitative measurement of vascular structures in fundus images. SERVAL integrates deep learning-based automatic artery and vein mask initialization, subpixel vessel centerline and boundary refinement, and interactive editing tools within a user-friendly graphical interface. From the refined artery and vein delineations, it enables accurate computation of a wide range of vessel assessment metrics, facilitating better characterization of complex vascular structures. We evaluate SERVAL through

Multimodal dynamic retinal vessel analysis offers new insights in microvascular defects in Fabry's disease.

Fabry's Disease (FD) is one of many disorders that result in altered vascular function. With the Dynamic Vessel Analyzer (DVA, IMEDOS Systems), retinal vessels can be recorded non-invasively in real time and dynamic responses to stimuli that affect vessel diameter can be directly visualized. The data obtained can be further mathematically evaluated in terms of dynamic time- and location-dependent vessel behavior. This principle of multimodal analysis of dynamic retinal vascular behavior has been applied to demonstrate specific structural and functional retinal microvascular changes in Fabry's

Validation of retinal oximetry vessel selection using fluorescein angiography in patients with optic disc drusen.

Retinal oximetry could provide insights into the pathophysiology of optic nerve disease, including optic disc drusen (ODD). Vessel selection for oximetry analysis is based on morphological characteristics of arterioles and venules and supported by an overlay of estimated blood oxygen saturations. The purpose of this cross-sectional study was to determine the validity of this vessel selection procedure by comparing it with vessel selection supported by video fluorescein angiography (FA). The study included 36 eyes of 36 patients with ODD who underwent retinal oximetry (Oxymap retinal oximeter T

Calculation of vessel pulse wave velocities in retinal vein segments within the optic disc centre.

The carotid-femoral pulse wave velocity (PWV) method is used clinically to determine degrees of stiffness and other indices of disease. It is believed PWV measurement in retinal vessels may allow early detection of diseases. In this paper we present a new non-invasive method for estimating PWVs in retinal vein segments close to the optic disc centre, based on the measurement of blood column pulsation in retinal veins (reflective of vessel wall pulsation), using modified photoplethysmography (PPG). An optic disc (OD) PPG video is acquired spanning three cardiac cycles for a fixed ophthalmodynam

Central retinal vessel trunk exit location: An important factor while evaluating the neuroretinal rim.

PURPOSE: Optic nerve head (ONH) evaluation plays a key role in differentiating normal from glaucomatous disk. Thinning of the inferior neuroretinal rim (NRR) has been noted in early glaucoma. However, NRR thickness in different quadrants appears to depend on various factors including central retinal vessel trunk exit (CRVT) position. We evaluated ocular parameters that determined the NRR thickness in the different quadrants of normal eyes. METHODS: Retrospective review of demographic and ocular data from 773 eyes of 388 subjects with normal ONH over one year was undertaken. RESULTS: Nearly 5

Altered retinal cerebral vessel oscillation frequencies in Alzheimer's disease compatible with impaired amyloid clearance.

Retinal vessels are similar to cerebral vessels in their structure and function. Moderately low oscillation frequencies of around 0.1 Hz have been reported as the driving force for paravascular drainage in gray matter in mice and are known as the frequencies of lymphatic vessels in humans. We aimed to elucidate whether retinal vessel oscillations are altered in Alzheimer's disease (AD) at the stage of dementia or mild cognitive impairment (MCI). Seventeen patients with mild-to-moderate dementia due to AD (ADD); 23 patients with MCI due to AD, and 18 cognitively healthy controls (HC) were exami

Rigidity of retinal vessel in untreated eyes of normal tension primary open-angle glaucoma patients.

BACKGROUND: To analyze pulse wave propagation in the ocular circulation and vessel stiffness in untreated eyes of normal tension primary open-angle glaucoma (NTG) patients. METHODS: Inferotemporal retinal vessels of 22 NTG eyes and 25 controls were examined with a Retinal Vessel Analyzer. Inferotemporal peripapillary retinal nerve fiber layer thickness was measured by ocular coherence tomography. Phase delay between venous trough and arterial peak was assessed at 3 sites centrifugal from the disc and a choroid-to-retina pulse delay was calculated as an estimation of vessel rigidity. RESULTS:

A vessel-guided multi-task deep learning framework with visual interpretability for simultaneous retinal vessel segmentation and multi-disease classification from fundus images.

INTRODUCTION: Retinal diseases represent a leading cause of visual impairment and blindness worldwide, with early and accurate diagnosis being crucial for preventing irreversible vision loss. Although deep learning techniques have achieved significant advances in fundus image analysis, existing methods predominantly focus on single tasks, treating vessel segmentation and disease diagnosis as independent problems without fully leveraging their intrinsic relationships. Furthermore, the lack of transparency in deep model predictions limits clinical adoption; while full interpretability remains an

Identification of CD157-Positive Vascular Endothelial Stem Cells in Mouse Retinal and Choroidal Vessels: Fluorescence-Activated Cell Sorting Analysis.

