BACKGROUND: Burn wound excision is crucial for infection control in burn patients. Based on previous studies, the Japanese Society of Burn Injuries recommended that the surgical debridement be completed within one week after injury for patients with a burned area of 30 % or more. However, there have been no reports examining the significance of completing resection of the burn wound within one week. This study was aimed to clarify the importance of completing burn excision within one week after injury for 30 % or more in flame burn patients.
METHODS: Data from 66 flame burn patients who under

INTRODUCTION: While generally reducing morbidity and mortality, electrical weapons have risks associated with their usage, including eye injuries and falls. With the presence of explosive fumes or fuels there also exists the possibility of burn injury.
METHODS: We searched for cases of fatal and non-fatal major burns with TASER® electrical weapon usage where there was a possibility that the weapon ignited the explosion.
RESULTS: We confirmed 6 cases of fatal burn injury and 4 cases of major non-fatal burns out of 3.17 million field uses. The mean age was 35.5 ± 9.7 years which is consistent

BACKGROUND: Massive fluid shift after severe burn injury leads to edema and intravascular fluid loss that may result in burn-induced compartment syndrome (BICS) when corrected by aggressive fluid resuscitation. Factors causing this fluid shift remain unclear. Because glycocalyx regulates endothelial permeability, we hypothesized that glycocalyx shedding would increase fluid requirements in burn patients. This prospective cohort study aimed to identify relationships between shedding of the glycocalyx and fluid requirements after burn injury.
METHODS: Patients aged more than 18 years with burn

OBJECTIVE: Strictures of the upper airway caused by burns have features distinct from other benign stenoses. The authors reviewed their experience with burn-related stenoses to define the principles of treatment.
SUMMARY BACKGROUND DATA: The combined effects of inhaled gases and heat in burn victims produce an intense, often transmural, inflammation of the airway, further complicated by intubation. The incidence of laryngotracheal strictures in survivors of inhalation injury is high, but the reported experience with their treatment is limited and often unduly separated into injuries of larynx

INTRODUCTION: Burn injuries are associated with high mortality and morbidity, especially in the elderly population. Although burns are preventable, they account for the fourth most common cause of trauma worldwide. The majority of the mortality associated with burn victims is also seen in the elderly age group. Most mortality predictor scores focus on age and total body surface area (TBSA) burned. However, data focusing specifically on elderly populations with respect to TBSA, particularly within the Appalachian region, remain limited. This rough terrain is accompanied by multiple challenges a

The New York City Fire Department (FDNY) is the largest fire department in the United States, with over 11,000 firefighters. In 1994, FDNY changed to a modern firefighting protective uniform. The major difference between traditional and modern uniforms is that modern uniforms include both protective over-coat and over-pant, whereas traditional uniforms include only the over-coat. Furthermore, modern uniforms are manufactured using improved thermal protective textiles that meet or exceed current National Fire Protection Association standards for structural firefighting. The purpose of this stud

OBJECTIVE: To characterize serious inhalation injuries seen during recent military operations, and assess whether bronchoscopic severity findings were associated with clinical presentation and outcomes.
METHODS: Service members who suffered inhalation injuries while deployed to Iraq, Afghanistan, or Syria from 2001-2018 were identified using ICD-9 and 10 codes from the Expeditionary Medical Encounter Database (EMED), which is abstracted from patient records in forward-deployed medical facilities. Further information including demographics, mechanism of injury, mortality, total burn surface ar

INTRODUCTION: Silver nitrate pencil is often used to treat local granuloma caused by tracheotomy and tracheostomy cannula orifice.
CASE REPORT: We report the case of a 69-year-old patient who accidentally inhaled silver nitrate lead from the tip of a pencil during treatment of local granuloma. Inhalation of this product, which is known to cause burns and a risk of perforation of the mucous membranes, could suggest locoregional complications. Clinical monitoring and radiological and endoscopic examinations were carried out. Antibiotics and corticosteroids were administered because of inflammat

BACKGROUND: The present study was conducted to examine topic of issuing early do-not-resuscitate (DNR) order at first diagnosis of patients with severe burn injuries in light of current law in Turkey and the medical literature. DNR requires withholding cardiopulmonary resuscitation in event of respiratory or cardiac arrest and allowing natural death to occur. It is frequently enacted for terminal cancer patients and elderly patients with irreversible neurological disorders.
METHODS: Between January 2009 and December 2014, 29 patients (3.44%) with very severe burns were admitted to burn unit.

