BACKGROUND: The purpose of this study was to determine patients' preoperative expectations before primary elective reverse shoulder arthroplasty (RSA). We hypothesized that younger patients, patients with better preoperative function, patients with shoulder osteoarthritis (OA), and patients with no prior joint replacements would have higher expectations of RSA.
METHODS: We prospectively studied 333 primary RSAs performed for cuff tear arthropathy (n = 242), OA (n = 68), or post-traumatic arthritis (n = 23). Expectations were assessed preoperatively using the Hospital for Special Surgery's sho

BACKGROUND: Arm elevation is composed of glenohumeral and scapulothoracic motion. Many reports have addressed changes of scapular position across a spectrum of shoulder disease. However, no study has examined changes in scapular position after reverse total shoulder arthroplasty (RTSA). The purpose of this study was to evaluate the changes in scapular position after RTSA compared to patients' contralateral, nonoperated shoulder.
METHODS: Seven patients that underwent RTSA for cuff tear arthropathy from July 2007 to October 2008 were enrolled. The distance between the long axis of the thoracic

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) has continued to increase in clinical utility and popularity as an effective treatment for cuff tear arthropathy (CTA), irreparable rotator cuff tears (RCTs), osteoarthritis, and acute 3- and 4-part proximal humeral fractures. Performing RTSA for acute proximal humeral fractures presents the unique challenges of tuberosity management, bone loss, and instability compared with elective indications such as CTA or irreparable RCTs. The purpose of this study was to compare the clinical outcomes, active range of motion (ROM), radiographic outcom

BACKGROUND: Reverse shoulder arthroplasty (RSA) offers promising functional outcomes for young patients, yet challenges persist in restoring internal rotation (IR). This study aimed to assess the restoration of IR after RSA in patients younger than 60 years of age and analyze the factors affecting IR recovery.
METHODS: A retrospective multicenter study was conducted, examining the functional outcome of patients who underwent RSA, with a minimum follow-up period of 2 years. Two subgroups of patients who underwent primary RSA were analyzed separately with respect to active internal rotation wit

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is an established treatment for cuff tear arthropathy, irreparable rotator cuff tears, and advanced glenohumeral osteoarthritis. However, despite advances in implant design and preoperative planning, rTSA remains technically demanding during early experience, and the influence of assistant experience on learning curves is unclear.
METHODS: This retrospective study evaluated the early learning curves of 3 shoulder surgeons (Surgeons A-C), each assessed over their first 60 consecutive rTSA cases. All surgeons shared similar subspecialty tra

BACKGROUND: External beam radiation therapy (XRT) is a commonly used therapeutic modality for the treatment of various chest wall and axillary malignancies. Despite the known risk of local soft tissue dysfunction, and possibly compromised bone ingrowth for cementless implants, there remains limited data on the impact of prior XRT in a shoulder arthroplasty (SA) cohort. This study evaluated the outcomes of primary SA in patients with prior XRT compared to a matched cohort (MC).
METHODS: Over a 27-year time period (1993-2020), 80 primary SAs (7 hemiarthroplasties [HAs], 29 anatomic total should

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is growing in popularity due to advancements in implant design, improvements in surgical techniques, and expanded indications. Hesitancy to perform rTSA in younger patients is due to concerns regarding implant longevity and functional outcomes. This study investigates outcomes of rTSA in patients ≤65 years old in the largest known cohort to date.
METHODS: Patients who underwent rTSA by 3 surgeons (C.M.J., M.L.K., W.N.L.) from 2014 to 2025 were retrospectively identified. Exclusion criteria were >65 years old, prior rTSA, and <12 months fo

BACKGROUND: Avascular necrosis (AVN) of the humeral head frequently results in humeral head collapse and end-stage arthritic changes of the glenohumeral joint. Despite the recent proliferation of reverse total shoulder arthroplasty (RTSA), reports on the use of RTSA for AVN remain limited. The purpose of this study was to document the outcomes of shoulders indicated for RTSA in the setting of humeral head AVN and compare these with AVN shoulders indicated for the gold standard, anatomic total shoulder arthroplasty (aTSA).

BACKGROUND: As the use of reverse total shoulder arthroplasty (rTSA) has steadily increased since Food and Drug Administration approval in 2003, there have been a number of unique complications recognized, including postoperative acromial stress fractures (ASFs). The incidence of ASF after rTSA is rare; however, the impact on clinical outcomes and patient satisfaction can be devastating. Despite a growing body of literature exploring risk factors for ASF following rTSA, there has been minimal investigation into prior subacromial decompression/acromioplasty (SAD) as a risk factor. The purpose o

BACKGROUND: It is common for patients to require staged bilateral shoulder arthroplasties. There is a unique cohort of patients who require an anatomic total shoulder arthroplasty (TSA) and a contralateral reverse shoulder arthroplasty (RSA). This study compared the outcomes of patients with a TSA in 1 shoulder and an RSA in the contralateral shoulder.
METHODS: Our institutional database was queried to identify all patients with a TSA and a contralateral RSA. Data collection included patient demographics, preoperative and latest follow-up shoulder range of motion, radiographic analysis, and p

BACKGROUND: Although most patients undergoing reverse total shoulder arthroplasty (rTSA) have substantial improvement in pain and function at early follow-up, improvements in pain and range of motion progress more slowly during postoperative rehabilitation in a subset of patients. The purpose of this study was to define a patient's risk of persistent shoulder dysfunction beyond the early postoperative period and identify risk factors for persistent poor performance.
METHODS: We retrospectively reviewed 292 primary rTSAs with early poor performance and a preoperative diagnosis of osteoarthriti

INTRODUCTION: Reverse total shoulder arthroplasty (rTSA) has become a widely accepted treatment for rotator cuff tear arthropathy. Recent studies have highlighted the importance of the subscapularis (SSC) tendon in maintaining postoperative stability and optimizing functional outcomes after rTSA. When SSC repair is not feasible or is associated with a high risk of failure, pectoralis major (PM) tendon transfer has been proposed as an alternative strategy to restore internal rotation (IR) function and stability. However, no comparative studies have directly evaluated whether PM tendon transfer

BACKGROUND: The last few years have been characterized by increasing numbers of reverse shoulder arthroplasties. In addition to the classic indication of cuff tear arthropathy, the use for complex proximal humeral fractures (PHFs) and rotator cuff tear (RCT) in very old patients have been established. The objective of this study is to clarify and substantiate the above statements specifically for Germany (based on official data from 2010 to 2022). Since substantial changes in the structure of the population are expected over time, all data must be adjusted for these changes. The hypotheses are

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) provides reliable outcomes for irreparable cuff tears and arthropathy, although most data are Western populations. Asian patients have smaller physiques, lower bone density, and greater internal rotation requirements, which may affect outcomes. Clinician-based scores (eg, Japanese Orthopedic Association [JOA] score) remain standard; however, patient-reported outcome measures (PROMs), including the American Shoulder and Elbow Surgeons (ASES) score, are increasingly adopted worldwide. Whether PROMs reflect recovery or replace clinician evalu