PURPOSE: CD157 (also known as Bst1) positive vascular endothelial stem cells (VESCs), which contribute to vascular regeneration, have been recently identified in mouse organs, including the retinas, brain, liver, lungs, heart, and skin. However, VESCs have not been identified in the choroid. The purpose of this study was to identify VESCs in choroidal vessels and to establish the protocol to isolate retinal and choroidal VESCs. METHODS: We established an efficient protocol to create single-cell suspensions from freshly isolated mouse retina and choroid by enzymatic digestion using dispase, co

Evaluation of Morphological Changes in Retinal Vessels in Type 1 Diabetes Mellitus Patients with the Use of Adaptive Optics.

Diabetes mellitus contributes to the development of microvascular complications in the eye. Moreover, it affects multiple end organs, including brain damage, leading to premature death. The use of adaptive optics technique allows to perform non-invasive in vivo assessment of retinal vessels and to identify changes in arterioles about 100 μm in diameter. The retinal vasculature may be a model of the cerebral vessels both morphologically and functionally. Aim. To evaluate morphological parameters of retinal arterioles in patients with type 1 diabetes mellitus (DM1). Material and methods. The stu

Movement of retinal vessels toward the optic nerve head after increasing intraocular pressure in monkey eyes with experimental glaucoma.

A shift or displacement of the retinal blood vessels (RBVs) with neuroretinal rim thinning indicates the progression of glaucomatous optic neuropathy. In chronic open angle glaucoma, individuals with RBV positional shifts exhibit more rapid visual field loss than those without RBV shifts. The retinal vessels reportedly move onto the optic nerve head (ONH) in response to glaucoma damage, suggesting that RBVs are pulled toward the ONH in response to increased cupping. Whether this phenomenon only applies to RVBs located in the vicinity or inside the ONH or, more generally, to RBVs also located f

Relationship between oxygen saturation of the retinal vessels and visual field defect in glaucoma patients: comparison with each hemifield.

PURPOSE: This study aimed to elucidate the relationship between visual field defects in the upper and lower hemifields and the corresponding oxygen saturation of the retinal vessels in patients with glaucoma. METHODS: Patients with glaucoma (n = 44) exhibiting more than a 10 dB difference between the upper and lower total deviation (TD) were enrolled in the study. After measuring the retinal vessel oxygen saturation by a non-invasive spectrophotometric retinal oximeter, the hemifields in one eye of each patient were divided into worse (worse TD) and better (better TD) hemifield areas. We addi

Defective myogenic response to posture change in retinal vessels of well-controlled type 1 diabetic patients with no retinopathy.

PURPOSE: The current approach to the prevention of diabetic retinopathy relies on intensive anti-diabetes treatment and is only partially successful. A marker of retinopathy risk would enable strategies of surveillance, screening of adjunct drugs, and targeted drug interventions. The authors sought to identify early abnormalities of retinal vessels that are not prevented by the current therapeutic approach. METHODS: Retinal thickness (an informer of vascular permeability) and hemodynamic parameters at baseline and longitudinally were measured in 27 subjects (age, 32 ± 9 years [mean ± SD]) wit

Retinal Optical Coherence Tomography-Angiography and Longitudinal Changes in Cognition and Cerebral Small Vessel Disease.

BACKGROUND AND OBJECTIVES: Cross-sectional studies suggest that retinal microvascular abnormalities measured by optical coherence tomography angiography (OCTA) are associated with cerebral small vessel disease (cSVD) and cognitive impairment, but longitudinal relationships remain unclear. We investigated whether OCTA measures are associated with brain cSVD markers and cognition over time and whether baseline OCTA was associated with brain and cognitive outcomes after mild ischemic stroke. METHODS: We conducted a prospective longitudinal cohort study recruiting patients with lacunar or mild co

Robust semi-automatic vessel tracing in the human retinal image by an instance segmentation neural network.

Vasculature morphology and hierarchy are essential for blood perfusion. Human retinal circulation is an intricate vascular system emerging and remerging at the optic nerve head (ONH). Tracing retinal vascular branching from ONH can allow detailed morphological quantification, and yet remains a challenging task. We presented a robust semi-automatic vessel tracing algorithm on human fundus images by an instance segmentation neural network (InSegNN). InSegNN separates and labels individual vascular trees and enables tracing each tree throughout its branching. We have three strategies to improve r
