Recent advances in understanding NRF2 as a druggable target: development of pro-electrophilic and non-covalent NRF2 activators to overcome systemic side effects of electrophilic drugs like dimethyl fumarate.

Dimethyl fumarate (DMF) is an electrophilic compound previously called BG-12 and marketed under the name Tecfidera ®. It was approved in 2013 by the US Food and Drug Administration and the European Medicines Agency for the treatment of relapsing multiple sclerosis. One mechanism of action of DMF is stimulation of the nuclear factor erythroid 2-related factor 2 (NRF2) transcriptional pathway that induces anti-oxidant and anti-inflammatory phase II enzymes to prevent chronic neurodegeneration. However, electrophiles such as DMF also produce severe systemic side effects, in part due to non-specif

Dimethyl fumarate ameliorates erectile dysfunction in bilateral cavernous nerve injury rats by inhibiting oxidative stress and NLRP3 inflammasome-mediated pyroptosis of nerve via activation of Nrf2/HO-1 signaling pathway.

OBJECTIVE: To investigate the therapeutic potential of dimethyl fumarate (DMF) in improving erectile function of bilateral cavernous nerve injury (BCNI) rats, along with elucidating its underlying mechanisms. METHODS: A BCNI rat model was established by clamping bilateral cavernous nerve (CN). DMF was given by gavage at low (20 mg/kg/day) and high (40 mg/kg/day) dosages for a duration of 4 weeks. Erectile function was assessed by electrical stimulation of CN. Penis and CN tissues were collected for subsequent analysis. Additionally, PC-12 cell line was used to verify the mechanism of DMF in v

Varicella-Zoster Reactivation in a Non-immunized Elderly Multiple Sclerosis Patient While on Delayed-Release Dimethyl Fumarate With Grade 2 Lymphopenia and Profoundly Low CD4+ and CD8+ Cell Counts: A Case Report.

Increased susceptibility to opportunistic infections (OI) in multiple sclerosis (MS) patients is a real concern amongst neurologists when using disease-modifying therapies (DMTs). DMTs used in modulating or suppressing the immune system for MS management may risk the patient with lymphocytopenia, raising the possibility of OI; however, this lymphopenia may contemplate as a biomarker for drug response, degree of immunomodulation, and drug compliance. The OI could be reactivation of varicella-zoster, progressive multifocal leukoencephalopathy (PML) induced by John Cunningham virus (JC virus), Pn

Eosinophilic fasciitis-like disorder developing in the setting of multiple sclerosis therapy.

IMPORTANCE: Dimethyl fumarate received FDA approval in March 2013 for treatment of multiple sclerosis and has had a rapid uptake in the field due in large part to a favorable safety profile. Side effects of dimethyl fumarate include flushing, gastrointestinal discomfort, and peripheral eosinophilia. We report a case of eosinophilic fasciitis-like disorder occurring in the setting of oral dimethyl fumarate therapy. Eosinophilic fasciitis is rare and may be related to the peripheral eosinophilia known to occur with this medication. OBSERVATIONS: We present a case of a 36-year-old male treated w

Astaxanthin and meclizine extend lifespan in UM-HET3 male mice; fisetin, SG1002 (hydrogen sulfide donor), dimethyl fumarate, mycophenolic acid, and 4-phenylbutyrate do not significantly affect lifespan in either sex at the doses and schedules used.

In genetically heterogeneous (UM-HET3) mice produced by the CByB6F1 × C3D2F1 cross, the Nrf2 activator astaxanthin (Asta) extended the median male lifespan by 12% (p = 0.003, log-rank test), while meclizine (Mec), an mTORC1 inhibitor, extended the male lifespan by 8% (p = 0.03). Asta was fed at 1840 ± 520 (9) ppm and Mec at 544 ± 48 (9) ppm, stated as mean ± SE (n) of independent diet preparations. Both were started at 12 months of age. The 90th percentile lifespan for both treatments was extended in absolute value by 6% in males, but neither was significant by the Wang-Allison test. Five othe

Industry Payments and Prescribing of Brand-Name Multiple Sclerosis Medications in Medicare.