BACKGROUND: Electrical injuries comprise a minority of burn center admissions but are associated with significant morbidity and mortality. This is a case of a patient who suffered high-voltage electrical injury who survived despite developing several sequalae, who had an unusual presentation of inhalation injury complicated by the aspiration of metal screws.
CASE PRESENTATION: This is a 20-year-old male who suffered electrical contact injury, and 45.5% total body surface area (TBSA) burns from electrothermal discharge and subsequent ignition of clothing, whose hospital course was complicated

BACKGROUND: An airway-associated fire in an operating room can have devastating consequences for patients. Breathing circuit warmers (BCWs) are widely used to provide heated and humidified anesthetic gases and eventually prevent hypothermia during general anesthesia. Herein, we describe a case of a BCW-related airway fire.
CASE PRESENTATION: In this case, an electrical short within a BCW wire caused a fire inside the circuit. Simultaneously, the fire was extinguished, ventilation was stopped, and the endotracheal tube was disconnected from the BCW. The patient was exposed to the fire for less

BACKGROUND: There is a common, well-known and established recommendation to excise burn wounds within 24-72 h in order to mitigate the systemic inflammatory and immunomodulatory response, shorten length of hospitalization through early grafting and optimize patient survival. Despite this apparent consensus, surprisingly few systematic studies have evaluated the actual adherence to this practice and its implications on patient outcomes. In this registry study, we sought to objectify the current status of early burn wound excision, its influencing factors and impact on patient outcomes for all G

INTRODUCTION: A burn injury is a severe global healthcare burden and is associated with high mortality risk. The mortality rate is marked, particularly in low-and-middle-income countries. Despite its clinical importance, recent epidemiological data on burn injury and mortality are limited in the area of critical care units in Ethiopia. Therefore, this study aimed to determine the survival status and predictors of mortality among burn patients admitted to intensive care units. MATERIAL AND METHODS: This multicentre retrospective cohort study was conducted at Addis Ababa governmental hospitals i

BACKGROUND: Burn injuries may be complex, life-changing and life-threatening. Knowing the factors that contribute to unfavourable outcomes, particularly mortality, is crucial so that they can be assessed and addressed. Well-recognised contributors to adverse outcomes in burn patients include age, burn size, revised Baux score, and inhalation injury. Less well described, but increasingly reported, is frailty status. Here we aim to expand the current literature by adjusting for confounders not previously investigated. This is important particularly with an increasing emphasis on frailty assessme

Psittacine birds can develop severe and often fatal pneumonitis when exposed to various noxious inhalants. Toxicosis in these birds caused by inhalation of pyrolysis products produced from overheated polytetrafluoroethylene-coated cooking pans on stove tops is well known, but compounds emitted from burned foods and other materials can also be toxic. We present a case of fatal pneumonitis in 10 psittacine birds associated with the operation of an oven in the self-cleaning mode.

Inhalation injury, a major contributor to burn-related mortality, has been difficult to quantify. A scoring system paralleling current adult respiratory distress syndrome systems has the potential to distinguish survivors from nonsurvivors. The utility of the PaO2/FiO2 (P/F) ratio in predicting injury severity was first examined. In a review of 120 patients with inhalation injury, those with P/F ratios greater than or equal to 300 after resuscitation were more likely to survive than those with ratios less than 300. The P/F ratio had no value when obtained before resuscitation. Next, a scoring

In burn patients, vitamin D deficiency has been associated with increased incidence of sepsis and infectious complications. The objective of this study was to assess the impact of vitamin D deficiency in adult burn patients on hospital length of stay (LOS). This was a multicenter retrospective study of adult patients at 7 burn centers admitted over a 3.5-year period, who had a 25-hydroxyvitamin D concentration drawn within the first 7 days of injury. Of 1147 patients screened, 412 were included. Fifty-seven percent were vitamin D deficient. Patients with vitamin D deficiency had longer LOS (18

We report here detection of helium in specimens derived from a burn autopsy case. A male was found in a burnt bedroom. Part of a heat-denatured plastic bag, sealing tape, and flexible tubing remained on his head and neck. In addition, five helium tanks were found near him. His history in conjunction with the discovery conditions suggested a suicide attempt by inhalation of helium. The body had extensive first to fourth degree burns caused by heat. A small amount of soot was deposited in the respiratory tract. Except for the thermal burns, no other injuries were found. Toxicologically, the bloo
