Presbycusis, or sensorineural hearing loss in the elderly population, affects approximately 40% to 50% of people over the age of 75. A variety of devices are available to those with hearing loss. Cochlear implants, for example, are especially useful for those with severe-to-profound hearing loss. The population is aging, so the demand for cochlear implantation in ambulatory surgery centers will likely increase. Ambulatory surgery centers (ASC) can provide a more convenient and less expensive location for cochlear implant surgery than hospital-based operating facilities. Patient selection using

IMPORTANCE: Histories of cochlear implant (CI) technology have often been inaccurate owing to the confusion of terms and anatomical situations or to biased reporting. This retrospective, published shortly after the death of inventor William F. House-and more than 50 years after placement of the first CI-offers a precise account of the early experimental period.
OBJECTIVE: To clarify the first steps in the development of the CI, ie, an electrical stimulating device partially inserted into the cochlea.
EVIDENCE REVIEW: Literature review based on published data, oral history material, interview

PURPOSE OF REVIEW: As the current trend in cochlear implantation is to prescribe cochlear implants for patients with residual hearing and to use electroacoustic stimulation, cochlear implant damage must be prevented. This article summarizes current research endeavors to prevent electrode insertion trauma and resulting hearing loss.
RECENT FINDINGS: Alteration in surgical technique is necessary with each new electrode design. Nontraumatic surgical technique also requires minimizing acoustic trauma due to drilling the cochleostomy, mechanical damage from electrode insertion, potential infection

There are an increasing number of young children undergoing cochlear implantation. Cholesteatoma is a serious, although rare, long-term complication. We present the only documented case in which cholesteatoma has presented with late abscess formation in the bed of a cochlear implant package of a child. Cholesteatoma may be an iatrogenic complication of implantation surgery. Damage to the bony part of the external auditory canal should be avoided. The successful two-staged surgical treatment using explantation, debridement, gentamycin bead insertion, cholesteatoma excision and modified radical

Expanding selection criteria and increasing numbers of cochlear implantations have revealed several challenging cases with regard to surgery and medical point of view. Cochlear implantation of congenitally deaf children with inner-ear malformations may involve difficulties in preoperative evaluation, surgical approach and postoperative follow-up. Cochlear nerve aplasia (as evidence of only one existing nerve in three dimensional magnetic resonance imaging) and hypoplastic cochleas are among the most difficult cases and sometimes children are considered unsuitable for cochlear implantation. How

Cochlear implants are electronic auditory prostheses used to rehabilitate deafened persons who have lost their hair cells. They are partly worn externally and partly implanted in the ear. They provide a direct stimulation of the spiral ganglion cells of the cochlear nerve by bypassing the destroyed hair cells. The objectives of this article are to summarise what head and neck surgeons need to know before cochlear implantation and to describe the imaging study protocol used and anomalies to look for. A few explanations are resumed about placement of a brainstem implant.

The recipient of a Nucleus 22 multichannel cochlear implant began to experience severe vestibular stimulation related to the implant. This patient's experience initiated a study with the objective of determining the frequency of implant-related vestibulo-ocular stimulation. Subjects consisted of 17 randomly selected patients who use cochlear implants. Included in the study were 14 Nucleus 22 and three Med-El Combi 40-devices. Stimulation of the implants was performed both by individual channel and with sound field broad-band 80-dB noise using the users' normal device settings. Eye movements we

During the last decade, the technology of assistive hearing devices for hearing-impaired children has improved significantly. Improvements in hearing aids and the introduction of cochlear implants and wearable tactile aids for children have made spoken language available to more deaf children and have changed the techniques for teaching listening and speaking skills. In June 1990, the Food and Drug Administration approved the Nucleus 22 channel cochlear implant for use with children. As a result, it is expected that more children will be implanted and, consequently, that the teachers and audio

The insertion of reliable extracochlear single-canal cochlear implants in cophotic and subcophotic patients allows the electrical stimulation of the acoustic nerve for rehabilitation, as well as audiovestibular explorations that are no longer possible in the acoustic mode. The authors therefore aim at replacing all these examinations with their Electrical equivalent: --early evoked potentials (E-BERA), --medium-latency evoked potentials (E-MLR), --Event-related evoked potentials (E-P300), for which, to our knowledge, theses are the first recordings, --electrical stapedial reflex, --vestibular

The speech production of three post-lingually deafened adults, who derived no benefit from wearing hearing aids, was assessed. Individual therapy programmes were devised based on these assessments and the subjects participated in a course of speech therapy. They then received cochlear implants and had a further course of speech therapy using their implants. Changes in speech production were judged by trained listeners, who heard tape-recorded samples of the subjects' speech. These samples were taken at various intervals before and after therapy alone, and with the cochlear implant. Significant

Intraoperative measurements are valuable tools in cochlear implant (CI) surgery, enabling the determination of implant functionality while the patient is still in the operating room. Standard measurements include impedances for stimulating and non-stimulating contacts, triggering of the stapedial reflex with or without estimating the threshold, and assessing cochlear nerve function by measuring electrically evoked compound action potentials. Additional measurements can also be performed. For example, electrocochleography can be used to observe and help preserve residual hearing in patients. Us

INTRODUCTION: Cochlear implant manufacturers currently contraindicate the use of electroconvulsive therapy (ECT) in CI users, citing theoretical evidence of potential harm to the patient or the implant despite a lack of clinical data. We report two uncomplicated cases of ECT in CI users, including the first reported case of bilateral ECT in a patient with bilateral CIs.
CASE SERIES: The first case involves a 66-year-old visually impaired male with bilateral CIs. He suffered from major depressive disorder complicated by refusal of oral intake despite maximal pharmacological therapy. He underwe

Cochlear implant (CI) systems differ in terms of electrode design and signal processing. It is likely that patients fit with different implant systems will experience different percepts when presented speech via their implant. The sound quality of speech can be evaluated by asking single-sided-deaf (SSD) listeners fit with a cochlear implant (CI) to modify clean signals presented to their typically hearing ear to match the sound quality of signals presented to their CI ear. In this paper, we describe very close matches to CI sound quality, i.e., similarity ratings of 9.5 to 10 on a 10-point sc

INTRODUCTION: Cochlear implants (CIs) provide access to sound for children and adults who do not receive adequate benefit from hearing aids. Age at implantation is known to affect outcomes across the lifespan.
AREAS COVERED: The effects of age on CI outcomes are examined for infants, children, adolescents, and older adults. A variety of outcome measures are considered, including speech perception, language, cognition, and quality of life measures.
EXPERT OPINION/COMMENTARY: For those meeting candidacy criteria, CIs are beneficial at any age. In general, younger age is related to greater bene

BACKGROUND: The aim of this study was to find out how candidacy criteria have evolved differently across the globe.
METHODS: Candidacy criteria and outcome measurements applied in 19 HEARRING clinics were analyzed.
RESULTS: Candidacy criteria vary between clinics. Overall, both bilateral implantation and cochlear implantation in patients with single-sided deafness are becoming more frequent.
CONCLUSION: Standardized outcome measurement instruments need to be applied to provide access to the hearing world to all patients with hearing loss who would benefit from cochlear implantation.