OBJECTIVES: Industry payments to clinicians influence prescribing, limiting use of lower-cost generics, and leading to nonadherence. This study of prescribing for multiple sclerosis examined whether payments from brand-name drug manufacturers were associated with prescribing of brand-name glatiramer and dimethyl fumarate after generics became available. METHODS: This cross-sectional study used 2021-2022 Open Payments data and 2022-2023 Medicare Part D prescription data. Payments from Teva (glatiramer) and Biogen (dimethyl fumarate) were categorized as none, <$1,000, or ≥$1,000. The outcome wa

[Comments on the GLIMPSE study on evaluating the efficacy of the drug cladribine in tablets in routine clinical practice in comparison with other tablet drugs for the pathogenetic treatment of multiple sclerosis].

This study analyzes data from the open clinical trial GLIMPSE comparing the efficacy of cladribine in tablets (n=633) in comparison with fingolimod (n=1195), dimethyl fumarate (n=912), and teriflunomide (n=735) in remitting multiple sclerosis (MS) in real clinical practice. Cladribine tablets have demonstrated significantly higher efficacy compared to fingolimod, teriflunomides and dimethyl fumarate. This indicates the need for more active use of cladribine in tablets with active forms of MS.

[Low-dose, long-term fumaric acid esters in recalcitrant cutaneous sarcoidosis : Report of two cases].

Fumaric acid esters lead to reduction of cell fusion and inhibition of giant cell formation in vitro, which is considered to be a reason for their therapeutic effect on cutaneous granulomatous diseases. We have reported that successful treatment of the skin lesions of a patient with recalcitrant systemic sarcoidosis with Fumaderm® (Biogen, Munich, Germany; 360-720 mg/day dimethyl fumarate) had to be discontinued due to the development of lymphocytopenia. Therefore, we treated two further patients with the low-dose Fumaderm® Initial (90 mg/day dimethyl fumarate) and observed a partial remission

Effects of disease modifying therapy on cognitive proficiency in pediatric-onset multiple sclerosis (POMS).

BACKGROUND: Disease-modifying therapies (DMTs) may mitigate well-documented cognitive challenges in pediatric-onset multiple sclerosis (POMS) by slowing disease progression and promoting production of neurotrophic factors, though studies are limited, especially in children. This study assessed cognitive proficiency in patients with POMS treated with DMTs. METHOD: The sample (n = 26; x̄ age at diagnosis = 14.6 years) was treated with dimethyl fumarate, natalizumab, or B-cell depletion therapies. Neuropsychological outcomes included attention/working memory (WM) and processing speed (PS) as mea

Unexpected exacerbations following initiation of disease-modifying drugs in neuromyelitis optica spectrum disorder: Which factor is responsible, anti-aquaporin 4 antibodies, B cells, Th1 cells, Th2 cells, Th17 cells, or others?

Some disease-modifying drugs for multiple sclerosis, which mainly act on T cells, are ineffective for neuromyelitis optica spectrum disorder and induce unexpected relapses. These include interferon beta, glatiramer acetate, fingolimod, natalizumab, and alemtuzumab. The cases reported here suggest that dimethyl fumarate, which reduces the number of Th1 and Th17 cells and induces IL-4-producing Th2 cells, is also unsuitable for neuromyelitis optica spectrum disorder, irrespective of anti-aquaporin 4 IgG serostatus. Although oral dimethyl fumarate with manageable adverse effects is easy to initia

Fumarate and fumarate hydratase: an immunometabolite regulator of inflammation and diseases.

Once regarded solely as an intermediate of the tricarboxylic acid (TCA) cycle involved in energy production, fumarate has now emerged as a pivotal immunometabolite with far-reaching effects on inflammatory signaling and immune cell fate. This review comprehensively delineates the dual nature of fumarate, which functions as a context-dependent rheostat of inflammation. Intracellular fumarate levels are tightly regulated by enzymatic activity, transport systems, and exogenous sources, including the pharmacological agent dimethyl fumarate (DMF). Fumarate can covalently modify critical cysteine re

DMF, but not other fumarates, inhibits NF-κB activity in vitro in an Nrf2-independent manner.

Fumarate-containing pharmaceuticals are potent therapeutic agents that influence multiple cellular pathways. Despite proven clinical efficacy, there is a significant lack of data that directly defines the molecular mechanisms of action of related, yet distinct fumarate compounds. We systematically compared the impact of dimethyl fumarate (DMF), monomethyl fumarate (MMF) and a mixture of monoethyl fumarate salts (Ca(++), Mg(++), Zn(++); MEF) on defined cellular responses. We demonstrate that DMF inhibited NF-κB-driven cytokine production and nuclear translocation of p65 and p52 in an Nrf2-indep
